Avoiding "the space" by the treatment of compromised teeth.

The advent of implant therapy has given new a dimension to dentistry particularly where teeth have been irretrievably damaged or pathologically involved. There are many situations however, where endodontic or combined therapy may effectively retain compromised teeth and clinicians should carefully evaluate evidence-based treatment. The main areas discussed are: transverse and crown root fractures and invasive cervical resorption. Transverse root fractures have an excellent natural healing capacity and will also respond favourably to treatment if pulp necrosis occurs in the coronal segment. Stu

Determinants of Severe Orthodontic Treatment Need Using Clinically Categorized Occlusal Features.

Background/Objectives: Systematic assessment of orthodontic treatment needs is essential for prioritizing care in public health programs. The Dental Aesthetic Index (DAI) provides a composite severity score; however, understanding which individual occlusal traits drive severe treatment needs has a practical screening value. This study aimed to determine which occlusal features, categorized using established clinical thresholds, independently associated with severe orthodontic treatment need. Methods: Pretreatment records of 292 patients (141 men, 151 women; aged 13-42 years) were analyzed. The

Artificial Intelligence in Orthodontics: From Laboratory Benchmarks to Clinical Care.

Artificial intelligence (AI) is now used in routine orthodontic practice, not only in research, but how well it works depends heavily on the application. This narrative review synthesises peer-reviewed studies published between 2019 and early 2026 across seven task domains: cephalometric landmark detection, cervical vertebral maturation (CVM) staging, treatment-decision support for extraction and orthognathic surgery, cone-beam computed tomography (CBCT) segmentation, aligner monitoring, treatment-outcome prediction, and large language model (LLM) patient communication, which differ markedly i

An Orthodontic Study of Non-syndromic Oligodontia: An Examination of Occlusion and Occlusal Support.

The aim of this study was to analyze orthodontic data to investigate occlusal conditions, the relationship between malocclusion and the number of congenitally missing teeth, and occlusal support of maxillomandibular teeth in patients with oligodontia. The study included 66 patients with permanent dentition from two orthodontic clinics belonging to Tokyo Dental College who had received a diagnosis of oligodontia between 2003 and 2014. The materials used for the analysis comprised intraoral photographs, panoramic radiographs, and lateral cephalometric radiographs. The antero-posterior occlusal r

Surgical Removal of an Orthodontic Mini-Screw Displaced Into the Lateral Pharyngeal Space: A Case Report and Review of Pertinent Literature.

Oral and maxillofacial surgeons are often faced with the clinical challenge of foreign body displacement into the perioral tissues and soft tissues of the head and neck. This mainly occurs either because of trauma or inadvertently during dental treatment. In addition to the maxillary sinus, iatrogenic foreign body displacement during dental treatment could happen into one of the 16 distinct fascial spaces of the head and neck region. Commonly displaced foreign bodies related to dental treatment include tooth roots or fragments, local anesthetic needles, implants and restorations. The clinical

Cone-beam computed tomography evaluation of dentoskeletal changes after asymmetric rapid maxillary expansion.

INTRODUCTION: The aims of this study were to quantitatively evaluate the changes in arch widths and buccolingual inclinations of the posterior teeth after asymmetric rapid maxillary expansion (ARME) and to compare the measurements between the crossbite and the noncrossbite sides with cone-beam computed tomography (CBCT). METHODS: From our clinic archives, we selected the CBCT records of 30 patients with unilateral skeletal crossbite (13 boys, 14.2 ± 1.3 years old; 17 girls, 13.8 ± 1.3 years old) who underwent ARME treatment. [Clinical and radiological features of inflammatory collateral cysts: retrospective case series].

Based on eight clinical cases the article analyzes the main clinical and radiographic features of eight children aged 6-14 years with rare cystic lesions: juvenile inflammatory collateral cyst (ICC). ICCs were associated with incompletely erupted first (6 children) or second (2 children) permanent molars. Only in 5 cases did they manifest as pain in the area of the affected teeth, in the remaining cases they were radiographic findings during examination before orthodontic treatment. At the same time, the teeth in the projection of the cyst retained their vitality in all cases, which was confir

The assessment of untreated surface orthodontic mini-implants' osseointegration through three successive methods.

Mini-implants (MIs) with untreated surfaces are conventionally retained solely through mechanical forces, without any secondary retention mechanism involved (osseointegration). A previously reported issue is the fracture of untreated surface MIs during removal, after orthodontic treatment. Several factors, including potential osseointegration, may cause these fractures. The current research investigates the possibility of osseointegration of untreated surface MIs using three consecutive techniques: removal torque (Tq) measurement using a customized device, immediately followed by spectroscopy

Multiple, supernumerary retained teeth in the course of cleido-cranial dysplasia. A case report.

Cleido-cranial dysplasia, often referred to as Scheuthauer-Marie-Sainton syndrome, is an autosomal dominant disorder of the musculo-skeletal system. Patients with cleido-cranial dysplasia are characterized by short stature, frequent varus or valgus hip, kyphoscoliosis, underdevelopment of the scapulas and the sternum, incorrect number of ribs. The most characteristic feature is unilateral or bilateral, partial or total underdevelopment of clavicles. Mental development is not affected in this syndrome. Malocclusion, occlusal irregularities, multiple supernumerary teeth, impacted teeth, and pers

The dentist's role in facial anti-aging.

