Interactive effect of family history and environmental factors on respiratory tract-related morbidity in infancy.

BACKGROUND: Family and environmental factors affect the development of respiratory morbidity. How these factors interact is unclear. OBJECTIVE: We sought to clarify the interactive effect of family history of asthma and environmental factors on the occurrence of respiratory morbidity. METHODS: Two hundred twenty-one infants with a positive family history of asthma (PFH) and 308 with a negative family history of asthma (NFH) were prenatally selected and followed until the age of 2 years. Exposure to environmental factors and the occurrence of respiratory morbidity were recorded. By using mult

[Diagnosis and therapy of laryngeal diseases in the history of medicine. I: The pre-laryngoscopic era].

BACKGROUND: ANATOMY, PHYSIOLOGY AND PATHOLOGY: Hippocrates and Aristotle did not yet have a clear idea of the anatomy and physiology of the larynx. 500 years later Galenos carried out subtle studies on animals and elaborated a fairly precise description of the different cartilages, muscles and nerves of the larynx; he was the first to demonstrate the superior and the recurrent nerves of the larynx. He performed numerous experiments on animals with temporary or permanent interruption of the nervous function and thus developed a correct opinion of the vocal physiology. However, he did not draw c

Characteristics of a new respiratory syndrome associated with the use of a humidifier disinfectant: humidifier disinfectant-related respiratory syndrome (HDRS).

OBJECTIVES: The purpose of this study was to characterize a new respiratory syndrome associated with exposure to a humidifier disinfectant (HD) in South Korea that is distinct from the well-known HD-related lung injury (HDLI). The authors identified this condition in 24 study subjects who were family members of patients with definite or probable HDLI (referred to as index cases), and were exposed to HD in the same room as the index cases. MATERIAL AND METHODS: The authors reviewed medical records of 236 family members in 110 families who were exposed to HD in the same rooms and residences as

Acquired upper airway obstruction.

Acquired upper airway obstruction is a common cause of respiratory emergencies in children. Most pathologic processes that result in upper airway compromise are a consequence of infection, trauma or aspiration. Today, many of the infectious causes of upper airway obstruction have lost their threat as a result of the progress made in preventing and treating these infections. Prompt recognition and appropriate management of the child presenting with upper airway obstruction remains critical, because certain causes can progress rapidly from a mild to a potentially life-threatening disease state.

Prevalence and clinical impact of mono- and co-infections with endemic coronaviruses 229E, OC43, NL63, and HKU-1 during the COVID-19 pandemic.

INTRODUCTION: Endemic human coronaviruses (eHCoVs) are found worldwide and usually result in mild to moderate upper respiratory tract infections. They can lead to more severe illnesses such as croup, bronchiolitis, and pneumonia in vulnerable populations. During the coronavirus disease 2019 (COVID-19) pandemic, information on HCoV prevalence and incidence and clinical impact of co-infections of HCoV with SARS-CoV-2 was lacking. OBJECTIVES: Thus, this study aimed to determine the prevalence and clinical significance of infections caused by eHCoVs during the COVID-19 pandemic in Bulgaria. METH

Health gradients in emergency visits and hospitalisations for paediatric respiratory diseases: A population-based retrospective cohort study.

BACKGROUND: Socio-economically deprived children face a disproportionate burden of respiratory diseases. The association between area-level material and social deprivation and emergency department (ED) visits and hospitalisations for paediatric respiratory diseases has not been explored. OBJECTIVES: We evaluated health inequalities in emergency department (ED) visits and hospitalisations for paediatric respiratory diseases according to material and social deprivation indexes. METHODS: This population-based retrospective cohort study deterministically linked birth, ED visits and hospitalisati

Pre-extubation ultrasonographic measurement of intracricoid peritubal free space: A pilot study to predict post-extubation airway obstruction in children.

OBJECTIVE: Post-extubation airway obstruction (PEAO) is common and difficult to predict in children. We hypothesized that Intracricoid Peritubal Free Space (IPFS) obtained by deducting the outer diameter of the endotracheal tube in situ (ODTT - provided by the manufacturer) from the ultrasonographically measured internal transverse cricoid diameter (ICDt) is likely to be inversely proportional to the risk of developing PEAO. This prospective observational study was planned to evaluate this hypothesis. METHODS: This study was conducted in a Pediatric Intensive Care Unit of a tertiary care teac

Which outcomes are important to patients and families who have experienced paediatric acute respiratory illness? Findings from a mixed methods sequential exploratory study.

