Trams--a risk factor for pedestrians.

In Gothenburg tram injuries were identified to be an important cause of traffic injuries and fatalities (48%) among pedestrians. During the summer middle-aged men, often under the influence of alcohol, were often severely injured and the injury rate was also high during the autumn. A majority (60%) of those fatally injured were under the influence of alcohol. Most injury events happened at or near a tram stop. The most serious injuries arose when the victim landed under a tram. In 1992, a runaway tram caused a major disaster, killing 10 pedestrians and injuring 30. The injury reducing measures

Exposure to road traffic and railway noise and postmenopausal breast cancer: A cohort study.

Exposure to traffic noise may result in stress and sleep disturbances. Studies on self-reported sleep duration and breast cancer risk have found inconsistent results. In a population-based Danish cohort of 29,875 women aged 50-64 years at enrolment in 1993-1997, we identified 1219 incident, postmenopausal breast cancer cases during follow-up through 2010. Mean follow-up time was 12.3 years. Road traffic and railway noise was calculated for all present and historical residential addresses from 1987 to 2010. We used Cox proportional hazard model for analyses and adjusted for hormone replacement

Modelling driver behaviour towards innovative warning devices at railway level crossings.

Improving safety at railway level crossings is costly and as funds are often limited, it is important to search for cost-effective, evidence-based solutions. The effect that the many existing alternative systems have on driver behaviour is not always known. This paper compares driver behaviour towards two novel warning devices (rumble strips and in-vehicle audio warning) at railway level crossings with two conventional warning devices (flashing light and stop sign). Regression models were developed to reflect driver's responses towards the four different types of devices based on data collecte

The experiences and perceptions of heavy vehicle drivers and train drivers of dangers at railway level crossings.

Heavy vehicle-train collisions have the potential to be catastrophic in terms of fatalities, environmental disaster, delays in the rail network, and extensive damage to property. Heavy vehicles, such as 'Road Trains' and 'B-Doubles', are vulnerable road users due to their size and mass and require specific risk management solutions. The present study aimed to capture the experiences of heavy vehicle drivers and train drivers at road-rail level crossings, with a view to exploring the contributing factors toward such accidents. A series of semi-structured focus groups was conducted, with a total

Electrical injury from subway third rails: serious injury associated with intermediate voltage contact.

BACKGROUND: Railway and subway-associated electrical trauma is rare and typically involves high voltage (> 20,000) arc injuries. Not all rail systems utilize such high voltage. We report 16 cases of electrical trauma due to 600 V direct contact with subway 'third' rails. METHODS: A case series of injured patients presenting to Shriners Burns Institute, Boston or Massachusetts General Hospital between 1970 and 1995 was retrospectively analyzed. RESULTS: A total of 16 cases was identified. Among seven subway workers, the mechanism of rail contact was unintentional by a tool, a hand or by falli

A critical assessment of studies on the carcinogenic potential of diesel exhaust.

After decades of research involving numerous epidemiologic studies and extensive investigations in laboratory animals, a causal relationship between diesel exhaust (DE) exposure and lung cancer has not been conclusively demonstrated. Epidemiologic studies of the transportation industry (trucking, busing, and railroad) show a small elevation in lung cancer incidence (relative risks [RRs] generally below 1.5), but a dose response for DE is lacking. The studies are also limited by a lack of quantitative concurrent exposure data and inadequate or lack of controls for potential confounders, particu

Physiological effects of railway vibration and noise on sleep.

This paper evaluates the relative contribution of vibration and noise from railway on physiological sleep outcomes. Vibration from railway freight often accompanies airborne noise, yet is almost totally absent in the existing literature. In an experimental investigation, 23 participants, each sleeping for six nights in the laboratory, were exposed to 36 simulated railway freight pass-bys per night with vibration alone (aWd,max = 0.0204 ms-2), noise alone (LAF,max = 49.8 dB), or both vibration and noise simultaneously. A fourth exposure night involved 52 pass-bys with concurrent vibration and n

Perievent panic attacks and panic disorder after mass trauma: a 12-month longitudinal study.

Panic attacks frequently lead to psychopathological disorders, including panic disorder. Even though panic disorder is a highly comorbid and disabling mental health problem associated with stressful life or traumatic events, perievent panic attacks and their association with panic disorder have hardly been investigated as a central topic after mass trauma. Using data from a longitudinal population-based assessment of Madrid residents after the March 11, 2004 train bombings (N = 1,589), with assessments conducted 1, 6, and 12 months after the attacks, the rate of perievent panic attacks was 10.

Atlanto-occipital disarticulation. Accident characteristics.

