Crosstalk between the angiotensin and endothelin-system in the cerebrovasculature.

Investigations have shown a multifactorial process as cause for the poor outcome after subarachnoid hemorrhage (SAH), including inflammation, early brain injury, cortical spreading depression, lack of cerebral autoregulation and the cerebral vasospasm (CVS) itself. Losartan may have a beneficial effect after SAH - preventing CVS, restoring cerebral autoregulation, reducing inflammation and early brain injury. Also some data is available for an AT1-receptor-upregulation and upregulated gene expression after subarachnoid hemorrhage, but the functional role of angiotensin on the cerebrovascular c

A neuronal population in hypothalamus that dramatically resists acute ischemic injury compared to neocortex.

Pyramidal neurons (PyNs) of the cortex are highly susceptible to acute stroke damage, yet "lower" brain regions like hypothalamus and brain stem better survive global ischemia. Here we show for the first time that a "lower" neuron population intrinsically resists acute strokelike injury. In rat brain slices deprived of oxygen and glucose (OGD), we imaged anoxic depolarization (AD) as it propagated through neocortex or hypothalamus. AD, the initial electrophysiological event of stroke, is a front of depolarization that drains residual energy in compromised gray matter. The extent of AD reliably

Microembolic air load during contrast-transcranial Doppler: a trigger for migraine with aura?

BACKGROUND: There is a well-known association between migraine with aura (MA) and right-to-left shunt (RILES) because of patent foramen ovale (PFO). The occurrence of MA attacks after microbubble (MB) injection during contrast-enhanced transcranial Doppler (ce-TCD) has been recently described. OBJECTIVES: The aim of this study was to analyze the prevalence of RILES in a consecutive cohort of MA patients and to characterize the occurrence of MA attacks after diagnostic ce-TCD. METHODS: A total of 159 consecutive MA patients underwent ce-TCD with air-mixed saline to disclose RILES. RILES was c

The emerging role of the meningeal lymphatic and glymphatic systems in migraine pathophysiology: a systematic review.

BACKGROUND/OBJECTIVE: Migraine is a common, debilitating neurological disorder of unclear pathophysiology. Recent evidence has implicated impaired meningeal lymphatic function and its interaction with the glymphatic system in the development of neuroinflammation and pain sensitization. This systematic review aimed to summarize existing evidence on the role of the meningeal lymphatic and glymphatic systems in migraine pathophysiology. METHODS: Following the PRISMA 2020 recommendations, we searched PubMed, Web of Science, and Scopus from inception to December 15, 2025. Inclusion criteria compri

Is migraine a common manifestation of CADASIL-Cons.

Headaches and transient neurological symptoms that bear resemblances to clinical manifestations of migraine, especially migraine with aura, are common among patients with cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) or cysteine-altering NOTCH3 genetic variants. However, according to the International Classification of Headache Disorders, Third Edition (ICHD-3), these patients should be diagnosed as headache attributed to cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) rather than migrai

Sex differences in the expression of the endocannabinoid system within V1M cortex and PAG of Sprague Dawley rats.

BACKGROUND: Several chronic pain disorders, such as migraine and fibromyalgia, have an increased prevalence in the female population. The underlying mechanisms of this sex-biased prevalence have yet to be thoroughly documented, but could be related to endogenous differences in neuromodulators in pain networks, including the endocannabinoid system. The cellular endocannabinoid system comprises the endogenous lipid signals 2-AG (2-arachidonoylglycerol) and AEA (anandamide); the enzymes that synthesize and degrade them; and the cannabinoid receptors. The relative prevalence of different component

Severe and progressive neurotransmitter release aberrations in familial hemiplegic migraine type 1 Cacna1a S218L knock-in mice.

Familial hemiplegic migraine type 1 (FHM1) is caused by mutations in the CACNA1A gene, encoding neuronal presynaptic Ca(V)2.1 (P/Q-type) Ca(2+) channels. These channels mediate neurotransmitter release at many central synapses and at the neuromuscular junction (NMJ). Mutation S218L causes a severe neurological phenotype of FHM and, additionally, ataxia and susceptibility to seizures, delayed brain edema, and fatal coma after minor head trauma. Recently, we generated a Cacna1a S218L knock-in mutant mouse, displaying these features and reduced survival. A first electrophysiological study showed

Mapping brain asymmetry in health and disease through the ENIGMA consortium.

Left-right asymmetry of the human brain is one of its cardinal features, and also a complex, multivariate trait. Decades of research have suggested that brain asymmetry may be altered in psychiatric disorders. However, findings have been inconsistent and often based on small sample sizes. There are also open questions surrounding which structures are asymmetrical on average in the healthy population, and how variability in brain asymmetry relates to basic biological variables such as age and sex. Over the last 4 years, the ENIGMA-Laterality Working Group has published six studies of gray matte

Altered responses of MeCP2-deficient mouse brain stem to severe hypoxia.

Rett syndrome (RTT) patients suffer from respiratory arrhythmias with frequent apneas causing intermittent hypoxia. In a RTT mouse model (methyl-CpG-binding protein 2-deficient mice; Mecp2(-/y)) we recently discovered an enhanced hippocampal susceptibility to hypoxia and hypoxia-induced spreading depression (HSD). In the present study we investigated whether this also applies to infant Mecp2(-/y) brain stem, which could become life-threatening due to failure of cardiorespiratory control. HSD most reliably occurred in the nucleus of the solitary tract (NTS) and the spinal trigeminal nucleus (Sp

Amnesia for the McCollough effect following unilateral electroconvulsive therapy: implications for laterality.

