Protocol analysis of dual-energy CT for optimization of kidney stone detection in virtual non-contrast reconstructions.

OBJECTIVES: Previous studies have shown that split-bolus protocols in virtual non-contrast (VNC) reconstructions of dual-energy computed tomography (DE-CT) significantly decrease radiation dose in patients with urinary stone disease. To evaluate the impact on kidney stone detection rate of stone composition, size, tube voltage, and iodine concentration for VNC reconstructions of DE-CT. METHODS: In this prospective study, 16 kidney stones of different sizes (1.2-4.5 mm) and compositions (struvite, cystine, whewellite, brushite) were placed within a kidney phantom. Seventy-two scans with nine d

Analysis of 451,891 canine uroliths, feline uroliths, and feline urethral plugs from 1981 to 2007: perspectives from the Minnesota Urolith Center.

Urolithiasis is a general term referring to the causes and effects of stones anywhere in the urinary tract. Urolithiasis should not be viewed conceptually as a single disease with a single cause, but rather as a sequela of multiple interacting underlying abnormalities. Thus, the syndrome of urolithiasis may be defined as the occurrence of familial, congenital, or acquired pathophysiologic factors that, in combination, progressively increase the risk of precipitation of excretory metabolites in urine to form stones (ie, uroliths). The following epidemiologic discussion is based on quantitative

A review of advances in research on mechanisms inhibiting kidney stone formation from a network pharmacology perspective.

Nephrolithiasis is a prevalent, highly recurrent urological disorder affecting 1%-15% of the global population, with 5-year recurrence rates reaching up to 50%. While surgical interventions achieve acute stone clearance, they do not resolve underlying metabolic derangements, crystal-induced tubular injury, or chronic inflammatory cascades. Natural products from global ethnomedicinal systems (including Traditional Chinese Medicine, Indian Ayurveda, Persian Unani medicine, European phytotherapy, and African ethnopharmacology) demonstrate potent multi-target anti-urolithic properties. However, de

Pantoea piersonii IIIF1SW-P2T triggers cytotoxicity through L-asparaginase-driven ammonium secretion and colonizes reproductive tract of Caenorhabditis elegans.

Pantoea piersonii IIIF1SW-P2T (basionym Kalamiella piersonii IIIF1SW-P2T), a bacterial species isolated from the International Space Station, was predicted to be non-pathogenic to humans, unlike its clinical counterpart P. piersonii YU22, which induces struvite crystallization and causes toxicity to Human Embryonic Kidney cell line (HEK 293 T). Here, we identify L-asparaginase-mediated cytotoxicity of IIIF1SW-P2T on HEK 293 T in vitro and demonstrate strain's colonization ability in the reproductive tract in vivo using Caenorhabditis elegans as a model system. The recombinant L-asparaginase of

Cystoscopy-assisted urolith retrieval via a perineal urethrostomy stoma in male cats.

ObjectivesWe sought to characterize cystoscopy-assisted urolith retrieval via a perineal urethrostomy stoma (CUPU) initially in a cadaveric model and then in clinical cases. We hypothesized that a CUPU would provide a reasonable alternative to the traditional approach of performing a cystotomy after perineal urethrostomy in male cats with complex or recurrent urolithiasis-associated urethral obstruction (UO).MethodsA perineal urethrostomy (PU) was performed in 20 male cat cadavers. A randomly assigned number of synthetic calculi (SynC) was placed retrograde into the urinary bladder, and an end

Changing composition of renal calculi in patients with musculoskeletal anomalies.

BACKGROUND AND PURPOSE: Calculi from patients with musculoskeletal (MS) anomalies who are largely immobile and prone to urinary infections have been traditionally composed primarily of struvite and carbonate apatite. Because of substantial improvements in the care of these patients in recent decades, stone etiology may have shifted from infectious to metabolic. We assessed the composition of renal calculi and metabolic characteristics in a contemporary cohort of patients with MS anomalies who underwent percutaneous nephrolithotomy (PCNL). PATIENTS AND METHODS: Retrospective analysis of patien

Alkaline desorption mechanisms and fixed-bed cyclic performance of ZnAlLa-LDHs for phosphate removal.

