Artificial intelligence across the cardiovascular diagnostic pathway: a case-based narrative review.

BACKGROUND AND OBJECTIVE: Artificial intelligence (AI) is increasingly embedded across cardiovascular care, serving as a powerful adjunct for symptom assessment, bedside examination, electrocardiography, and multimodality imaging interpretation. We aim to thoroughly review current evidence for AI applications across the cardiovascular diagnostic pathway and to highlight key considerations for clinical integration. METHODS: We performed a narrative review of clinical trials and observational studies retrieved from MEDLINE/PubMed, Embase, and Google Scholar (January 1st 2000-July 10th 2025), li

Identifying pediatric heart murmurs and distinguishing innocent from pathologic using deep learning.

OBJECTIVE: To develop a deep learning algorithm to perform multi-class classification of normal pediatric heart sounds, innocent murmurs, and pathologic murmurs. METHODS: We prospectively enrolled children under age 18 being evaluated by the Division of Pediatric Cardiology. Parents provided consent for a deidentified recording of their child's heart sounds with a digital stethoscope. Innocent murmurs were validated by a pediatric cardiologist and pathologic murmurs were validated by echocardiogram. To augment our collection of normal heart sounds, we utilized a public database of pediatric h

Effectiveness of intrapartum fetal surveillance to improve maternal and neonatal outcomes: a systematic review and network meta-analysis.

BACKGROUND: Cesarean delivery is the most common surgical procedure worldwide. Intrapartum fetal surveillance is routinely offered to improve neonatal outcomes, but the effects of different methods on the risk of emergency cesarean deliveries remains uncertain. We conducted a systematic review and network meta-analysis to evaluate the effectiveness of different types of fetal surveillance. METHODS: We searched MEDLINE, Embase and CENTRAL until June 1, 2020, for randomized trials evaluating any intrapartum fetal surveillance method. We performed a network meta-analysis within a frequentist fra

Automatic adventitious respiratory sound analysis: A systematic review.

BACKGROUND: Automatic detection or classification of adventitious sounds is useful to assist physicians in diagnosing or monitoring diseases such as asthma, Chronic Obstructive Pulmonary Disease (COPD), and pneumonia. While computerised respiratory sound analysis, specifically for the detection or classification of adventitious sounds, has recently been the focus of an increasing number of studies, a standardised approach and comparison has not been well established. OBJECTIVE: To provide a review of existing algorithms for the detection or classification of adventitious respiratory sounds. T

Teaching and testing physical examination skills without the use of patients.

OBJECTIVE: To design a cardiopulmonary physical exam curriculum that does not involve the use of patients. Bedside teaching is becoming a lost art, and the use of alternative methods of instruction such as simulation has become increasingly important. Simulators have been shown to enhance physical examination skills of students and physicians in training.(1) DESCRIPTION: In 1995, a program was started to improve cardiopulmonary physical diagnosis and the teaching of auscultation at the University of Texas Medical Branch at Galveston (UTMB). The teaching manikin "Harvey" played a vital role in

Point-of-Care Ultrasound-History, Current and Evolving Clinical Concepts in Emergency Medicine.

Point-of-care ultrasound (PoCUS) has become an indispensable standard in emergency medicine. Emergency medicine ultrasound (EMUS) is the application of bedside PoCUS by the attending emergency physician to assist in the diagnosis and management of many time-sensitive health emergencies. In many ways, using PoCUS is not only the mere application of technology, but also a fusion of already existing examiner skills and technology in the context of a patient encounter. EMUS practice can be defined using distinct anatomy-based applications. The type of applications and their complexity usually depe

Blood pressure control status of patients with hypertension on treatment in Dessie City Northeast Ethiopia.

INTRODUCTION: Uncontrolled blood pressure contributes a huge contribution to many hypertension-related complications and it is one of the unbeaten problems for patients taking antihypertensive drugs. The association of social support and other factors with uncontrolled blood pressure during the covid-19 pandemic is not well investigated. Therefore, this study explored the determinants of blood pressure control status during the COVID-19 pandemic among patients with hypertension who were on an antihypertensive treatment. METHOD: A cross-sectional study was done from March to May 2021 among adu

Anesthetic Implications of Ebola Patient Management: A Review of the Literature and Policies.

As of mid-October 2014, the ongoing Ebola epidemic in Western Africa has affected approximately 10,000 patients, approached a 50% mortality rate, and crossed political and geographic borders without precedent. The disease has spread throughout Liberia, Guinea, and Sierra Leone. Isolated cases have arrived in urban centers in Europe and North America. The exponential growth, currently unabated, highlights the urgent need for effective and immediate management protocols for the various health care subspecialties that may care for Ebola virus disease patients. We conducted a comprehensive review

Accuracy of the OMRON M4 automatic blood pressure measuring device.

Despite widespread use of the automatic blood pressure (BP) measuring device OMRON M4, there is little formal validation on its accuracy. A study was conducted to assess the accuracy of the OMRON M4 compared with the true indirect BP measured by mercury sphygmomanometer (MS). A rapid method for the evaluation of automatic blood pressure measurement devices (READ) was applied for this study. The READ is based on numerous BP measurements at rest and during a standardised postural challenge in a small number of subjects who exhibit a wide range of BPs. Blood pressure measurements were done in sup

Transcranial Doppler in the Era of Personalized Medicine: An Important Tool for the Assessment of Cerebrovascular Function.

