From tooth pain to trigeminal sensitization: translational models, experimental readouts, and mechanism-guided therapeutic opportunities in odontogenic and headache-related orofacial pain.

Odontogenic pain is one of the most intense forms of orofacial pain, yet durable mechanism-directed therapies remain limited. Traditionally, tooth pain has been framed as a local consequence of pulpal, dentinal, periodontal, or periapical disease. However, contemporary orofacial pain and headache frameworks increasingly position dental pain within a broader trigeminal pain network, where nociceptive input from the teeth and surrounding tissues may contribute to referred facial pain, headache-related orofacial pain overlap, and trigeminal sensitization. In this review, we synthesize odontogenic

Accuracy of mean grey density values obtained with small field of view cone beam computed tomography in differentiation between periapical cystic and solid lesions.

AIM: To determine if small and medium field of view (FOV) cone beam computed tomography (CBCT) adjusted grey density values can be used to distinguish between periapical cystic and solid lesions. METHODOLOGY: Fifty-seven patients with periapical lesions having retrievable small or medium FOV CBCT images and biopsy samples were included. Two oral and maxillofacial pathologists examined the biopsy samples to provide the gold standard diagnosis of cystic or solid lesion. From the CBCT images, two independent examiners recorded the minimum adjusted grey density value of each lesion twice. Intra-e

Validation of a dental pain screening questionnaire in a semi-urban hospital setting in South India.

AIMS: To assess the validity of modified DePaQ items in correctly classifying dental pain patients into three groups of common dental conditions when compared against clinically-determined classification; generate a model of classification equations from a sub-population of the sample; and assess the validity of the model in predicting group membership of new dental pain cases. METHODS: Consecutive adult hospital dental patients aged 18 years and older attending with a dental pain complaint were diagnosed by a dentist. Before treatment commenced a self-complete questionnaire was completed. Th

Prevalence and oral hygiene-related correlates of self-reported dentin hypersensitivity among Egyptian adults: a cross-sectional study.

BACKGROUND: Dentin hypersensitivity (DH) is a prevalent clinical condition characterized by pain arising from exposed dentin in response to external stimuli. Despite its clinical relevance, epidemiological data from Egypt and the Middle East remain limited. This study aimed to assess the prevalence, symptom characteristics, and oral hygiene-related correlates of self-reported dentin hypersensitivity among Egyptians adults. METHODS: A cross-sectional questionnaire-based study was conducted among 370 Egyptian adults recruited from multiple governorates between January and August 2025. Data was

Effectiveness of a dentifrice containing formalin and sodium monofluorophosphate on dental hypersensitivity.

A double-blind investigation comparing a control dentifrice with one containing active ingredients was conducted to determine their effect on dentinal hypersensitivity in 67 human subjects. Baseline data was obtained by patient questionnaire, dental examination and by soliciting subjective responses to controlled mechanical and thermal stimuli. A calcium carbonate-based toothpaste constituted the base for both the control and active dentifrice. The active dentifrice contained 0.8% sodium monofluorophosphate and 1.3% formalin while the control lacked these ingredients. Dentifrices were supplied

Fluoride therapy in clinical practice.

Research into the use of fluorides is dynamic and ongoing. As a consequence, anyone interested in clinical application must be up to date in the current literature. A number of other trace elements have also been found to have impact, particularly on the disease of dental caries. Therefore, to treat oral disease effectively with these products, it is important that the clinician keep current with published studies. Fluorides are, at present, a major therapeutic measure in the treatment of both dental caries and periodontal disease. However, fluorides will only reduce disease, not eliminate it.

Evaluation of two mineral trioxide aggregate compounds as pulp-capping agents in human teeth.

AIM: The present randomized, controlled prospective study evaluated the histomorphological response of human dental pulps capped with two grey mineral trioxide aggregate (MTA) compounds. METHODOLOGY: Pulp exposures were performed on the occlusal floor of 40 human permanent pre-molars. The pulp was capped either with ProRoot (Dentsply) or MTA-Angelus (Angelus) and restored with zinc oxide eugenol cement. After 30 and 60 days, teeth were extracted and processed for histological examination and the effects on the pulp were scored. The data were subjected to Kruskal-Wallis and Conover tests (alph

Diagnostic competence of senior dental students in detecting caries on panoramic radiographs with and without artificial intelligence assistance: a cross-sectional studycaries detection on panoramic radiographs.