The hemispheric lateralization of retrograde amnesia following unilateral electroconvulsive therapy (ECT) was measured by a novel nonverbal probe, the McCollough effect, which allowed equal and exclusive access to each hemisphere. Alternating exposure to perpendicular gratings of complementary colors (e.g., red vertical stripes alternated with green horizontal stripes) will cause subsequently presented black and white gratings to appear colored hours or even days later (the McCollough effect). Three of the patients examined (3 of 10) lost the effect in just the half visual field contralateral

Effect of neonatal melatonin administration on behavioral and brain electrophysiological and redox imbalance in rats.

INTRODUCTION: Melatonin (MLT) reportedly has beneficial effects in neurological disorders involving brain excitability (e.g., Epilepsy and Migraine) and behavioral patterns (e.g., Anxiety and Depression). This study was performed to investigate, in the developing rat brain, the effect of early-in-life administration of two different doses of exogenous MLT on behavioral (anxiety and memory) and electrophysiological (CSD analysis) aspects of brain function. Additionally, brain levels of malondialdehyde (MDA) and superoxide dismutase (SOD), both cellular indicators of redox balance status, were e

Ibogalogs reduce acute and chronic headache-like behaviours in rodents by different co-modulatory mechanisms involving 5-HT1 and 5-HT2 receptor subtypes.

The main objective of this work is to assess whether ibogalogs decrease headache-like behaviors in rats using the cortical spreading depression (CSD) and medication overuse headache (MOH) models. The CSD and MOH results showed that tabernanthalog (TBG), ibogainalog (IBG), and ibogaminalog (DM506) significantly decreased allodynia induced by acute injection of KCl or repetitive administration of sumatriptan. The effect of ibogalogs in the CSD model was sex-independent. The observed effects lasted 6-8 h, with TBG showing comparatively higher efficacy in both models. We also assessed the potentia

Persistent cognitive dysfunction in patients with migraine: a case series.

BACKGROUND: Cognitive symptoms, including impaired concentration, slowed thinking, and forgetfulness, are common in patients with migraine. Although these symptoms are most prominent during the headache and postdrome phases, some patients experience persistent cognitive symptoms that extend beyond individual migraine attacks and interictal periods. Cortical spreading depression, a key pathophysiological mechanism of migraine, may contribute to neurological and cognitive manifestations and can be visualized indirectly using arterial spin-labeled magnetic resonance imaging (ASL-MRI). We investig

Brain imaging in migraine with and without aura: Similarities and differences.

BackgroundWhether migraine with aura (MA) is a biologically independent entity from migraine without aura (MO) is still debated. Similarities and differences between MO and MA have been extensively investigated in recent years through several neuroimaging studies, providing valuable insights into their underlying pathophysiology. To provide a better understanding of functional and structural differences between MO and MA, we conducted a narrative review of neuroimaging studies in these two conditions.MethodsA comprehensive PubMed search for neuroimaging studies in MO and MA was conducted in Ma

Investigation of lymphotoxin α genetic variants in migraine.

Migraine is a common neurological disease with a genetic basis affecting approximately 12% of the population. Pain during a migraine attack is associated with activation of the trigeminal nerve system, which carries pain signals from the meninges and the blood vessels infusing the meninges to the trigeminal nucleus in the brain stem. The release of inflammatory mediators following cortical spreading depression (CSD) may further promote and sustain the activation and sensitization of meningeal nociceptors, inducing the persistent throbbing headache characterised in migraine. Lymphotoxin α (LTA)

Coexpression of c-fos and hsp70 mRNAs in gerbil brain after ischemia: induction threshold, distribution and time course evaluated by in situ hybridization.

Levels of mRNAs encoding the proto-oncogene, c-fos, and the 70 kDa stress protein, hsp70, were evaluated in gerbil brain following transient cerebral ischemia of varied duration by in situ and blot hybridization techniques. Blots of total hippocampal RNA obtained after 5 min ischemic insults confirmed a characteristic, transient time course of c-fos expression with a striking elevation within 1 h and a return to control levels by 3 h recirculation. Hsp70 hybridization was significant at 1 h and continued to increase until 3-6 h after the insult. Striking accumulation of c-fos mRNA was detected

Beyond AQP-4: convergent glymphatic-meningeal lymphatic dysfunction underlying multifactorial migraine pathogenesis.

The glymphatic system (GS) functions as a critical pathway for waste clearance from the brain, facilitating soluble protein and metabolite drainage. Recently, GS dysfunction has emerged as a potential contributor to migraine pathophysiology. GS operates similarly to the peripheral lymphatic system, dependent on astrocytes for metabolic waste removal. The clearance process involves cerebrospinal fluid entering the peri-arterial spaces, moving into the interstitial fluid via aquaporin-4 (AQP-4) channels at astrocyte feet, and eventually being drained into the cervical lymph nodes. As a downstrea

Engineered Extracellular Vesicles As a New Delivery Platform for Migraine.

Migraine represents a complex neurovascular disorder that is challenging to treat due to the blood-brain barrier (BBB) and complex pathophysiology involving the trigeminovascular system, neuroinflammation, and cortical spreading depression. Current systemic therapies, including calcitonin gene-related peptide (CGRP) inhibitors, offer benefits but have limited efficacy and may cause adverse effects; thus, highlighting the need for targeted delivery across the BBB. This review introduces extracellular vesicles (EVs) as an appropriate pharmaceutical engineering platform to address such challenges
