Advances since the paper by Zander and Glass (1949) on the pursuit of healing methods for pulpal exposures: historical perspectives.

Clinical observations and experimental studies in humans and laboratory animals have demonstrated that healing and repair of pulpal exposures by caries, trauma, or iatrogenic causes are possible with a variety of wound treatment methods. Yet clinical trials have shown that predictable long-term pulp tissue preservation may be an elusive goal and has led to doubts about pulp capping and pulpotomy as valid clinical procedures. Nevertheless substantial knowledge has accumulated over the years on the mechanisms and the treatment factors that are important to promote/support continued vital pulp fu

Approaches to vital pulp therapies.

Tooth decay, which leads to pulpal inflammation due to the pulp's response to bacterial components and byproducts is the most common infectious disease. The main goals of clinical management are to eliminate sources of infection, to facilitate healing by regulating inflammation indental tissue, and to replace lost tissues. A variety of novel approaches from tissue engineering based on stem cells, bioactive molecules, and extracellular matrix-like scaffold structures to therapeutic applications, or a combination of all these are present in the literature. Present status and future directions-Vital pulp treatment and pulp preservation strategies.

Therapeutic strategies focussed on the pulp preservation, are important when managing vital teeth with deep caries and an exposed pulp. These vital pulp treatments (VPTs); however, are not new, with indirect and direct pulp capping procedures being described as a therapy for carious teeth for over a century. As a result of unpredictable outcomes, the traditional indications for VPT particularly when the pulp was exposed were limited to the treatment of immature teeth with incomplete root formation. Over the last 20 years, the advent of regenerative endodontics and the promotion of biologically

Preclinical 3D-printed laboratory simulation of deep caries and the exposed pulp reduced student anxiety and stress, while increasing confidence and knowledge in vital pulp treatment.

AIM: To evaluate the impact of a preclinical laboratory session using 3D printed teeth on dental student stress, anxiety, confidence and knowledge when treating deep caries and pulp exposure. METHODOLOGY: This was a two-centre controlled study, with randomized distribution of students into two groups: a vital pulp treatment (VPT) lecture-only (control) group and a VPT-lecture combined with a VPT-laboratory (experimental) group. In both universities, preclinical students with endodontic or operative dentistry laboratory sessions could participate. All students were invited to the lecture. Two

Diagnosis and treatment of pulpal distress.

Both diagnosis and treatment of vital pulp tissues encompass some fairly comprehensive varieties of methods and materials. Diagnosis is arrived at in terms of probability. No one can be absolutely sure that the diagnosed degree of pulpal distress is in fact the actual state of affairs. Treatment, therefore, must be best be a probability also. There can be no guarantee that the treatment rendered will succeed in every instance. Any diagnosis and treatment of reversible pulpal distress is a gamble on the part of both practitioner and patient. However, utilization of every diagnostic means at his

Calcium silicate-based cements: composition, properties, and clinical applications.

Mineral trioxide aggregate (MTA) is a calcium silicate-based cement (CSC) commonly used in endodontic procedures involving pulpal regeneration and hard tissue repair, such as pulp capping, pulpotomy, apexogenesis, apexification, perforation repair, and root-end filling. Despite the superior laboratory and clinical performance of MTA in comparison with previous endodontic repair cements, such as Ca(OH)2 , MTA has poor handling properties and a long setting time. New CSC have been commercially launched and marketed to overcome the limitations of MTA. The aim of the present review was to explore

Exposed human pulp caused by trauma and/or caries in primary dentition: a histological evaluation.

The aim of this study was to compare the pulpal reactions after exposure due to trauma and/or caries and to suggest the appropriate treatment options for the management of pulp, in the light of the histological findings in the primary teeth. Fifteen primary maxillary incisors with pulpal exposure were examined with light microscope in a blind study. After the histological evaluation, teeth were divided into three groups for the etiological factors due to the exposure according to the patient's files: trauma group, caries group and caries and trauma group. The inflammatory infiltration was dimi

Congenital defect of maxillary primary central incisor associated with exposed pulp and gingival [fibrosis]: case report.

