Infants born at term requiring mechanical ventilation suffer significant mortality and morbidity, yet few studies have tried to identify the optimum respiratory support for such infants. We, therefore, hypothesised that practice would vary, particularly between different levels of neonatal care provision. The lead clinicians of all 212 UK neonatal units were asked to complete an electronic web-based survey regarding respiratory support practices for term-born infants. Survey questions included the level of neonatal care provided, number of term-born infants ventilated per annum, initial and re

Determination of the erythrocyte sedimentation rate in capillary blood can be of value in identifying neonates with infection. The normal values for the method described here range from 1 mm/l hour at 12 hours of age to 17 mm/l hour at 14 days of age. Most noninfected neonates with moderate to severe RDS or with other serious illness had values well within the normal range. Infected patients had marked elevations, and the majority of values returned to normal with clinical improvement. Coombs-positive ABO hemolytic disease was also responsible for elevated values. In about half of the infected

IMPORTANCE: Newborn genomic sequencing (nGS) may provide health benefits throughout the life span, but there are concerns that it could also have an unfavorable (ie, negative) psychosocial effect on families.
OBJECTIVE: To assess the psychosocial effect of nGS on families from the BabySeq Project, a randomized clinical trial evaluating the effect of nGS on the clinical care of newborns from well-baby nurseries and intensive care units.
DESIGN, SETTING, AND PARTICIPANTS: In this randomized clinical trial conducted from May 14, 2015, to May 21, 2019, at well-baby nurseries and intensive care u

OBJECTIVES: Decreased fetal movements (DFM) is associated with adverse pregnancy outcomes. We aimed to look at the risk factors associated with DFM and outcomes of women who presented with and without DFM and outcomes before and after the implementation of a locally developed flow chart based on an evidence-based guideline.
STUDY DESIGN: This was a retrospective audit of 1165 women ≥ 28 weeks' gestation with a singleton pregnancy who presented with concerns regarding DFM. We compared labor and neonatal outcomes to 4706 in a control group who did not present with concerns regarding FM. We also

BACKGROUND: Congenital heart disease is one of the most common birth defects. It is not detected in some newborns until after their hospital discharge. Pulse oximetry (SpO2) screening for critical congenital heart disease (CCHD) is practiced in some settings, mainly based on evidence derived from studies done in lowland areas. This study aimed to assess the role of SpO2 screening performed before discharge in detecting CCHD in our setting (Addis Ababa) which is located at high altitude.
METHODS: Oxygen saturation of 941 apparently healthy term newborns in the nursery unit of St Paul's Hospita

BACKGROUND: Women, who have been subjected to female genital mutilation (FGM), can suffer serious and irreversible physical, psychological and psychosexual complications. They have more adverse obstetric outcomes as compared to women without FGM. Exploratory studies suggest radical change to abandonment of FGM by communities after migration to countries where FGM is not prevalent. Women who had been subjected to FGM as a child in their countries of origin, require specialised healthcare to reduce complications and further suffering. Our study compared obstetric outcomes in women with FGM to wo

BACKGROUND: Prenatal echocardiography can identify the fetus that has complex congenital heart disease and may improve early management and surgical outcome. Prenatal diagnosis may be particularly beneficial to patients who have hypoplastic left heart syndrome (HLHS) and who are at risk for hypoxic-ischemic insult at presentation.
OBJECTIVES: We sought to determine whether prenatal diagnosis reduces neurologic morbidity and operative mortality in patients who undergo palliative surgery for the HLHS.
METHODS: Data from all patients who had HLHS, except for those with lethal genetic anomalies,

Understanding how late preterm infants (34-36 completed weeks' gestation) are affected by discharge policies created for term infants (37-41 completed weeks' gestation) is essential for preventing postdischarge neonatal morbidity among late preterm infants. We analyzed linked birth certificate and hospital discharge data for Massachusetts between 1998 and 2002 to evaluate the risk of neonatal morbidity (defined as hospital readmission, observational stay, or both) between all vaginally delivered, live-born singleton late preterm and term infants. All infants were born at a Massachusetts hospit

One of the goals of Healthy People 2010 (set in 1998) was to reduce preterm birthrates from 11.6% to 7.6%. However, in 2004, the preterm birthrate of 12.5% was actually higher than the rate in 1998. Approximately 65% of this increase in prematurity rate is attributed to the increasing birthrate of the late preterm infant. Care of the late preterm infant is far more complicated than many hospital policies and clinical guidelines imply. It cannot be stressed enough to frontline clinicians that late preterm infants are not full-term infants. Their care should not be defined by the same policies a

PURPOSE: Outdoor exposure has been shown to help protect against myopia; however, low spatial frequency exposure alone cannot fully explain myopia risk. This study aimed to explore contrast polarity differences between indoor and natural environments.
METHODS: A total of 53,620 images from the Massachusetts Institute of Technology Indoor Scenes, Mapillary Vistas and Places365 databases were categorised as natural, urban or indoor (with further subdivisions). Local contrast polarity was quantified by calculating the proportions of positive-contrast (ON) and negative-contrast (OFF) pixels after

BACKGROUND: Mucocutaneous fungal infections, particularly oral and genital candidiasis, are the most common fungal diseases in neonatal populations. Although generally mild, non-invasive Candida infections can serve as sources of colonization increasing the risk of progression to invasive candidiasis, especially in preterm neonates.
OBJECTIVE DESIGN SETTING PATIENTS AND INTERVENTIONS: This study aimed to describe the infection prevention and control (IPC) measures, including contact precautions, disinfection of environment and shared devices and the review of hand hygiene protocols implemente

BACKGROUND: Foetal growth restriction (FGR) increases risks of neonatal morbidity and mortality. Identifying and preventing FGR is key to improving neonatal outcomes. FGR risk assessment tools are designed to recognise women at high risk of FGR.
AIMS: This study aims to compare the theoretical effectiveness of four different FGR risk factor assessment pathways in identifying women at risk for FGR: New South Wales (NSW) Foetal Safety Risk Assessment, Safer Baby Bundle FGR pathway, NHS Saving Babies' Lives risk assessment tool and RCOG Screening for Small Gestational Age foetus pathway. The importance of a Neonatal Intensive Care Unit (NICU) database lies in its critical role in improving the quality of care for very preterm neonates and other high-risk newborns. These databases contain extensive information regarding maternal exposures, pregnancy complications, and neonatal care. They support quality improvement (QI) initiatives, facilitate clinical research, and track health outcomes in order to identify best practices and improve clinical guidelines. The Parkland Memorial Hospital NICU database was originally part of the Maternal and Neonatal Data Acquisition, Transmission
