The lived experiences of transgender and gender-diverse people in accessing publicly funded penile-inversion vaginoplasty in Canada.

BACKGROUND: Canada's health care systems underserve people who are transgender and gender diverse (TGD), leading to unique disparities not experienced by other patient groups, such as in accessing gender-affirmation surgery. We sought to explore the experiences of TGD people seeking and accessing gender-affirmation surgery at a publicly funded hospital in Canada to identify opportunities to improve the current system. METHODS: We used hermeneutic phenomenology according to Max van Manen to conduct this qualitative study. Between January and August 2022, we conducted interviews with TGD people

Transvaginal mesh in Australia: An analysis of news media reporting from 1996 to 2021.

INTRODUCTION: Transvaginal mesh (mesh) surgeries have been used to treat stress urinary incontinence (incontinence) and/or pelvic organ prolapse (prolapse). In Australia, as in many other countries, the harms caused by mesh eventually prompted individual and collective attempts to achieve redress. The rise of mesh surgery as a procedure, the experience of mesh-affected women and the formal inquiries and legal actions that followed all occurred in social, cultural and discursive contexts. One strategy to understand these contexts is to track how the mesh and key actors in the mesh stories have

"Keep it a secret": Leaked Documents Suggest Philip Morris International, and Its Japanese Affiliate, Continue to Exploit Science for Profit.

INTRODUCTION: The tobacco industry has a long history of manipulating science to conceal the harms of its products. As part of its proclaimed transformation, the world's largest tobacco company, Philip Morris International (PMI), states it conducts "transparent science." This paper uses recently leaked documents from PMI and its Japanese affiliate, Philip Morris Japan (PMJ), to examine its contemporary scientific practices. AIMS AND METHODS: Twenty-four documents dating 2012 through 2020 available from the Truth Tobacco Industry Documents Library were examined using Forster's hermeneutic appr

Parents' experiences of living with, and caring for children, adolescents and young adults with Mucopolysaccharidosis (MPS).

BACKGROUND: Many rare diseases of childhood are life-threatening and chronically debilitating, so living with a rare disease is an on-going challenge for patients and their families. MPS is one of a range of rare inherited metabolic disorders (IMDs) that come under category 3 of life-limiting conditions, where there is no curative treatment available at present. Although the study of rare diseases is increasingly novel, and of clinical importance to the population, the lack of empirical data in the field to support policy and strategy development is a compelling argument for further research t

Life-Sustaining Treatment Decisions and Parent Caregiving Experiences for Children With Medical Complexity.

IMPORTANCE: Children with medical complexity (CMC) frequently experience recurrent deterioration requiring life-sustaining treatment (LST) decisions in the pediatric intensive care unit. Parents serve as primary caregivers and decision makers; however, how accumulated caregiving experiences shape thresholds for acceptable LST during crises remains insufficiently understood. OBJECTIVES: To explore how parents of CMC interpret their longitudinal caregiving experiences and how these interpretations shape parents' orientations toward LST during episodes of clinical deterioration. DESIGN, SETTING

Narrative identity at the end of life: a qualitative analysis of dignity therapy interviews from an existential perspective.

BACKGROUND: Confronting terminal illness and the prospect of death and dying can severely undermine an individual's sense of identity, meaning and purpose in life, leading to existential suffering, increasing hopelessness, depressive symptoms, and death anxieties. OBJECTIVES: This study explored how narrative approaches can alleviate existential suffering, promote emotional healing, and consolidate identity for individuals confronted with death and dying. Specifically, the study aimed to (1) analyze dimensions of identity reflected in Dignity Therapy (DT) legacy documents from a randomized co

Advance Care Planning as Key to Person-Centered Care: A Co-Design Study Involving Adults with Chronic Kidney Disease, Family Members, and Healthcare Professionals.

INTRODUCTION: Advance care planning is a process that supports adults at any age or stage of health in understanding and sharing their personal values, life goals, and preferences regarding future medical care. Advance care planning is recommended for adults with chronic kidney disease and is an approach to facilitating person-centered care. The approach is useful at all stages of the disease and not solely related to end of life or serious disease events but also concerns management of physical, emotional, and psychological challenges throughout the illness trajectory. Living with chronic kid

Decommissioning normal: COVID-19 as a disruptor of school norms for young people with learning disabilities.

