Drug therapy for obesity in the elderly.

The prevalence of obesity is increasing rapidly in the US and other developed countries. Even though the percentage of older individuals is increasing worldwide, obesity has only recently become a recognised problem in this population. Obesity occurs when energy intake chronically exceeds energy expenditure. Moreover, advancing age is associated with an inability to couple energy intake with energy expenditure. Obesity contributes to many adverse health outcomes, including non-insulin-dependent (type II) diabetes mellitus, as well as to an increase in both cardiovascular and all-cause mortalit

Pharmacotherapy of obesity.

Obesity has become a significant health problem in industrialised and developing countries, and despite all nutritional and behavioural approaches, its prevalence is still increasing. In recent years, the identification and characterisation of central and peripheral mechanisms involved in the regulation of energy balance has made remarkable progress and provided numerous targets for novel anti-obesity agents. However, only few anti-obesity drugs are on the market and not many compounds have entered clinical development. In the present review, the clinically available agents are discussed and t

Mass spectrometric evidence of covalently-bound tetrahydrolipstatin at the catalytic serine of Streptomyces rimosus lipase.

We have recently detected that the lipase from Streptomyces rimosus belongs to a large but poorly characterised family of SGNH hydrolases having the alpha beta alpha-fold. Our biochemical characterisation relates to the specific inhibition of an extracellular lipase from Streptomyces rimosus (SRL, 24.2 kDa, Q93MW7) by the preincubation method with tetrahydrolipstatin (THL). In high molar excess (THL/SRL=590 at 25 degrees C, pH=7.0) and after 2 h of incubation in an aqueous system, 56% of the enzyme inhibition was reached. Under the same conditions and in the presence of 50% (v/v) 2-propanol/wa

Appetite suppression through delayed fat digestion.

High-fat diets are often associated with greater caloric intake and weight gain. Since satiety during fat intake is induced by fat in the intestine we investigated the efficiency of a lipid compound that retards fat digestion to regulate fat intake. We found this compound to reduce high-fat food intake, body weight and blood lipids in Sprague-Dawley rats, without causing steatorrhea. The absence of steatorrhea is explained by an increased pancreatic lipase/colipase secretion, compensating the impaired lipolysis by the added compound. The animals also had an elevated CCK secretion. The satiety

Inhibition of lipases from Chromobacterium viscosum and Rhizopus oryzae by tetrahydrolipstatin.

Tetrahydrolipstatin is known as an inhibitor for pancreatic lipase but not for microbial lipases. In this paper we demonstrate that in the presence of water-insoluble substrates like tributyrin or olive oil, tetrahydrolipstatin inhibits the lipases of Chromobacterium viscosum and Rhizopus oryzae, although with different potency. In contrast to porcine pancreatic lipase, which forms an irreversible and covalent enzyme-inhibitor complex with tetrahydrolipstatin, the inhibition of the microbial lipases is reversible as the inhibitor can be removed from the enzyme-inhibitor complex by solvent extr

Safe and effective management of the obese patient.

The prevalence of overweight and obesity has increased dramatically in the recent decades, and obesity is now a major public health problem. Obesity negatively influences an individual's health by increasing mortality and raising the risk for multiple medical conditions such as type 2 diabetes mellitus, hypertension, dyslipidemia, and coronary heart disease. In addition, the obese individual is often the brunt of social discrimination. Weight loss has been shown to reduce the risk for many of these comorbid conditions. A multifaceted approach to the obese patient should include identifying pot

Managing type 2 diabetes mellitus in patients with obesity.

Obesity is the most important modifiable risk factor for type 2 diabetes mellitus and most patients with diabetes are overweight or obese. It is well known that excess bodyweight induces or aggravates insulin resistance, which is a characteristic feature of type 2 diabetes. Thus, bodyweight plays a central role in the prevention and treatment of diabetes. Recent data suggest that lifestyle intervention in patients with impaired glucose tolerance results in an impressive reduction in the conversion to overt diabetes, which is greater than the effect of early intervention with drugs such as metf

Nonpharmacologic and pharmacologic management of weight gain.