PURPOSE: Accurate detection of proximal dental caries on panoramic radiographs is essential for effective treatment planning and preventive care. While senior dental students gradually develop interpretative competence during their training, artificial intelligence (AI) systems have emerged as promising adjuncts to enhance diagnostic performance. This study aimed to compare the diagnostic competence of fourth- and fifth-year dental students in detecting proximal carious lesions on panoramic radiographs, with and without AI assistance. It was hypothesized that AI-assisted evaluation would signi

A critical review of the validity and reliability of caries detection methods for non-cavitated lesions.

OBJECTIVE: Dental caries is the most prevalent chronic disease that affects people of all ages. Early detection of dental caries is crucial to prevent irreversible dental tissue damage, infection, and tooth loss, yet traditional detection methods face challenges in reliably identifying non-cavitated lesions. The review aims to determine the validity and reliability of epidemiological and clinical diagnostic methods used to detect non-cavitated caries. METHODS: Twenty in-vivo studies from 1997 to 2025 were analyzed, focusing on clinical visual examination, radiographs, tactile examination, and

Evaluation of Fluoraphat Pro and VivaSens® In-Office Desensitizing Agents in Controlling Natural Dental Hypersensitivity: A Clinical Study.

Introduction Dental erosion from acidogenic diets, vigorous tooth brushing, excessive tooth whitening, gingival recession, periodontal debridement, or surgery may induce dentine hypersensitivity (DH). It manifests as a phenomenon observed in structurally intact teeth devoid of pathological or defective conditions. Hence, this study aimed to evaluate DH risk reduction after using Fluoraphat Pro (Neumunster, Germany) and VivaSens® (Ivoclar Vivadent, Schaan, Liechtenstein, Switzerland) in-office desensitizing agents. Method Twenty-two participants with at least two hypersensitive teeth were rando

NO-cGMP Signaling in Endothelial Function of the Healthy and Inflamed Dental Pulp.

The intra- and intercellular signaling molecule nitric oxide (NO) is produced in endothelial cells by the activity of endothelial NO synthase (eNOS). Upon formation, NO diffuses into the underlying vascular smooth muscle cells, where it activates NO-sensitive guanylyl cyclase (NO-GC) resulting in the production of cyclic guanosine 3',5'-monophosphate (cGMP) from guanosine 5'-triphosphate (GTP). Inducing vasodilatation, inhibiting platelet aggregation and leukocyte adhesion, and inhibiting the proliferation and migration of vascular smooth muscle cells, the NO-cGMP signaling leads to a number o

Silver diamine fluoride (SDF) used in childhood caries management has potent antifungal activity against oral Candida species.

BACKGROUND: The microbiome of Severe-Early Childhood Caries (S-ECC), is characterized by an ecosystem comprising bacterial and fungal species, with a predominance of Candida species. Hence, an anti-cariogen effective against both bacteria and fungi would be valuable in the management of S-ECC. Here we evaluate the antifungal effect of silver diamine fluoride (SDF) against 35-clinical yeast isolates (Ten-each of C. albicans, C. krusei, C. tropicalis and five C. glabrata strains) from dentinal caries-lesions from S-ECC. RESULTS: Disc-diffusion and time-kill assays as well as MIC50 and MIC90 eva

Iron deposition and ferritin heavy chain (Fth) localization in rodent teeth.

BACKGROUND: An iron rich layer on the labial surface is characteristic of the enamel of rodent incisors. In order to address a role for iron content in continuously growing incisors during odontogenesis, we studied iron deposition patterns in enamel and dentine using Perls' blue staining and ferritin heavy chain (Fth) immunolocalization. Fth expression is regulated by iron level; therefore its localization can be used as a sensitive indicator for iron deposition. RESULTS: Sagittal sections of 4-week old rat incisors showed a gradual increase in iron level in the enamel organ from secretory to

Early tissue reactions after circulatory and skeletal damage to the mandible of the rat.

Skeletal and circulatory damage in the form of a fracture of the mandible, proximal to the entry of the inferior alveolar artery and other perforantes, alone or combined with unilateral or bilateral mucoperiosteal extirpation, was inflicted on growing young rats. A morphological study of the tissue changes in the bone, dental tissues and soft tissues was performed. Cutting of the nutrient vessels together with extirpation of the periosteum was followed after 10 days by extensive infarction of the incisor pulp, the odontoblasts and the apical part of the enamel organ. The bone was damaged centr
