Region-specific evaluation of Hounsfield units on elbow computed tomography as a predictor of osteoporosis in patients at risk.

BACKGROUND: Assessment of osteoporosis is crucial for favorable outcomes after trauma surgery, but a formal dual energy X-ray absorptiometry (DXA) scan may not be available preoperatively. The aim of the present study was to assess standardized measurements of Hounsfield units (HU) on computed tomography (CT) images of the elbow as a predictor for the presence of osteoporosis. METHODS: Patients who underwent both a CT scan of the elbow and a DXA measurement within a period of two years were included. Standardized HU measurements with a diameter of 3 mm each were carried out in defined regions

The characteristics and treatment strategy for transolecranon fracture-dislocation of the elbow in children: a retrospective study.

BACKGROUND: Transolecranon fracture-dislocation of the elbow is rarely seen in children. The purpose of this retrospective study was to discuss the pathological characteristics and treatment strategy for this injury in children. METHODS: From October 2016 to March 2019, 15 patients seen and treated at our institutions for transolecranon fracture-dislocation of the elbow were identified, and their medical records and radiographs were reviewed retrospectively. There were 11 boys and 4 girls, with an average age of 8.3 years (from 5 to 14 years). The left arm was involved in 10 cases, and the ri

Three-dimensional quantitative study and functional outcome analysis of coronoid fracture in different elbow injury patterns.

BACKGROUND: Coronoid fractures are often part of a complex fracture-dislocation of the elbow. For an optimum prognosis, it is important to understand the characteristics of coronoid fractures in different elbow injury patterns. Significant differences in these characteristics have been observed in various studies, but these previous studies have suffered from limitations, such as insufficient quantification and unknown prognostic differences among different injury patterns. Therefore, we aimed to quantitatively analyze coronoid fracture characteristics and functional outcomes in different elbo

sEMG-based prediction of human forearm movements utilizing a biomechanical model based on individual anatomical/ physiological measures and a reduced set of optimization parameters.

For assistive devices such as active orthoses, exoskeletons or other close-to-body robotic-systems, the immediate prediction of biological limb movements based on biosignals in the respective control system can be used to enable intuitive operation also by untrained users e.g. in healthcare, rehabilitation or industrial scenarios. Surface electromyography (sEMG) signals from the muscles that drive the limbs can be measured before the actual movement occurs and, hence, can be used as source for predicting limb movements. The aim of this work was to create a model that can be adapted to a new us

Olecranon osteotomy fixation using a novel device: the olecranon sled.

A posterior approach to the elbow utilizing an olecranon osteotomy has been shown to provide excellent visualization of the distal humerus articular surface. However, many bony stabilization and fixation methods for the olecranon osteotomy are usually prominent, frequently symptomatic, and often require a second operation for removal. This paper evaluates the use of an innovative device, the olecranon sled, in fixation of olecranon osteotomies for exposure of intra-articular distal humerus fractures and provides follow-up results. A retrospective review of all patients with intra-articular dis

K-wire position in tension-band wiring technique affects stability of wires and long-term outcome in surgical treatment of olecranon fractures.

BACKGROUND: Tension-band wiring (TBW) has been accepted as the treatment of choice for displaced olecranon fractures. The aim of this study was to examine the effect of K-wire position on instability of the K-wires in relation to local complications and radiological and clinical long-term outcome. METHODS: We reviewed the early follow-up of 59 patients (mean age, 60 years) who underwent TBW osteosynthesis for displaced olecranon fractures. Follow-up information was available from medical records and radiographs. The main outcome measurements were proximal migration of the wires, gap, step, ra

Outcomes of Unstable Scaphoid Nonunion With Segmental Defect Treated With Plate Fixation and Autogenous Cancellous Graft.

PURPOSE: Treatment of scaphoid nonunion with a segmental defect presents a challenging clinical problem. Various techniques have been proposed, often involving structural grafting with vascularized and nonvascularized bone. The authors hypothesize that satisfactory clinical and radiographic outcomes are possible with a relatively simplified technique of volar plate fixation with autogenous, purely cancellous graft. METHODS: The authors performed a retrospective review of 34 patients with scaphoid nonunions with segmental defects, treated with plate fixation and purely cancellous bone grafting

Stabilisation of articular fractures of the distal humerus: early experience with a novel perpendicular plate system.

INTRODUCTION: The treatment of fractures of the distal humerus is difficult and involves the risk of bad functional results, particularly with articular fractures or impaired bone quality. Anatomical reconstruction and stable fixation allowing early mobilisation of the elbow are the basic prerequisites for good clinical outcomes. The aim of our study was to evaluate whether the Distal Humerus Plate (DHP, Synthes), a novel, perpendicular, fixed-angle plate system, fulfils the requirements for this treatment goal. METHODS: From 2004 to 2007, 46 consecutive patients (19 men and 21 women; mean ag

Elbow hemiarthroplasty for acute distal humeral fractures and their sequelae: medium- and long-term follow-up of 41 cases.

