Isolated Glossopharyngeal Nerve Herpes Zoster Infection Following Varicella Vaccination in an Emergency Physician: A Case Report.

BACKGROUND: Herpes zoster (HZ) can affect the cranial nerves, but rarely it involves the glossopharyngeal nerve in isolation. Varicella-zoster virus (VZV) infection most often occurs in the context of previous varicella infection, but it is exceedingly rare in immunocompetent vaccinated patients without previous infection. CASE REPORT: We report a unique case of a 34-year-old immunocompetent male presenting with severe throat pain, odynophagia, and headache, accompanied by vesicles on the left side of hard and soft palate, base of the tongue, and the posterior pharyngeal wall. Notably, the pa

Nausea and Vomiting as Initial Manifestations of Pediatric NMOSD.

Intractable nausea and vomiting are commonly attributed to gastrointestinal (GI) conditions but can sometimes be a symptom of an underlying central nervous system disease. One potentially overlooked neurologic cause of intractable nausea and vomiting that is refractory to antiemetics is area postrema syndrome (APS). APS is a condition characterized by lesions of the dorsal caudal medulla and is considered a core clinical feature of neuromyelitis optica spectrum disorder (NMOSD). APS is present in up to 30% of patients ultimately diagnosed with NMOSD and can be the first presenting symptom of N

The Etiology and Outcome of Area Postrema Syndrome in Childhood: Two Cases and a Literature Review.

BACKGROUND: Area postrema syndrome (APS), a rare childhood condition, manifests as intractable nausea and hiccups. APS has high diagnostic significance in neuromyelitis optica syndrome spectrum disorders (NMOSD) and can be the initial presentation of other critical diseases, including brainstem glioma. METHODS: We described two representative cases of unrelated Japanese patients with APS. An etiologic evaluation, including a detailed intracranial neuroradiological examination and autoantibodies assessment, was performed. We also reviewed the literature focusing on the prognosis of pediatric A

Fiberoptic bronchoscopic treatment of blood aspiration and use of sugammadex in a patient with epistaxis: A case report.

RATIONALE: In patients with oropharyngeal and nasopharyngeal bleeding, blood aspiration can make airway management difficult and lead to severe pulmonary complications. PATIENT CONCERNS: A 44-year-old male patient with recurrent epistaxis underwent surgery for hemostasis. The patient aspirated blood through the endotracheal tube when he hiccupped during the surgery. DIAGNOSIS: The patient was diagnosed with blood aspiration after intraoperative fiberoptic bronchoscopy revealed a blood clot and viscous mucus in the airways, but no sign of active bleeding. INTERVENTIONS: Tracheobronchial suct

Reflections on the brainstem dysfunction in neurologically disabled children.

This article deals with the neurological basis of brainstem-related symptoms in disabled children. Synaptic interactions of respiratory and swallowing centers, which are briefly reviewed in this study, highlight the significance of the nucleus of solitary tract (NTS) in the stereotyped motor events. Coordination mechanisms between these two central pattern generators are also studied with a focus on the inhibitory action of decrementing expiratory neurons that terminate the inspiratory activity and become activated during swallowing. Dorsal brainstem lesions in hypoxic-ischemic encephalopathy

[Midazolam (Dormicum) in terminal anxiety and agitation. The last choice alternative in palliative care].

Although midazolam has been proposed for the treatment of a variety of conditions such as anxiety, dyspnoea, hiccups and status epilepticus, terminal agitation is the only condition where its use is based on a reasonably large number of published clinical studies. A causal approach is generally recommended. Whenever possible, the aetiological condition (pain, fever, constipation, etc.) should be corrected. Such general measures as ensuring a peaceful, familiar environment, and the use of a night light, fluid therapy to counteract dehydration, and antipyretics for fever are beneficial. When sym

Post-COVID-19 Area Postrema Syndrome With SARS-CoV-2 in CSF: A Dual-Case Report and Review of the Literature.