This report describes a rare case of hypoplastic primary incisor in which the pulp was exposed at the crown portion and covered by the gingiva in a 1-year-11-month-old boy. The patient was referred to us due to swelling of his labial cervical gingiva of the maxillary right primary central incisor, and on examination, extended to the hypoplastic labial surface. Radiographically, there was a round radiolucent area on the crown including the edge. Surgical removal of the swollen gingiva revealed a large defect of the labial aspect of the incisor, showing pulpal tissue inside. The tooth was treate

A 2-year follow-up of primary molars, pulpotomized with a gentle technique and capped with calcium hydroxide.

The prognosis for pulpotomy of primary molars with calcium hydroxide as wound dressing was evaluated clinically and radiographically. Thirty-three primary mandibular molars were amputated under the following conditions: (1) chronic coronal pulpitis, (2) the amputation was done with diamond instruments and a high-speed machine under aseptic conditions, and (3) application of calcium hydroxide in contact with the wound surface. After 1 year the treatment was regarded as successful for 22 of the teeth (67%); 11 teeth showed internal dentin resorption. After 2 years the frequency of success, allow

Clinical evaluation of dental hard tissue applications of carbon dioxide lasers.

This clinical study examined the pulpal safety of selected CO2 laser hard tissue applications. A total of 187 hard tissue procedures were performed on 54 patients. The mean period of follow-up following laser treatment was 13.5 months (range 2-24 months). The procedures included etching (n = 96), desensitizing (n = 56), laser-enhanced fluoride (n = 28), treatment of external resorption (n = 4), and pulp capping or pulpotomy (n = 3). Only the latter two procedure types were performed with anesthesia. The total irradiation received during these procedures ranged from 2 to 12 J. No patients compl

European Society of Endodontology position statement: Management of deep caries and the exposed pulp.

This position statement on the management of deep caries and the exposed pulp represents the consensus of an expert committee, convened by the European Society of Endodontology (ESE). Preserving the pulp in a healthy state with sustained vitality, preventing apical periodontitis and developing minimally invasive biologically based therapies are key themes within contemporary clinical endodontics. The aim of this statement was to summarize current best evidence on the diagnosis and classification of deep caries and caries-induced pulpal disease, as well as indicating appropriate clinical manage

Management of deep caries and the exposed pulp.

Caries prevalence remains high throughout the world, with the burden of disease increasingly affecting older and socially disadvantaged groups in Western cultures. If left untreated, caries will advance through dentine stimulating pulpitis and eventually pulp infection and necrosis; however, if conservatively managed, pulpal recovery occurs even in deep carious lesions. Traditionally, deep caries management was destructive with nonselective (complete) removal of all carious dentine; however, the promotion of minimally invasive biologically based treatment strategies has been advocated for sele

Correction of malocclusion secondary to maxillary impaction fractures using a mandibular symphyseal realignment in eight cats.

Eight cats with stable impaction fractures of the maxilla, sufficient to cause malocclusion, were treated by mandibular symphyseal realignment. The mandibular symphyseal realignment allowed occlusion to be restored with a minimal amount of dental work (e.g., enameloplasty, endodontics, or tooth extraction) in a majority of the cases. Enameloplasties were performed in two cats, one of which had a vital pulpotomy. Tooth extractions (range, two to five teeth; mean, 3.3 teeth) were performed in four cats. All cats were doing well at the time of in-hospital follow-up at one month to four years (mea

Pulp condition of successfully formocresol-treated primary molars.

The aim of the study was to assess the state of the pulp of primary teeth successfully treated by formocresol pulpotomy. Nineteen teeth, all of them with clinically and radiographically successful treatments, were extracted 3-24 months after treatment. The pulpal condition had changed in all the teeth. Inflammatory reaction or necrosis was observed in all teeth. Dentinal resorption followed by apposition of hard tissue were common findings. In the teeth with vital tissue only in the apical area, the tissue had minor or no signs of inflammation. Microorganisms could be demonstrated in the necro