OBJECTIVES: To identify the outcome priorities of parents of children who had experienced an acute respiratory infection (ARI). DESIGN: This was a two-phase, mixed methods study with a sequential exploratory design. We used a cross-sectional quantitative web-based survey to elicit parents' priorities for paediatric ARI. We then used a discussion moderated via Facebook to elucidate richer descriptions of parents' priorities. SETTING: Survey and discussion data were collected via the internet. PARTICIPANTS: 110 parents (90% women, median age 35 years, 92.7% urban dwelling, 94.5% with a postse

Evaluation of pediatric subglottic cysts.

INTRODUCTION: Subglottic cysts (SGCs) are increasingly recognized as a cause of upper airway obstruction in previously intubated infants. Endoscopic marsupialization with cold steel instruments or CO(2) laser has been reported to be the standardised treatment method for SGCs. SGC case series of 9 patients who were treated with endoscopic marsupialization with cold steel instruments or CO(2) laser and mechanical decompression with balloon dilatation are presented. PATIENTS AND METHODS: Retrospective study of 9 cases of subglottic cysts treated between 2003 and 2010 was done. Diagnoses were mad

Rapid diagnosis of Mycoplasma pneumoniae infection in children by polymerase chain reaction.

BACKGROUND AND PURPOSE: Endemic atypical pneumonia was noted in central Taiwan during 2005. The serological response to Mycoplasma pneumoniae infection was usually poor in its early course; convalescent serum was needed in most cases, which was sometimes difficult to obtain in children. Empiric antimicrobial therapy was usually initiated before serological testing. A rapid test would be useful to define the etiology and initiate appropriate management. We studied the usefulness of polymerase chain reaction (PCR) analysis for diagnosis in this setting. METHODS: This 1-year prospective study co

Acute viral lower respiratory infections in children in a rural community in Thailand.

The present study was conducted as a population based cohort in a rural community of Amphoe Takhli, Nakhon Sawan province for the determination of the prevalence of acute viral lower respiratory infection (ALRI) in pediatric cases under 5 years of age from November 1998 to February 2001. There were 472 ALRI episodes during the study period; and there were 5 cases who contracted ALRI twice. The etiologic agents were determined by indirect immunofluorescence (IIF) test using specific monoclonal antibodies for the staining of exfoliated cells in nasopharyngeal aspirate (NPA) samples. The slides o

Hospitalizations with primary versus secondary discharge diagnoses of asthma: implications for pediatric asthma surveillance.

Asthma-related hospitalizations are sentinel events in pediatric asthma surveillance. Little is known about hospitalizations that are assigned a secondary discharge diagnosis of asthma (SDDA). The National Hospital Discharge Survey (NHDS) was analyzed to compare hospitalizations with primary versus secondary discharge diagnoses of asthma. Most hospitalizations with SDDA had primary diagnoses of respiratory origin. Surveillance systems tracking the hospitalization burden of pediatric asthma should consider including selected hospitalizations with a secondary diagnosis of asthma.

Quantitative shedding patterns of respiratory syncytial virus in infants.

Quantitative shedding patterns of respiratory syncytial virus in 40 infants hospitalized with acute disease of the lower respiratory tract were determined for elucidation of the pathophysiology of infection with the virus. Nasal wash specimens were collected on admission and daily thereafter and were tested for the presence and quantities of respiratory syncytial virus. The following pattern of shedding was observed. (1) The virus was shed for prolonged periods. For the first seven days of hospitalization, 92%-100% of the infants tested continued to shed virus. At discharge 87% were still shed

Indications for intubation in laryngotracheobronchitis in black children.

Fifty-eight children with laryngotracheobronchitis (LTB) were studied. Many were malnourished, or in the post-measles state, or had some lower respiratory tract or cardiac involvement. In these patients, factors helpful in assessing the need for relief of airway obstruction by tracheal intubation have been evaluated. A set of indications for intubation, which includes clinical and arterial blood gas features of LTB is suggested. Cognisance of complications of disease, where present, has also been taken. If cyanosis or muscular hypotonia or unawareness is present, intubation is urgently needed.

Respiratory Distress in the Pediatric ED: A Case-based Self-directed Learning Module.

AUDIENCE: Emergency medicine residents, pediatric residents on an EM rotation. INTRODUCTION: Emergency medicine residents are expected to recognize and treat patients of all ages and diseases of all varieties, yet most education and training is focused on the adult patient. Exposure to pediatrics is generally integrated into training across all years of residency, but time spent in the pediatric emergency department is still a small portion of resident education. This module aims to enhance the ability of the emergency medicine residents to recognize and treat respiratory distress in children