A retrospective study of cases of atlanto-occipital disarticulation was conducted to describe incident characteristics: 24 cases were identified, including nine motor vehicle drivers, two passengers, seven pedestrians, and five motorcyclists; one other person had fallen four stories. The highest rates were found among motorcyclists and pedestrians. Atlanto-occipital disarticulations occur in high-energy impacts and collisions and are associated with aortic laceration in 25% and basilar skull fracture in 21% of such cases. Current restraint systems and motorcycle helmets do not appear to preven

Prevalence of posttraumatic stress disorder and major depression in Ohio, Pennsylvania, and West Virginia 9 months after the East Palestine train derailment.

On February 3, 2023, a Norfolk Southern train derailed in East Palestine, Ohio, releasing toxic compounds into the surrounding air, water, and soil. Technological chemical disasters represent potentially traumatic events capable of influencing mental disorders, such as posttraumatic stress disorder (PTSD) and major depression (MD). This study reports the first comprehensive investigation of the prevalence and correlates of mental disorders among potentially exposed residents of Ohio, Pennsylvania, and West Virginia 9 months postderailment. In this cross-sectional study, a nonprobability sample

Determinants of sleepiness at work among railway control workers.

In the last two decades the control of the Portuguese railway network has become much more centralized in three centres, there integrating the functions of route flow management, electrical control and signalling. This study aimed to investigate the influence of work and individual determinants in sleepiness among railway control workers, namely socio-demographic factors, work ability, psychosocial factors, shiftwork characteristics, fatigue perception, and sleep. Sleepiness by shift was associated with quality of sleep, job satisfaction, fatigue perception, quantitative demands, and age. The

Traffic noise and cardiovascular health in Sweden: the roadside study.

Long-term exposure to traffic noise has been suggested to increase the risk of cardiovascular diseases (CVD). However, few studies have been performed in the general population and on railway noise. This study aimed to investigate the cardiovascular effects of living near noisy roads and railways. This cross-sectional study comprised 25,851 men and women, aged 18-80 years, who had resided in Sweden for at least 5 years. All subjects participated in a National Environmental Health Survey, performed in 2007, in which they reported on health, annoyance reactions and environmental factors. Questio

'Person under train' incidents from the subway driver's point of view--a prospective 1-year follow-up study: the design, and medical and psychiatric data.

UNLABELLED: From the subway driver's point of view, a 'person under train' (PUT) incident is a serious life event. This study focuses on the 1-yr consequences of such events. Follow-up was made 3 wk, 3 months and 1 yr after the event. 40 consecutive PUT subway drivers were followed. For each PUT driver, a control driver matched with regard to gender, age and country of birth was followed at identical intervals. MAIN RESULTS: the PUT group had significantly more sick days during the interval from the event to 3 wk later. During the period 3 wk to 3 months after the event no difference between

Integrated Assessment of Discrepancy Between Tracheal Tube and Tube Exchanger as Advancement: A Manikin Simulation Study.

BACKGROUND: Advancing a tracheal tube over a tracheal tube exchanger into the trachea frequently causes difficulties because of the tube impingement on laryngeal structures. In the present study, we measured the resistance of tube advancement both objectively and subjectively with a variety of combinations of tube exchanger sizes and tracheal tubes using a manikin simulator. METHODS: Lubricated 7.5 mm ID standard and Parker Flex-Tip (PFT) tracheal tubes were railroaded over the tube exchangers (OD 1-6 mm) into the trachea through the oral route in a manikin. Consequently, 12 combinations of t

Recent results on the spatiotemporal modelling and comparative analysis of Black Death and bubonic plague epidemics.

BACKGROUND: This work demonstrates the importance of spatiotemporal stochastic modelling in constructing maps of major epidemics from fragmentary information, assessing population impacts, searching for possible etiologies, and performing comparative analysis of epidemics. METHODS: Based on the theory previously published by the authors and incorporating new knowledge bases, informative maps of the composite space-time distributions were generated for important characteristics of two major epidemics: Black Death (14th century Western Europe) and bubonic plague (19th-20th century Indian subcon

Speech privacy and annoyance considerations in the acoustic environment of passenger cars of high-speed trains.

It is necessary to consider not only annoyance of interior noises but also speech privacy to achieve acoustic comfort in a passenger car of a high-speed train because speech from other passengers can be annoying. This study aimed to explore an optimal acoustic environment to satisfy speech privacy and reduce annoyance in a passenger car. Two experiments were conducted using speech sources and compartment noise of a high speed train with varying speech-to-noise ratios (SNRA) and background noise levels (BNL). Speech intelligibility was tested in experiment I, and in experiment II, perceived spe