BACKGROUND: The earliest distal humeral hemiarthroplasty (or elbow hemiarthroplasty [EHA]) implants, which date back to the late 1940s, were performed in patients with severe elbow joint injuries as an alternative to arthrodesis. After some clinical reports and case studies with a short follow-up, published in the 1990s, a new "anatomically convertible" EHA model was introduced in 2005 and became a common surgical option to treat complex elbow fractures and their sequelae. We describe the mid- and long-term outcomes of EHA performed to treat acute intra-articular fractures or their sequelae.

Reconstruction of Monteggia-like proximal ulna fractures using different fixation devices: A biomechanical study.

BACKGROUND: Comminuted proximal ulna fractures are rare and reconstruction of these fractures is discussed controversially. The aim of this study was to test three currently available plate devices in a standardized comminuted four-part fracture model created in proximal ulna sawbones. MATERIAL AND METHODS: A standardized four-part fracture of the proximal ulna was created in 80 sawbones. Reconstruction was performed by five experienced test surgeons according to a standardized reconstruction protocol. Each surgeon reconstructed 4 fractures with a 3.5mm eight-hole reconstruction plate, 4 frac

Outcomes and challenges in the surgical treatment of trans-olecranon fracture-dislocations: A case series study.

INTRODUCTION: Transolecranon fracture-dislocations represent a complex subset of elbow injuries characterized by concomitant fractures of the olecranon, coronoid process, and radial head, often associated with posterior dislocation. These injuries pose significant surgical challenges due to their inherent instability and high risk of poor functional outcomes. Despite previous studies describing different fixation techniques, there is no standardized surgical protocol to optimize outcomes. This study aims to evaluate the clinical and functional results of a structured surgical approach in the m

Outcome of Double Tension Band Wiring Method for the Treatment of Intercondylar Fractures of Humerus.

Distal humeral fractures are relatively rare injuries that constitute approximately 2% of all fractures which occur due to high energy trauma in young males. Because they involve the articular surface and usually cause an instable elbow, surgical treatment is necessary for the recovery of elbow functions in the majority of cases. The principles of absolute stabilization and early mobilization in the elbow are of more importance than in any other joint. As the AO classification, nonunion and implant loosening is more common in type C fractures of the distal humerus involving the joint surface.

Primary Hinged External Fixation of Terrible Triad Injuries and Olecranon Fracture-Dislocations of the Elbow.

This study aimed to introduce a new treatment that apples primary hinged external fixation for complex fracture-dislocations of the elbow in 12 cases. We retrospectively assessed the functional outcomes of eight patients with terrible triad injuries and three patients with an olecranon fracture-dislocation of the elbow, who were treated at our hospital using a primary hinged external fixator between June 2012 and December 2014. Ten patients underwent repair or replacement of the radial head, while three underwent repair of the olecranon. In principle, they were treated without reconstruction o

Technique Using Isoelastic Tension Band for Treatment of Olecranon Fractures.

The isoelastic ultrahigh-molecular-weight polyethylene tension band may be considered an alternative to stainless steel wire for tension band fixation of olecranon fractures. In this article, we present our technique using this isoelastic tension band and describe the outcomes of 7 patients who underwent open reduction and internal fixation of closed, displaced olecranon fractures with minimal or no articular surface comminution. We reviewed medical records and performed physical examinations and functional assessments. Anatomical reduction was maintained in all elbows through union. Physical

Technique of olecranon bone grafting for surgical fixation of scaphoid fractures.

Currently described sources of bone graft, such as iliac crest and distal radius, for supplemental fixation of scaphoid fractures are suboptimal. In our experience, olecranon bone has the advantage of providing a convenient source of corticocancellous block graft that can be harvested within the same sterile operative field used for fixation of the scaphoid fracture, and it also causes less postoperative pain compared to that obtained from iliac crest. Here, we describe our surgical technique for harvest and use of olecranon bone graft for fixation of scaphoid fractures.

Giant cell tumour of mandible: report of a rare case.

INTRODUCTION: Elbow bony stability relies primarily on the high anatomic congruency between the humeral trochlea and the ulnar greater sigmoid notch. No practical tools are available to distinguish different morphotypes of the proximal ulna and herewith predict elbow stability. The aim of this study was to assess inter-observer reproducibility, evaluate diagnostic performance and determine responsiveness to change after simulated coronoid process fracture for three novel elbow radiographic indexes. METHODS: Ten fresh-frozen cadaver specimens of upper limbs from human donors were available for
