Our aim is to summarize the new European Resuscitation Council (ERC) 2021 guidelines on paediatric life support. In children, exhaustion of compensatory mechanisms in respiratory or circulatory failure leads to cardiac arrest. Recognition and treatment of children in critical condition are the most important element of its prevention. With the ABCDE approach, life-threatening problems can be identified and treated using simple interventions (bag-mask ventilation, intraosseous access, fluid bolus, etc.). Important new recommendations: 4-hand ventilation during bag-mask ventilation, target satur

BACKGROUND: Pediatric magnetic resonance imaging (MRI) and computed tompgraphy (CT) require patient immobility and therefore often require sedation or general anesthesia of patients. Consensus on these procedures is lacking in France.
OBJECTIVE: Thus, the aim of this study was to describe the current sedation practices for pediatric MRI and CT in France.
MATERIAL AND METHODS: From January 2019 to December 2019, an online questionnaire was delivered by electronic mail to a representative radiologist in 60 pediatric radiology centers registered by the French-speaking pediatric and prenatal ima

High dietary sodium impairs cerebral blood flow regulation in rodents and is associated with increased stroke risk in humans. However, the effects of multiple days of high dietary sodium on cerebral blood flow regulation in humans is unknown. Therefore, the purpose of this study was to determine whether ten days of high dietary sodium impairs cerebral blood flow regulation. Ten participants (3F/7M; age: 30 ± 10 years; blood pressure (BP): 113 ± 8/62 ± 9 mmHg) participated in this randomized, cross-over design study. Participants were placed on 10-day diets that included either low- (1000 mg/d)

Studies have shown that up to 63% of pediatric intensive care unit patients admitted with acute respiratory or cardiorespiratory illness require mechanical ventilation. Mechanical ventilator support can be divided into three phases: initiation, escalation, and resolution. Noninvasive ventilation is typical during the initiation phase in the management of acute pediatric respiratory failure. The major advancements in the use of noninvasive ventilation involve the emergence of high-flow nasal cannula and how widespread the use of high-flow nasal cannula has become in pediatric critical care prac

PURPOSE: Severe manifestations of facioscapulohumeral dystrophy (FSHD) may be associated with sleep-disordered breathing (SDB), including obstructive sleep apnea (OSA) and nocturnal hypoventilation (NH), but prevalence data are scarce. In patients with respiratory muscle weakness, detection of NH can be facilitated by transcutaneous capnometry, but respective data derived from FSHD patients have not yet been published.
METHODS: We collected sleep studies and capnometry recordings from 31 adult patients with genetically confirmed FSHD who were admitted to our sleep laboratory for first-ever ev

BACKGROUND: Hypoxia is a very common adverse event that occurs during gastrointestinal endoscopy under sedation, especially in older patients, owing to limited reservation of heart, brain, lung, and other organs. Prolonged or severe hypoxia can cause ischemia of the coronary artery and permanent nervous system damage, and even result in death. Hence, it is imperative to reduce or prevent hypoxia during gastrointestinal endoscopy under sedation in older patients. Although several oxygen delivery methods would reduce hypoxia during this procedure, early detection of respiratory depression and ea

BACKGROUND: Double-lumen endobronchial tubes (DLTs) are used for one-lung ventilation (OLV) during thoracic surgery. Overinflation into the bronchial cuff causes damage to the tracheobronchial mucosa, whereas underinflation leads to an incomplete collapse of the nonventilated lung or incomplete ventilation of the ventilated lung. However, how to determine the appropriate bronchial cuff volume and pressure during OLV is unclear. The objective of this study is to compare the required bronchial cuff volume for lung separation obtained by 2 different cuff inflation methods under closed- and open-c

BACKGROUND: Emergency endotracheal intubation results in accidental oesophageal intubation in up to 17% of patients. This is frequently undetected thereby adding to the morbidity and mortality. No current method to detect accidental oesophageal intubation in an emergency setting is both highly sensitive and specific. We hypothesized that, based on differences between the mechanical properties of the oesophagus and the trachea/lung, ventilation pressures could discriminate between tracheal and oesophageal intubation. Such a technique would potentially not suffer some of the limitations of curre

BACKGROUND: Arterial blood gas analysis is the gold standard for assessing the adequacy of ventilation. However, arterial blood sampling may be associated with serious complications in neonates. The aim of the study was to utilize the side-stream capnometry measurement of end-tidal carbon dioxide (PetCO₂) via nasal cannula circuits and to verify the reliability of PetCO₂ in reflecting the arterial blood carbon dioxide(PaCO₂) level in nonintubated neonates.
METHODS: A retrospective medical record review analysis was performed in nonintubated neonates admitted to the neonatal ward in a medical

