Changes in dorsal cochlear nucleus blood flow during noise exposure.

Changes in dorsal cochlear nucleus (DCN) blood flow during noise exposure were assessed using both intravital microscopy (IVM) and laser Doppler flowmetry (LDF). Mature Syrian golden hamsters were anesthetized, tracheotomized and implanted with a carotid artery catheter for monitoring blood pressure and infusion of fluorescent dye. An occipital craniectomy was performed and the cerebellum partly aspirated for access to the DCN. Fluorescent dye was infused to enhance observations of the DCN surface using a customized IVM. Red blood cell velocity, vessel diameter and blood pressure were continuo

Inhibition of post-meningitic cochlear injury with cerebrospinal fluid irrigation.

OBJECTIVE: Labyrinthitis ossificans, the pathologic ossification of the otic capsule associated with profound deafness and loss of vestibular function occurs frequently as a sequella of bacterial meningitis and subsequent purulent labyrinthitis. Experimentally, in Streptococcus pneumoniae meningitis, it has been shown that a vigorous inflammatory response to teichoic acids in the bacterial cell wall contributes to cochlear damage and subsequent fibrosis and ossification. The hypothesis of this study is that a dilution of concentration of inflammatory mediators through cerebrospinal fluid (CSF)

Comparative Analysis of Intervention With Hearing Implants.

INTRODUCTION: Hearing rehabilitation aims to enhance or restore auditory perception, which is critical for oral language development and to mitigate the cognitive and psychosocial impacts of hearing loss. Although air-conduction hearing aids represent the standard approach, they may be unsuitable for conductive or mixed hearing loss, particularly in cases of ear malformations or chronic otitis media. In such cases, bone-conduction implants, available since the 1980s, offer an alternative by bypassing the outer and middle ear. These implants, passive or active transcutaneous, percutaneous, and

Intracochlear Bleeding Enhances Cochlear Fibrosis and Ossification: An Animal Study.

The aim of this study was to investigate the effects of intracochlear bleeding during cochleostomy on cochlear inflammatory response and residual hearing in a guinea pig animal model. Auditory brainstem response threshold shifts were greater in blood injected ears (p<0.05). Interleukin-1β, interleukin-10, tumor necrosis factor-α and nitric oxide synthase 2, cytokines that are related to early stage inflammation, were significantly increased in blood injected ears compared to normal and cochleostomy only ears at 1 day after surgery; with the increased IL-1β being sustained until 3 days after th

'Asymptomatic' South Auckland preschool children have significant hearing loss and middle ear disease.

BACKGROUND: Seven hundred children were recalled for hearing screening at age 2-3 years due to a problem with their newborn hearing screen. They had all been well babies with no identified risk factors for hearing loss and hence were not scheduled for targeted follow-up to retest hearing. METHODS: There were 485 children (69%) that attended the recall. The average age was 36 months (SD 3.7). Family ethnicity was Pacific Island (36%), Asian (26%), NZ European (13%), and Māori (11%), and there was a high level of deprivation in the study population. Children were screened using distortion produ

The effect of head-related filtering and ear-specific decoding bias on auditory attention detection.

OBJECTIVE: We consider the problem of Auditory Attention Detection (AAD), where the goal is to detect which speaker a person is attending to, in a multi-speaker environment, based on neural activity. This work aims to analyze the influence of head-related filtering and ear-specific decoding on the performance of an AAD algorithm. APPROACH: We recorded high-density EEG of 16 normal-hearing subjects as they listened to two speech streams while tasked to attend to the speaker in either their left or right ear. The attended ear was switched between trials. The speech stimuli were administered eit

Long-term outcomes of speech therapy for seven adolescents with visual feedback technologies: ultrasound and electropalatography.

This follow-up study investigated the speech production of seven adolescents and young adults with hearing impairment 2-4 years after speech intervention with ultrasound and electropalatography. Perceptual judgments by seven expert listeners revealed that five out of seven speakers either continued to generalize post-treatment or maintained their level of performance post-treatment. Targets included fricatives, vowels and the rhotic /ɹ/. Speakers ranged in age from 14 to 19 years. Listeners were considered to be expert listeners. All listeners had extensive backgrounds in phonetics and phonolo

Middle ear implants: functional gain in mixed hearing loss.

UNLABELLED: Osseous atresia and chronic otitis media are diseases benefit with middle ear implants. Surgery for atresia is technically complicated, has significant number of complications and functional results are often poor. The osseointegrated hearing aids are an alternative. They provide a very good functional gain, but have many problems with the skin and osseointegration. In chronic otitis media, the ossiculoplasty solved partially the hearing problem. Unfortunately in some cases of otitis media and in open cavities fitted with conventional hearing aids the gain is unsatisfactory. AIM:

The unilateral stapes gusher.

