Prevalence and factors associated with percutaneous injuries and splash exposures among health-care workers in a provincial hospital, Kenya, 2010.

INTRODUCTION: Accidental occupational exposure of healthcare workers to blood and body fluids after skin injury or mucous membrane contact constitutes a risk for transmission of blood-borne pathogens. Such pathogens include Human Immunodeficiency Virus (HIV), Hepatitis B virus (HBV) and Hepatitis C virus (HCV). We conducted a study to determine the prevalence and associated factors for percutaneous injuries and splash exposures among health-care workers in Rift Valley provincial hospital. METHODS: A cross-sectional study was carried out from October to November 2010. Self reported incidents,

Multiple outbreaks of hepatitis B virus infection related to assisted monitoring of blood glucose among residents of assisted living facilities--Virginia, 2009-2011.

Between February 2009 and November 2011, the Virginia Department of Health (VDH) was notified of acute hepatitis B virus (HBV) infections occurring in residents of four separate assisted living facilities (ALFs) in the Central Health Planning Region of Virginia. In each outbreak, the initial acute HBV infections were identified through routine viral hepatitis surveillance. VDH conducted epidemiologic and laboratory investigations of these reports. Infection control practices, particularly surrounding assisted monitoring of blood glucose (AMBG), were assessed by direct observation and staff mem

Lipid abnormalities in a healthcare worker receiving HIV prophylaxis.

Healthcare workers (HCWs) worldwide risk occupational exposure to HIV and other blood-borne pathogens. Post-exposure prophylaxis (PEP) may decrease the risk of seroconversion after occupational or sexual exposure. Current guidelines recommend immediate PEP with at least 2 drugs following HIV exposure. In high-risk exposures, the guidelines recommend the use of a protease inhibitor (PI) as well as 2 reverse transcriptase inhibitors. Protease inhibitors have been associated with dyslipidaemias, other metabolic abnormalities and lipodystrophy syndromes in AIDS patients. We report a case of new tr

Planning a hepatitis C postexposure management program for health care workers. Issues and challenges.

1. Approximately 4 million individuals in the United States are infected with hepatitis C virus (HCV), many of whom are unaware of this infection. Thus, health care workers are faced with the potential for needlestick and blood exposure to another bloodborne pathogen in addition to hepatitis B and HIV. 2. The Centers for Disease Control and Prevention recommended that health care facilities follow guidelines from the U.S. Public Health Service for prevention and control of HCV infection. 3. Hospitals and other health care institutions must consider legal, ethical, and health issues and outcome

Single limb patency of polytetrafluoroethylene dialysis loop grafts maintained by traumatic fistulization.

The purpose of this report is to describe an unusual presentation of obstructive neointimal hyperplastic lesions in loop prosthetic dialysis grafts. The case histories and imaging studies of two patients with partial graft thrombosis are presented. The literature of unexpected fistulae from prosthetic dialysis grafts to adjacent veins is reviewed. Signs and symptoms that would lead a clinician to suspect the diagnosis are emphasized. There were two dialysis grafts with partial thrombosis and arterial limb patency maintained by iatrogenic fistula. These fistulae occurred from the erosion of pse

Postexposure prophylaxis for HIV.

Health care workers are at risk for human immunodeficiency virus (HIV) and other infectious pathogens through exposure to blood and body fluids. Antiretroviral medications have been prescribed for postexposure prophylaxis following occupational exposure to the HIV since the early 1990s. This practice has since been extended to nonoccupational situations, such as sexual assaults. The efficacy of prophylactic therapy may be highly time-dependent and should be initiated as soon as possible. Wound care management and referral for social, medical, or advocacy services remain important for all cases