Facial aging is a dynamic process involving the aging of soft tissue and bony structures. The shape, size, and volume of the bones of the face have all been shown to change. The cosmetic community has focused on soft tissue changes, particularly changes in the skin and loss of fat volume. However, the bony components of the face are also important for overall facial 3D contour. "The bones of the face retain the capacity for remodeling at any age." Dentofacial orthopedics is the area of dentistry concerned with the supervision, guidance, and correction of the growing and mature orofacial struct

Orthodontic tooth movement: bone formation and its stability over time.

INTRODUCTION: Orthodontic tooth movement can lead to the creation of bone. The purposes of the study were to investigate the amount of bone formed in orthodontic patients during treatment (maxillary canine distalization) and retention and to assess the long-term stability of the new bone. METHODS: The sample consisted of 80 patients with 128 missing lateral incisors who were treated with distalization of the maxillary canines. They were examined at the beginning of orthodontic treatment (T1), at the end of treatment (T2), 2 years after treatment (T3A), and 5 years after treatment (T3B). The i

Treatment and postretention changes in dental arch width dimensions--a long-term evaluation of influencing cofactors.

The aim of the present long-term follow-up study of orthodontically treated patients was to analyze postretention changes in arch width dimension and to isolate factors that may serve as predictors of long-term prognosis. Pretreatment, end-of-treatment, and postretention (at least 10 years) models of 226 cases with different malocclusions were used to measure intercanine and intermolar width, arch length, sum of the mesiodistal dimension of the incisors, irregularity index, crowding, molar and canine relationship, overjet, and overbite. To assess the influence of initial and end-of-treatment a

Alveolar bone grafting in the treatment of midline alveolar cleft and diastema in incomplete median cleft lip.

Median cleft lip is a rare congenital anomaly. The wide diastema with mesial tipping observed in these patients has been largely overlooked. A midline submucosal alveolar cleft prevents adequate treatment. The purpose of this article is to describe an alveolar bone grafting (ABG) technique used in the combined surgical-orthodontic approach to diastema treatment in patients presenting with incomplete median cleft lip. Patients treated for incomplete median cleft lip and diastema were identified in the clinic registry from 1981 to 2007. Six patients were identified; 4 underwent ABG before perman

Restoration and retention of maxillary anteriors with severe root resorption.

BACKGROUND: The authors describe the treatment of three patients who had severe apical root resorption of maxillary lateral incisors caused by ectopically erupting canines. Ectopically erupting canines derive from a normal path of eruption and may cause resorption of the adjacent teeth. CASE DESCRIPTION: The authors present the three cases of severe root resorption caused by ectopically erupting canines. They then discuss various prosthetic options; the indications, advantages and disadvantages of various treatment modalities; the long-term retention and esthetic concerns of the patients; and

Intrusive luxation of permanent incisors in Norwegians aged 6-17 years: a retrospective study of treatment and outcome.

BACKGROUND/AIM: External replacement resorption (ankylosis-related) is a severe complication leading eventually to tooth loss. Little information exists regarding the influence of variables such as degree of intrusion or treatment method on the development of replacement resolution in intruded permanent incisors. The aim of this study was to report the most frequently involved age group, the preferred type of treatment, and the type and frequency of healing complications. Special attention was paid to the effect of treatment on the occurrence of replacement resorption. MATERIAL AND METHODS: F

Long-term complications and management of dental trauma in the adult patient - Part 2: discoloured, displaced and missing teeth.

The second paper in this two-part review series aims to outline the long-term complications of dental trauma in the adult patient in relation to discoloured, displaced or missing teeth. A brief overview of the cause of discolouration is outlined, followed by assessment and management options. These can include bleaching, veneers, or crown restorations to mask the discolouration. Displacement of teeth from the socket can occur due to luxation injuries. Management can include simple digital manipulation, surgical repositioning, or orthodontic extrusion based on the severity of extrusion and the

Mini-implant and Modified Nance Button Assisted Alignment of a Horizontally Impacted Maxillary Canine - A Case Report.

Orthodontic alignment of a horizontally impacted canine placed high and deep in the maxilla represents a challenging clinical scenario. This article describes a case report of a 16-year-old postpubertal male patient who was concerned about an unesthetic smile. The clinical and radiographic investigations revealed that the patient had retained deciduous canines and bilaterally impacted maxillary canines. The right impacted canine had a good prognosis. The left canine was horizontally impacted in the labial side with a Kau-Pan-Gallerano index score of 19, which indicates a "difficult" degree of

Long-term changes in pharyngeal airway morphology after mandibular setback surgery.

Long-term changes in pharyngeal airway morphology were evaluated after mandibular setback surgery in 10 women who were diagnosed as having skeletal mandibular prognathism and underwent mandibular setback surgery by bilateral sagittal split ramus osteotomy (SSRO) and orthodontic multi-bracket treatment. The subjects were assessed before treatment (T1), 3-6 months after SSRO (T2), and 2 or more years after SSRO (T3). From T1 to T2, the pharyngeal airway constricted significantly. On the other hand, from T2 to T3, the lower facial morphology showed no significant changes. The pharyngeal airway mo

Maxillary tooth transpositions: characteristic features and accompanying dental anomalies.

Transposition is a dental anomaly manifested by a positional interchange of 2 permanent teeth. The maxillary permanent canine usually transposes with the first premolar and occasionally with the lateral incisor. The records of 65 orthodontically treated individuals with maxillary tooth transpositions (40 females and 25 males) with an age range of 9 to 25 years (mean age, 13.4 years) were studied to determine the distribution of transposition and to evaluate the accompanying dental anomalies. Thirty-six individuals (55%) had a transposition of the maxillary canines and first premolars, 27 (42%)