UNLABELLED: The COVID-19 pandemic has forced everyone to live at a social distance from other people. This has changed the way people live and are included socially. This paper focuses on the unexpected ways schools have altered and deepened social inclusion for children with learning disabilities during the COVID-19 pandemic. We interviewed six people: two people who work for a Local Authority, one Headteacher of a special school, one Special Educational Needs and Disability Consultant, one young person with a learning disability and her mother. The findings and conclusions show the "new normal"

[Psychosomatic pain in the field of gynecology. Stages of the formation of psychosomatic reactions and symptoms with examples of cases (author's transl)].

1. Understanding disease in the context of a hermeneutic of the ill person's life history forces us to abandon the old understanding of causality which is represented by the model of body-soul polarity. The real onset of an illness has a biographic pre-history. Long latent periods may occur before and between "functio laesa" and actual damage to the involved organ. On the basis of psychosomatic case histories, we can conclude that the future-orientation of an individual's life, that for which man must make decisions, is also that which causes the crises situation for those who are suffering. 2

Involving patient partners as appraisers of shared decision making: an explanatory sequential mixed-methods study.

BACKGROUND: Patient and public involvement (PPI) is increasingly recognised as a valuable component of health research, yet patients are rarely included in data analysis within shared decision making (SDM) studies. We aimed to assess the feasibility and value of involving patient partner appraisers of SDM using the OPTION5 instrument. Specific objectives were to examine interrater reliability between patient partners and researchers, and to explore patient partners' perspectives on observed differences in interrater reliability. METHODS: An explanatory sequential mixed-methods design was used

A transdisciplinary framework for chronic illness and chronic pain: a sequential qualitative meta-synthesis.

INTRODUCTION: Chronic pain is a common, disabling feature of many long-term conditions, yet its clinical management remains fragmented across disciplines, diagnoses, and organizations. Although a substantial qualitative literature describes patients' lived experience of chronic illness and chronic pain, these findings have rarely been synthesized into a transdisciplinary framework explicitly oriented toward coordinated clinical care. We therefore aimed to develop such a framework grounded in lived experience and relevant to clinical application. METHODS: We conducted a sequential qualitative

Care Integration for Hepatitis C Virus Treatment Through Facilitated Telemedicine Within Opioid Treatment Programs: Qualitative Study.

BACKGROUND: Telemedicine has the potential to remove geographic and temporal obstacles to health care access. Whether and how telemedicine can increase health care access for underserved populations remains an open question. To address this issue, we integrated facilitated telemedicine encounters for the management of hepatitis C virus (HCV), a highly prevalent condition among people with opioid use disorder (OUD), into opioid treatment programs (OTPs). In New York State, OTPs are methadone-dispensing centers that provide patient-centered, evidence-based treatment for OUD. We investigated the

Assessment of left atrial functions in cardiac syndrome X.

INTRODUCTION: Maternity unit closures in rural and remote settings of Australia have left a substantial gap in services for pregnant women. In the absence of midwives, and when women are unable to attend a maternity facility, registered nurses (RNs) are required to fill the void. While maternity education can attempt to prepare RNs for such encounters, there is little documented to suggest it meets all their physical and psychological needs. The existing challenges for health professionals, practising a vast generalist scope of practice while living and working in a rural and remote location,

Care and support when a baby is stillborn: A systematic review and an interpretive meta-synthesis of qualitative studies in high-income countries.

INTRODUCTION: Approximately 2 million babies are stillborn annually worldwide, most in low- and middle-income countries. Present review studies of the parental and healthcare providers' experiences of stillbirth often include a variety of settings, which may skew the findings as the available resources can vary considerably. In high-income countries, the prevalence of stillbirth is low, and support programs are often initiated immediately when a baby with no signs of life is detected. There is limited knowledge about what matters to parents, siblings, and healthcare providers when a baby is st