A stapes gusher is an alarming situation occurring during stapes surgery for mixed hearing loss. It is characterized by the sudden profuse flow of cerebrospinal fluid in the middle ear and external ear canal after opening the vestibule of the inner ear. An adult patient with unilateral stapes gusher is described; he was operated on both ears with a good hearing result on both sides. The stapes on the side with the cerebrospinal fluid leak was malformed, especially in the posterior crus--a finding which may suggest the possibility of a stapes gusher. The small-hole stapedotomy is a safe techniq

Comprehensive management of bilateral acoustic neuromas. Current perspectives.

Bilateral acoustic neuromas are pathognomonic for neurofibromatosis 2. Patients with neurofibromatosis 2 present complex and challenging management problems, because growth or surgical removal of the acoustic neuroma may result in total hearing loss. Early diagnosis with gadolinium-enhanced magnetic resonance imaging and refinements in hearing preservation surgery have improved our ability to prevent total hearing loss. For patients with larger tumors or no useful hearing, the auditory brain-stem implant allows restoration of some auditory function when the tumor is removed. We describe our ma

Metagenomic analysis of the middle ear microbiome: A next-generation sequencing approach in pediatric patients with and without effusion.

OBJECTIVES: Otitis media with effusion (OME) is one of the most common causes of reversible hearing loss in childhood. In recent years, host-microbiota interactions and alterations in microbiota composition associated with health and disease have gained increasing attention in the context of OME. This study aimed to investigate the bacterial microbiota composition of middle ear cavity (MEC) samples obtained from pediatric patients with and without OME. Microbiome differences were analyzed according to clinical groups and variables such as age and gender. METHODS: 16S rRNA-based metagenomic se

Making a Difference from Day One: The Urgent Need for Universal Neonatal Hearing Screening.

Neonatal hearing screening (NHS) is a critical public health measure for early identification of hearing loss, ensuring timely access to interventions that can dramatically improve a child's language development, cognitive abilities, and social inclusion. Beyond clinical benefits, NHS provides long-term advantages in education and quality of life. Given that congenital hearing loss affects approximately 1-2 in every 1000 newborns worldwide, the case for universal screening is clear. Countries like the United States and Australia have successfully implemented NHS, leading to earlier diagnoses,

A Three-Year Follow-Up of a Patient With Large Vestibular Aqueduct Syndrome Who Underwent Bilateral Endolymphatic Duct Blockage Surgery.

Large vestibular aqueduct syndrome (LVAS) is a congenital malformation characterized by an abnormally large vestibular aqueduct, diagnosed primarily via CT scans. Patients with LVAS often experience progressive hearing loss and recurrent vertigo, with treatment strategies mirroring those for Meniere's disease. Traditional surgical interventions such as endolymphatic sac decompression (ESD) are common; however, the efficacy of endolymphatic duct blockage (EDB) as an alternative still remains under investigation. We present the case of a female in her 20s who was diagnosed with Meniere's disease

Impact of Hearing Loss Type on Linguistic Development in Children: A Cross-Sectional Study.

Background/Objectives: Hearing loss in childhood is associated with significant challenges in linguistic and cognitive development, particularly affecting language skills such as syntax, semantics, and pragmatics, which are essential for effective communication and social integration. This study aimed to analyze how different types and degrees of hearing loss impact linguistic development in children, and to identify clinical factors-such as age at diagnosis and years of language intervention-that may predict language performance. Methods: This study included a sample of 140 children aged 6 to

Deficient Recurrent Cortical Processing in Congenital Deafness.

The influence of sensory experience on cortical feedforward and feedback interactions has rarely been studied in the auditory cortex. Previous work has documented a dystrophic effect of deafness in deep cortical layers, and a reduction of interareal couplings between primary and secondary auditory areas in congenital deafness which was particularly pronounced in the top-down direction (from the secondary to the primary area). In the present study, we directly quantified the functional interaction between superficial (supragranular, I to III) and deep (infragranular, V and VI) layers of feline'

Rheumatoid arthritis in a patient with compound heterozygous variants in the COL11A2 gene and progressive hearing loss: A case report.

RATIONALE: Collagen type XI alpha 2 chain is a component of type XI collagen and is expressed in various tissues including articular cartilage and tectorial membrane of the cochlea. Variants in the COL11A2 gene, which encodes collagen type XI alpha 2 chain, has been reported to cause hearing loss and has been associated with osteoarthritis and ossification of the posterior longitudinal ligament of the spine. Despite the importance of type XI collagen in the joints, association of rheumatoid arthritis (RA) with COL11A2 has not been reported. PATIENT CONCERNS: The patient is a 60-year-old femal
