Centrifugation versus filtration-based cell salvage: impact on perioperative bleeding in cardiac surgery-the COLTRANE randomised clinical trial - study protocol.

INTRODUCTION: Cardiac surgery remains a high-risk procedure for bleeding despite advances in patient blood management. Conventional centrifugation-based autotransfusion devices primarily recover red blood cells, losing platelets and coagulation factors. The SAME autotransfusion device (i-SEP, Nantes, France) introduces an innovative filtration-based approach, recovering erythrocytes, leucocytes and platelets to enhance perioperative haemostasis. The main objective is to determine whether the filtration-based SAME device reduces significant perioperative bleeding compared with the centrifugatio

Aortic recoarctation among infants: risk factors and the significance of catheter-based intervention in the treatment.

BACKGROUND: Advances in aortic coarctation (CoA) repairs now focus on reducing restenosis and hypertension. There is debate overusing median sternotomy, which may reduce re-coarctation, versus lateral thoracotomy, which avoids bypass; anatomical factors influence the method chosen. Re-coarctation rates vary (5–30%), impacted by arch size, low weight, and age. While balloon angioplasty is less invasive, it has high restenosis risk in neonates; high-pressure balloons may help. Evidence from prior studies is limited due to short follow-up, small cohorts, and retrospective design. AIM: To identify

Surgical Explantation of an Amplatzer Device for Patent Foramen Ovale Closure in a Patient With Nickel Allergy: A Case Report.

Patent foramen ovale (PFO) closure using percutaneous devices, such as the Amplatzer occluder, is a common treatment for patients with a history of cryptogenic stroke or transient ischemic attack (TIA). Although generally well-tolerated, some patients may develop adverse reactions to the device materials, particularly in the presence of a nickel allergy. Symptoms can include chest pain, rashes, and migraines, which may necessitate surgical removal of the device. In such cases, careful surgical planning and execution are essential to ensure successful outcomes and symptom resolution. We present

Re-repair of the failed mitral valve: insights into aetiology and surgical management.

OBJECTIVES: Mitral valve (MV) repair is the gold standard for treatment of degenerative mitral regurgitation. A variety of surgical techniques allow surgeons to achieve a high rate of MV repair even with MV diseases of other aetiologies. However, a certain number of repairs fail over time. The aim of this study was to review our single-centre experience of MV re-repair and analyse the mode of repair failure, re-repair safety and efficiency in relation to the initial aetiology. METHODS: Between 1997 and 2015, 91 patients underwent redo MV re-repair. The first MV repair was performed in our ins

Complete atrioventricular block after isolated aortic valve replacement.

BACKGROUND: Temporary atrioventricular (AV) conduction disturbances are a common complication following cardiac surgery, especially involving the aortic valve. Permanent complete AV block is a serious and rare complication. Its prevalence has been estimated at 3-6% of all patients undergoing aortic valve replacement. Identification of factors that affect the occurrence of complete AV block requiring permanent pacemaker implantation might help reduce the risk of this problem in the future. AIM: To evaluate clinical, anatomical and surgical factors that might affect occurrence of complete AV, r

ABO-incompatible heart transplantation: analysis of the Pediatric Heart Transplant Study (PHTS) database.

BACKGROUND: ABO incompatible (ABOi) heart transplantation is an accepted approach to increasing organ availability for young patients. Previous studies have suggested that early survival for ABOi transplants is similar to ABO compatible (ABOc) transplants. We analyzed the Pediatric Heart Transplant Study (PHTS) database from 1/96 to 12/08 to further assess this strategy. METHODS: We analyzed the numbers of ABOi and ABOc done at the PHTS centers. We then compared the clinical characteristics, and short-term freedom from death, rejection and infection in the ABOi patients with the patients that

Staphylococcus aureus bacteremia in the surgical patient: a prospective analysis of 73 postoperative patients who developed Staphylococcus aureus bacteremia at a tertiary care facility.

BACKGROUND: Staphylococcus aureus is a frequent cause of infection and bacteremia in the postoperative patient. Unfortunately, there have been no prospective studies evaluating these patients, so the incidence of complications, subsequent treatment algorithms, and prognosis remain undefined. The objectives of this prospective study of postoperative Staphylococcus aureus bacteremia (SAB) were to define the primary sources of bacteremia and to identify the common complications of SAB in the postoperative setting. METHODS: A registry was developed into which 309 consecutive adult patients with S

Unifocalization for pulmonary atresia with ventricular septal defect and major aortopulmonary collateral arteries.

To extend the indications for corrective operation in patients with pulmonary atresia, ventricular septal defect, and major aortopulmonary collateral arteries, surgical procedures were done to unify the blood sources for pulmonary perfusion. Since December 1985, 50 patients have undergone unifocalization at ages from 2 months to 26 years with a mean of 6 +/- 7 years. In total, 84 staged unifocalization procedures and 5 other palliative procedures were done in 49 patients. These included several operative procedures: simple ligation of major aortopulmonary collateral arteries in 8; pulmonary an

Surgical Embolectomy for a Clot-in-Transit Located in a Patent Foramen Ovale: A Case Report.

Pulmonary embolism (PE) is a serious and potentially life-threatening medical condition that arises when a blood clot, typically originating in the deep veins of the legs or pelvis, travels through the bloodstream and lodges in the pulmonary arteries. This obstruction can impede blood flow to the lungs, leading to complications. PEs are a critical manifestation of venous thromboembolism, a condition characterized by the formation of blood clots within veins. The consequences of PE can vary, ranging from mild respiratory distress to severe respiratory failure or cardiac arrest, depending on the

Our 7-year experience supporting the Ross autograft with the novel technique of Personalized External Aortic Root Support.

OBJECTIVE: The Ross operation is a widely accepted option for aortic valve replacement in children, and evidence shows its excellent results in terms of hemodynamics and durability. However, indications are still limited due to the fact that it is a technically demanding procedure, only performed by specialized surgeons. On top of that, and despite numerous techniques being applied, autograft dilatation remains a key disadvantage, which can lead to graft failure. In recent years, the ExoVasc Personalized External Aortic Root Support (PEARS) has proven to be a safe and effective option to preve