BACKGROUND: Neuromyelitis optica spectrum disorder (NMOSD) is a rare autoimmune astrocytopathy characterized by inflammatory demyelinating lesions in the central nervous system. Area postrema syndrome (APS), marked by intractable nausea, vomiting, and hiccups, is a recognized but less common initial manifestation. Post-infectious autoimmunity triggered by SARS-CoV-2 has been increasingly associated with NMOSD pathogenesis; however, the clinical significance of direct viral neuroinvasion and its relationship to divergent patient outcomes remains poorly understood. METHODS: We report two female

Electrocardiogram Synchronized Physiological Feedback During Obex Cavernoma Surgery: 2-Dimensional Operative Video.

This operative video presents a physiology-guided strategy for resection of a right paramedian obex cavernoma in a 57-year-old man with intractable hiccups, progressive dysphagia with aspiration, and hoarseness. Magnetic resonance imaging showed interval enlargement and recurrent hemorrhagic evolution, exophytic presentation to the caudal fourth ventricle, and an associated developmental venous anomaly. Because continued observation carried a risk of recurrent hemorrhage and worsening bulbar dysfunction, resection was performed through a midline suboccipital telovelar approach with multimodal

A case of falx cerebelli dural arteriovenous fistula with perimedullary venous reflux after occipital bone fracture.

Intracranial dural arteriovenous fistulas (dAVFs) with perimedullary venous reflux are uncommon but can lead to progressive myelopathy. Although most dAVFs are idiopathic or associated with dural sinus thrombosis, a falx cerebelli dAVF developing after an occipital bone fracture is exceedingly rare and clinically important. We report a 77-year-old patient who presented with 1 month of progressive bilateral lower-extremity weakness, urinary retention, and hiccups, 1 year after an occipital bone fracture. Magnetic resonance imaging showed longitudinal T2-weighted imaging hyperintensity from the

Complex Vascular Ring Diagnosed on Cardiovascular MR in a 3-Day-Old Infant.

Prenatal ultrasonography in the early third trimester showed an unusual branching pattern of the right aortic arch. Echocardiography performed 4 h after birth showed the right aortic arch with mirror-image branching, patent ductus arteriosus, and patent foramen ovale. Because the location of the ductus arteriosus was unclear on echocardiography, cardiovascular magnetic resonance imaging was performed 3 days after birth. Advanced techniques including contrast-enhanced time-resolved magnetic resonance angiography and 3D time-of-flight magnetic resonance angiography allowed accurate diagnosis of

Cerebrospinal fluid dissemination of anaplastic intraventricular meningioma: report of a case presenting with progressive brainstem dysfunction and multiple cranial nerve palsies.

BACKGROUND: It is extremely rare to see cerebrospinal fluid dissemination of intraventricular meningioma, particularly with the development of acute, progressive brainstem/cerebellar dysfunction with an absence of mass formation in the corresponding anatomical sites. CASE PRESENTATION: An 81-year-old man was admitted because of double vision, right facial nerve palsy and truncal ataxia. Brain magnetic resonance imaging showed normal findings except for a tumor mass in the left lateral ventricle, which had been noted over 6 months previously. The patient developed hiccups, hyperventilation, an

[The effect of short peptides isolated from the brain of a hibernating suslik on the rat EEG and behavior].

The intracerebroventricular injection of the peptides kyotorphin (KT), newkyotorphin (NKT), and Aps-Tyr (BS) changed rat behaviour. There was an increase in the rate of exploratory reactions, rearing, grooming, stereotypic scratching movements, yawning, stretching, hiccupping, sneezing. Similar effects had been earlier observed in rats after administration of the fraction (F, 1-10 kD) from the brain of hibernating ground squirrels in which the studied peptides were found. The EEG effects depended on a peptide dose. As in the case of F injection, KT in the dose of 4 mcg suppressed the theta- an

Myelodysplastic syndrome-like response after voriconazole treatment of systemic lupus erythematosus complicated with fungal infection: a case report.

BACKGROUND: Voriconazole is mainly used to treat progressive and potentially life-threatening infections in immunocompromised patients. The adverse drug reactions related to voriconazole are varied. In some rare cases, the use of voriconazole can result in myelodysplastic syndrome (MDS)-like adverse reactions. CASE PRESENTATION: Here, we present a rare case of systemic lupus erythematosus patient with a fungal infection that developed MDS-like adverse reactions after treatment with voriconazole. The patient was admitted to the hospital because of 3 days of chest tightness and dyspnea. After t