Obesity increases the risk of several serious health problems, including heart disease, type II diabetes mellitus, hypertension, and osteoarthritis. Patients taking certain psychotropic medications may gain a significant amount of weight (as much as a 5% increase in body weight within 1 to 2 months), placing them at risk for obesity. Body weight monitoring and prudent drug selection are the best approaches to preventing weight gain in patients taking psychotropic drugs. When weight gain (> 5% of initial body weight) is unavoidable, intervention counseling should begin. Nonpharmacologic measure

Indication-specific adverse event profiles of semaglutide and liraglutide: a FAERS disproportionality analysis.

OBJECTIVE: By 2021, regulatory agencies had approved two glucagon-like peptide-1 receptor agonists (GLP-1RAs), semaglutide and liraglutide, for the treatment of type 2 diabetes and obesity. Since then, concerns have emerged about whether adverse events (AEs) differ by GLP-1RA or indication. RESEARCH DESIGN AND METHODS: We analyzed the Food and Drug Administration Adverse Event Reporting System (FAERS) reports (2021-2025) to compare adverse events associated with semaglutide and liraglutide indications. Comparisons with metformin and orlistat helped distinguish drug-related signals from diseas

Characterisation of potential vaccine targets in Haemonchus contortus.

In a recent study, bioinformatics and RNA interference (RNAi) were used to look for the genes responsible for the acute lethal or developmental arrest in Haemonchus contortus. Out of the fifty-six H. contortus genes selected as potential targets, 41 provided strong to moderate phenotype. Seven genes were then selected for expression, purification and characterisation in the present study. A 981 bp full-length cDNA encoding H. contortus fatty acid synthase thioesterase domain (HcFAS-TE) was cloned, expressed in Escherichia coli, and the recombinant protein purified and biochemically characteris

Jiangtang Tiaozhi Formula alleviates obesity by enhancing adipose thermogenesis via TGR5-mediated gut-liver-adipose axis.

BACKGROUND: Development of safe and effective anti-obesity drugs has emerged as an urgently needed issue. Jiangtang Tiaozhi Formula (JTTZF), a traditional Chinese medicine formulation, has demonstrated potential antidiabetic and hypolipidemic properties. However, the anti-obesity efficacy of JTTZF and its mechanisms remain insufficiently understood. METHODS: We assessed the pharmacodynamic effects of JTTZF in diet-induced obese (DIO) mice, as well as in HepG2 cells and adipocytes, focusing on body weight and glucose/lipid metabolism in vivo and in vitro, respectively. 16S rDNA sequencing prof

Obesity Therapy: How and Why?

BACKGROUND: Obesity represents the second preventable mortality cause worldwide, and is very often associated with type 2 Diabetes Mellitus (T2DM). The first line treatment is lifestyle modification to weight-loss, but for those who fail to achieve the goal or have difficulty in maintaining achieved results, pharmacological treatment is needed. Few drugs are available today, because of their side effects. OBJECTIVE: We aim to review actual pharmacological management of obese patients, highlighting differences between Food and Drug Administration - and European Medicine Agency-approved molecul

Targeting fatty acid metabolism in hepatic macrophages mitigates acetaminophen-induced liver injury and promotes regeneration in mice.

BACKGROUND: An overdose of acetaminophen (APAP) can cause severe liver injury and may even lead to death. However, the mechanisms that underlie APAP-induced liver injury (AILI) are not fully understood. The extensive liver necrosis resulting from APAP overdose aggravates the sterile inflammatory response by activating resident macrophages and recruiting various immune cells. Numerous studies have highlighted the crucial role of lipid metabolism in macrophage function. This study investigated fatty acid metabolism in hepatic macrophages and its impact on the progression of AILI. METHODS: We ad