Eutrophication is a significant challenge in global water environmental management, with excessive phosphate discharge being one of its primary causes. Layered double hydroxides (LDHs) have attracted considerable attention as efficient phosphate removal adsorbents due to their excellent adsorption performance and tunable structure, making them a key focus of research for phosphate wastewater treatment. This study investigates the alkaline desorption mechanisms and fixed-bed cyclic performance of ZnAlLa-LDHs in phosphate removal. Experimental results demonstrated that approximately 74.4% of ads

Innovative use of the new pulsed-thulium: YAG laser for ureteroscopic lithotripsy: can the "kidney stone calculator" predict lithotripsy duration?

To assess the accuracy of Kidney Stone Calculator(KSC), a software designed for surgical planning, in predicting the duration of lithotripsy during flexible ureteroscopy(FURS) when using the novel pulsed-Thulium: YAG(p-Tm: YAG) laser. From February to August 2023, a single-center prospective study was conducted, including patients with kidney or ureteral stones through non-contrast computed tomography(NCCT), who underwent FURS with p-Tm: YAG laser lithotripsy. KSC used three-dimensional segmentation of the stones from NCCT images, along with an interactive user interface for laser settings, to

Encrustation of intraprostatic stents--a comparative study.

Metallic stents have been used in the management of prostatic disease in patients unfit for surgery. A variety of stents have become available but the optimum design and metal of construction has not yet been defined. This study examined one of the potential complications of stent insertion by demonstrating that they are susceptible to encrustation. Using an in vitro model it compares the ability of the different materials used in their construction to resist this encrustation. Titanium appears less able to resist deposition than the other metals examined. The long-term complication of encrust

Clinical observations on 2086 patients with upper urinary tract stone.

The present series comprises 2086 consecutive patients who were studied in a stone clinic during a period of 15 years. Each patient was subjected to a comprehensive protocol including a fully biochemical investigation. Calcium stones were by far the commonest accounting for 61% of cases; infection, uric acid/calcium oxalate and cystine stones accounted respectively for 24%, 8%, 5% and 2%. Nephrolithiasis was more prevalent in males with the male to female ratio 1:0.76, on the other hand infection stones were more frequent in females with the male to female ratio 1:1.6. Modeling the crystallization of magnesium ammonium phosphate for phosphorus recovery.

The crystallization of magnesium ammonium phosphate (MAP) is one of the main processes for recovering P and N from wastewater. Chemically defined solution systems were designed; the saturation indices (SIs) of the solution systems with respect to MAP were derived by using a geochemical aqueous model Program, PHREEQC 2.11; the effects of the solution conditions were evaluated using thermodynamic theories. The concentrations of P and Mg in the tested solutions were 10-600 mg l(-1) and 24-720 mg l(-1), respectively, the molar ratios of N/P and pH values of the solutions varied in the ranges of 1-

Optimal artificial urine formulations for in vitro urolithiasis models.

PURPOSE: To develop and characterize standardized in vitro artificial urine (AU) models for three major stone-forming environments-calcium oxalate (CaOx), uric acid (UA), and infection-related struvite-and to identify assay operating conditions that maximize stent-associated surface deposition while limiting excessive bulk precipitation. MATERIALS AND METHODS: Homemade AU models were formulated from a physiologic base composition and modified to create disease-specific lithogenic conditions. The CaOx model used sodium oxalate fixed at 80 mg/L with stepwise calcium chloride escalation to 600%.

What Factors Drive Staghorn vs Nonstaghorn Pattern Growth in Patients with Metabolic Stones?

Purpose: There has been an increase in the number of staghorn calculi that form in the absence of infection (metabolic staghorns). It is unknown why some large metabolic calculi form as solitary or multiple separate, nonbranching caliceal stones, whereas others develop as staghorn stones forming a cast of the collecting system. We sought to compare these two groups of metabolic stone formers (SFs) in an attempt to shed light on these disparate stone-forming phenomena. Materials and Methods: From January 2017 to September 2018, 190 patients underwent percutaneous nephrolithotomy for stones >2 c