INTRODUCTION: This study describes the epidemiological and clinical profile of ICU patients undergoing percutaneous tracheostomy in Costa Rica and identifies predictors of acute complications, addressing ongoing debates on timing, technique, and risk stratification.
METHODS: We performed a prospective multicenter cohort study in eight CCSS hospitals (2019-2022), including adult ICU patients undergoing percutaneous tracheostomy. Demographic, clinical, and procedural data were collected, and multivariable logistic regression identified predictors of complications.
RESULTS: A total of 516 patie

Background/Objectives: Sedation is a fundamental component of gastrointestinal endoscopy, improving patient comfort, procedural quality, and overall satisfaction. However, traditional drug-centered sedation models are increasingly challenged by rising procedural volumes, aging populations, and limited anesthesiology resources. The aim of this narrative review is to provide an integrated overview of evolving pharmacological agents, monitoring strategies, organizational models, and future directions toward personalized, technology-supported sedation pathways. Methods: A structured literature sea

BACKGROUND: Postoperative respiratory depression (PORD) is a common but underreported postoperative pulmonary complication with potentially serious short- and long-term consequences. Despite its prevalence, no universally accepted expert definition exists, making identification, diagnosis, and epidemiologic study challenging.
METHODS: An e‑Delphi study was conducted with a multidisciplinary panel of 10 board‑certified physicians in anesthesiology, surgery, pulmonology, emergency medicine, and critical care. Over four rounds, panelists anonymously rated 20 literature‑derived statements on PORD

Background and Objectives: To evaluate the prognostic value of end-tidal carbon dioxide (ETCO2), arterial carbon dioxide (PaCO2), their difference (ΔCO2 = PaCO2-ETCO2), and the ETCO2/PaCO2 ratio for 30-day mortality in adult patients intubated for acute respiratory failure in the emergency department (ED). Materials and Methods: In this single-center, prospective observational study, we enrolled consecutive adults intubated in the ED who had at least two paired arterial blood gas and capnography measurements within 90 min post-intubation. PaCO2 and ETCO2 were recorded at 0, 30, 60, and 90 min,

BACKGROUND: High ventilation-CO2 output, signaling increased wasted ventilation in the physiological dead space (V D phys), has a dominant role in eliciting activity-related dyspnoea in subjects with COPD showing only mild airflow obstruction. Exposing the mechanisms driving wasted ventilation is key to advancing the field towards new therapeutic approaches to improve dyspnoea and exercise tolerance in this growing patient sub-population.
CENTRAL HYPOTHESIS: Increased areas of high alveolar ventilation (V̇ A)/capillary perfusion (Q̇ c) due to impaired Q̇ c in nonemphysematous, non-air trappin

PURPOSE: The efficacy of noninvasive ventilation (NIV) during procedures that require sedation and analgesia has not been established. We evaluated whether NIV reduces the incidence of respiratory events.
METHODS: In this randomized controlled trial, we included 195 patients with an American Society of Anesthesiologists Physical Status of III or IV during electrophysiology laboratory procedures. We compared NIV with face mask oxygen therapy for patients under sedation. The primary outcome was the incidence of respiratory events determined by a computer-driven blinded analysis and defined by h

UNLABELLED: Capnography has been the standard in the operating room for a long time now. When variable amounts of intrapulmonary shunt and intracardiac shunt are taken into account, arterial carbon dioxide (CO2) and end-tidal CO2 typically match rather well. The gap between arterial and end-tidal CO2 widens in patients with cardiopulmonary disorders. The current study sought to determine how arterial and end-tidal CO2 correlated with each other and with hemoglobin saturation both before and after pulmonary catheterization in a pediatric population with congenital heart disease.
METHODS: Fifty

OBJECTIVE: Gliderite®, one of the first stylets designed specifically to assist tracheal intubation with non-channeled curved blades video laryngoscopes, can cause injury. The S-Guide® is a new, malleable, intubating guide allowing oxygenation. Its soft tip is designed to prevent trauma. We aimed to compare the duration of tracheal intubation with S-Guide compared to Gliderite using a C-MAC® D-Blade® video laryngoscope in patients with simulated difficult airways.
METHODS: We performed a single-blinded prospective randomized study, with 50 adult patients requiring orotracheal intubation under
