Quantifying the physiologic motions of the pelvic viscera during evacuation in nulligravid asymptomatic women.

INTRODUCTION AND HYPOTHESIS: Pelvic organ prolapse (POP) is often diagnosed during an in-office examination, which looks for motion of the vaginal wall while performing a strain maneuver. It is believed that the pelvic organs in adequately supported women are relatively stationary. This study was aimed at investigating the physiological displacements of pelvic organs using MR defecography. METHODS: This prospective cohort study included 19 volunteers. Midsagittal slices representing rest and the maximum movement of the posterior vaginal fornix during three maneuvers were identified. Normalize

[The Latin-American Consensus on Chronic Constipation].

The Latin-American Consensus on Chronic Constipation aimed to establish guidelines to improve the identification, diagnosis and treatment of this disorder in the region. Two coordinators and an honorary coordinator established the process and the topics to be discussed, based on a systematic review of the literature published in the previous 10 years, since 1995. Seventeen members participated with the support of their local gastroenterology societies. The members reviewed the different subjects based on the levels of evidence and grades of recommendation; the topics were then discussed in a p

No surgery for full-thickness rectal prolapse: what happens with continence?

BACKGROUND: Surgery is the only validated means of treating overt rectal prolapses, but both patients and physicians may decline or postpone the surgical approach. However, little is known on the functional outcome of nonoperated rectal prolapse. The aim of the present study was to highlight the natural history of overt rectal prolapse in patients for whom surgery was avoided or delayed. PATIENTS AND METHODS: A total of 206 patients complaining of full-thickness rectal prolapse were referred to a single institution that provided anorectal physiology for functional anorectal disorders. What causes chronic idiopathic perineal pain?

AIM: Chronic idiopathic perineal pain is poorly understood. Underlying structural abnormalities have been clinically suspected but rarely demonstrated objectively. The condition has been frequently considered to be a psychological disorder. We aimed to evaluate how commonly a structural explanation for such pain symptoms is present. METHOD: Patients seen in a pelvic floor clinic with severe chronic functional anorectal pain that was classified as chronic idiopathic perineal pain (study group) were prospectively registered in a pelvic floor database and underwent pelvic floor work up (defaecat

Anal manometric predictors of significant rectocele in constipated patients.

The diagnosis of significant rectocele is currently made on the basis of cinedefecographic findings. Clinical examination alone will only allow assessment of the presence but not the significance of a rectocele. Therefore, the aim of this study was to determine if anal manometric findings can predict the significance of a rectocele. All patients with a diagnosis of constipation and rectocele confirmed on cinedefecography between 1992 and 1998 were retrospectively reviewed. Significant rectocele was defined as the presence of three of the following five parameters: rectocele >4 cm in diameter a

A prospective audit of the usefulness of evacuating proctography.

Fifty-nine evacuating proctograms were performed over a 4 month period. We sought to identify how useful this technique is in diagnosing the cause of various anorectal symptoms and indicating which treatment option may be beneficial to the patient. The main reasons for referral were faecal soiling (60%) and obstructed defaecation (47%). Of the proctograms, 90% revealed some pathology. The most common abnormalities detected were rectocele (56%), rectal intussusception (39%), enterocele (19%) and rectal prolapse (12%). Of the patients, 45% were treated with an operation specific to the pathology

Predictive value of impaired evacuation at proctography in diagnosing anismus.

OBJECTIVE: We aimed to determine the positive predictive value of impaired evacuation during evacuation proctography for the subsequent diagnosis of anismus. SUBJECTS AND METHODS: Thirty-one adults with signs of impaired evacuation (defined as the inability to evacuate two thirds of a 120 mL contrast enema within 30 sec) during evacuation proctography underwent subsequent anorectal physiologic testing for anismus. A physiologic diagnosis of anismus was based on a typical clinical history of the condition combined with impaired rectal balloon expulsion or abnormal surface electromyogram. RESU

Robot-assisted anterior abdomino-vaginal mesh suspension for stress urinary incontinence associated with anterior compartment pelvic organ prolapse: technique, imaging workflow, and 12-month pilot outcomes.

BACKGROUND: Stress urinary incontinence (SUI) frequently coexists with pelvic organ prolapse (POP). Advanced prolapse may mask SUI, which can also develop de novo after prolapse reduction. Current management is based on standardized clinical evaluation using IUGA/ICS terminology and the POP-Q system, integrated with functional imaging such as transperineal ultrasound and dynamic pelvic floor MRI, including MR defecography when indicated. These tools allow accurate assessment of multicompartment prolapse and urethro-cervical mobility. This study describes a robot-assisted anterior abdomino-vagi

The Levator Hiatus Area detected by 3D-TPUS as an indicator of rectal prolapse severity.

PURPOSE: Rectal prolapse (RP) is a clinically significant condition with vaginal delivery as a major risk factor, especially in elderly females, needs precise evaluation for guiding treatment. Given the limitations of current diagnostic methods in terms of convenience, this study aims to develop an improved measure for RP. METHODS: A retrospective analysis of 181 female patients undergoing both dynamic three-dimensional transperineal ultrasound (3D-TPUS) and radiographic (X-ray or MRI) defecography (X-ray and MRI) was conducted to investigate the correlation between 3D-TPUS parameters and RP

Are There Sex Differences in Defecation Patterns in Patients with Defecation Disorders? - A Single-center Observational Study.

OBJECTIVES: Defecation disorders (DD) are characterized by impaired rectal evacuation due to inadequate defecatory propulsion and/or dyssynergic defecation. DD are assessed by rectal and anal pressures during attempted defecation using anorectal manometry (ARM). Thus far, at least four types of dyssynergic patterns have been recognized on ARM. This study aimed to compare the manometric parameters and dyssynergic patterns between men and women with DD. METHODS: This study enrolled consecutive patients undergoing anorectal tests for symptoms of DD. Anorectal pressure was measured using a wavefo

Abnormal Perception of Urge to Defecate: An Important Pathophysiological Mechanism in Women With Chronic Constipation.

INTRODUCTION: Although the association of absent or attenuated "call to stool" with constipation is well-recognized, no studies have systematically evaluated the perception of urge to defecate in a well-defined cohort of patients with chronic constipation (CC). METHODS: A prospective study of 43 healthy adult women and 140 consecutive adult women attending a tertiary center for investigation of CC. All participants completed a 5-day viscerosensory questionnaire, and all women with CC also underwent anorectal physiologic investigations. Normal urge perception and abnormal urge perception were

Sexual and functional outcomes after prolapse surgery: a randomized prospective comparison of trocarless transvaginal mesh and pelvic organ prolapse suspension.

PURPOSE: Stress urinary incontinence (SUI) related to pelvic organ prolapse represents a common condition that negatively impacts female sexual activity. Laparoscopic pelvic organ prolapse surgery (POPs) and the anterior repair with a trocar-less trans-vaginal mesh (TTMs) represent two different surgical techniques to treat SUI secondary to POP. This study aimed to report the results of these techniques comparing the sexual and functional outcome improvement.