BACKGROUND: Transcranial Doppler (TCD) and transcranial color-coded duplex sonography (TCCS) have been referred to as the "brain stethoscope" as they provide real-time monitoring of both hemodynamic parameters and the structural image of the vessel and neighboring tissues. Its use has made important contributions to the field, with the ability to monitor important variables such as cerebral blood velocity to study cerebrovascular function. Although the use of B-mode with color-coded imaging allows identification of the vessel of interest in a more user-friendly approach compared to non-imaging

Technical note: a nose ring sensor system to monitor dairy cow cardiovascular and respiratory metrics.

Monitoring cardiovascular and respiratory measurements corresponds to the precision livestock farming (PLF) objective to continuously monitor and assess dairy cows' welfare and health. Changes in heart rate, breathing rate, and oxygen saturation (SpO2) are valuable metrics in human and veterinary medicine to assess stress, pain, illness, and detect critical conditions. The common way to measure heart rate is either manually or with a stethoscope. Under research conditions, heart rate is usually measured with a sports watch chest belt. Breathing rate is obtained by counting the cow's flank move

Assessment of Auscultation Skills Among Junior and Senior Healthcare Providers for Pediatric Patients: A Retrospective Study.

Background Heart murmurs are defined as whooshing or swishing sounds, unlike the normal lub-dub sounds detected by physicians while using a stethoscope. They usually develop due to numerous pathologies, with congenital defects accounting for the majority of pediatric murmurs. Few studies have addressed the difference in auscultating skills between senior and junior healthcare providers. Therefore, this study aims to collect local data on this topic as well as identify the gap between experienced and inexperienced providers in their ability to accurately detect heart murmurs. Methodology This s

Detection and localization of S1 and S2 heart sounds by 3rd order normalized average Shannon energy envelope algorithm.

Adults born after 1970s are more prone to cardiovascular diseases. Death rate percentage is quite high due to heart related diseases. Therefore, there is necessity to enquire the problem or detection of heart diseases earlier for their proper treatment. As, Valvular heart disease, that is, stenosis and regurgitation of heart valve, are also a major cause of heart failure; which can be diagnosed at early-stage by detection and analysis of heart sound signal, that is, HS signal. In this proposed work, an attempt has been made to detect and localize the major heart sounds, that is, S1 and S2. The

Incorporating simulators in a standardized patient exam.

OBJECTIVE: Using simulated patients during a clinical skills exam that involves many students has the advantage of standardizing the delivery of historical data. One major disadvantage is the inability to standardize the physical exam findings. We designed a simulated patient exam that incorporates simulated abnormal physical exam findings. DESCRIPTION: The simulated patient exam case was divided into three separate stations: (1) the simulated patient's history, (2) the simulated physical exam, and (3) the presentation station. Dyspnea was chosen as the chief complaint because of the broad di

A nurse-led bedside diagnostic model using cervical tracheal breath sounds to detect silent aspiration in dysphagia patients: A prospective diagnostic accuracy study.

BACKGROUND: Silent aspiration poses significant clinical risks in dysphagia patients, contributing to recurrent respiratory infections and aspiration pneumonia that ultimately result in elevated hospitalization rates and increased mortality. While video fluoroscopic swallowing studies (VFSS) and fiberoptic endoscopic evaluation of swallowing (FEES) remain the diagnostic gold standards, their high costs create substantial barriers to efficient large-scale screening. Conventional bedside swallowing assessments demonstrate limited sensitivity for silent aspiration detection due to inherent subjec

Machine learning-based diagnostic model for neonatal intestinal diseases in multiple centres: a cross-sectional study protocol.

BACKGROUND: Neonatal intestinal diseases often have an insidious onset and can lead to poor outcomes if not identified early. Early assessment of abnormal bowel function is critical for timely intervention and improving prognosis, underscoring the clinical importance of reducing mortality related to these conditions through rapid diagnosis and treatment. Bowel sounds (BSs), produced by intestinal contractions, are a key physiological indicator reflecting intestinal function. However, manual clinical assessment of BSs has limitations in terms of consistency and interpretative accuracy, which re

Medical Assistance in the De-Occupied Ukrainian Territory.

INTRODUCTION: The Russian invasion of Ukraine in 2022 has affected more people and destroyed a local public health facility. When some territories in Ukraine were de-occupied, national and international mobile clinics (MCs) were involved for medical assistance to local inhabitants. Knowledge about population health, medical, and humanitarian needs after they have been de-occupied has to improve planning for health system response. OBJECTIVE: The aim of this study was to summarized the MC experience at the first month after the area was de-occupied, as well as to show out-patient visits and to

Unsupervised detection and classification of heartbeats using the dissimilarity matrix in PCG signals.

BACKGROUND AND OBJECTIVE: Auscultation is the first technique applied to the early diagnose of any cardiovascular disease (CVD) in rural areas and poor-resources countries because of its low cost and non-invasiveness. However, it highly depends on the physician's expertise to recognize specific heart sounds heard through the stethoscope. The analysis of phonocardiogram (PCG) signals attempts to segment each cardiac cycle into the four cardiac states (S1, systole, S2 and diastole) in order to develop automatic systems applied to an efficient and reliable detection and classification of heartbea
