Effects of bleaching agents on human enamel light reflectance.

OBJECTIVES: Tooth whitening has been associated with splitting-up chromogenic molecules by hydrogen peroxides. Though micromorphological alterations are well documented, little is known about optical changes as a function of shifting in wavelengths. Therefore, the aim of the current study was to measure reflectance changes after bleaching in vitro by using a spectrometer. METHODS: Forty-eight enamel slabs (diameter = 5 mm) were prepared from the sound enamel of extracted human teeth that were: 1) fully impacted, 2) from juveniles ages 10 to 16 years, 3) from adults 35 to 45 years of age and 4

Stepwise excavation in a permanent molar: 17-year follow-up.

The current study presents a 17-year clinical report of stepwise excavation and indirect pulp capping in a lower right first molar, with great dentin destruction and a lack of dentin support of the cusps. At the first appointment, indirect pulp capping with calcium hydroxide and a temporary filling with zinc oxide cement were performed to minimize the risk of pulp exposure during excavation. After 45 days, the remaining carious tissue was removed and a restoration with glass-ionomer lining (Vitrebond) and resin composite (P-50) was performed. Satisfactory morphology and function of the restora

Esthetic 7/8 crown--the Tucker technique.

As restorative dentists, one of the largest challenges we face is to restore proper form and function while considering the impact on the esthetic outcome. One must consider the clinical strengths and weaknesses of various materials and their applicability in different situations. For each clinical situation, there is always a most favorable choice coordinating material and design for optimum physiological, functional and esthetic results. The cast gold 7/8 crown with esthetic buccal margin, as designed and advocated by Dr Richard V Tucker, is an extremely valuable procedure, providing a very

Occurrence and causing stimuli of postoperative sensitivity in composite restorations.

Despite improvements in composite treatments over the past decade, postoperative sensitivity still remains a problem. Therefore, this clinical study evaluated the appearance of postoperative sensitivity after composite treatments and the stimuli that may have caused it. A total of 600 teeth in 231 patients was included in this study. All treatments were performed by dental students working under close supervision following standard procedures and using the bonding system Optibond FL and the nanofilled composite Ceram X. At baseline (visit 1), the restorations were grouped according to the foll

Two-year color changes of light-cured composites: influence of different light-curing units.

This study determined color changes in a composite cured with various types of curing units after two years. A hybrid (Clearfil AP-X) composite was cured with a conventional halogen, a high intensity halogen, a plasma arc and a light emitting diode unit. The specimens were stored in light-proof boxes after the curing procedure to avoid further exposure to light and stored in 37 degrees C in 100% humidity. Colorimetric values of the specimens immediately after curing and after two years were measured using a colorimeter. The CIE 1976 L*a*b color system was used to determine color differences. D

Curing of pit & fissure sealants using Light Emitting Diode curing units.

Light Emitting Diode (LED) curing units are attractive to clinicians, because most are cordless and should create less heat within tooth structure. However, questions about polymerization efficacy have surrounded this technology. This research evaluated the adequacy of the depth of cure of pit & fissure sealants provided by LED curing units. Optilux (OP) and Elipar Highlight (HL) high intensity halogen and Astralis 5 (A5) conventional halogen lights were used for comparison. The Light Emitting Diode (LED) curing units were Allegro (AL), LE Demetron I (DM), FreeLight (FL), UltraLume 2(UL), Ultr

Biocompatibility of a flowable composite bonded with a self-etching adhesive compared with a glass lonomer cement and a high copper amalgam.

This study evaluated the pulpal response and in-vivo microleakage of a flowable composite bonded with a self-etching adhesive and compared the results with a glass ionomer cement and amalgam. Cervical cavities were prepared in monkey teeth. The teeth were randomly divided into three groups. A self-etching primer system (Imperva FluoroBond, Shofu) was applied to the teeth in one of the experimental groups, and the cavities were filled with a flowable composite (SI-BF-2001-LF, Shofu). In the other groups, a glass ionomer cement (Fuji II, GC) or amalgam (Dispersalloy, Johnson & Johnson) filled th

Effect of power density on shrinkage of dental resin materials.

This study compares volumetric changes and rates of shrinkage during different stages of polymerization of dental resin composites and compomers exposed to the same total energy by using two different combinations of power density and exposure duration. A hybrid composite and its equivalent flowable and a compomer and its equivalent flowable were exposed using a halogen curing unit set at 400 mW/cm2 for 40 seconds and 800 mW/cm2 for 20 seconds: delivering 16 J/cm2 in both cases. Volumetric changes were recorded every 0.5 seconds using a mercury dilatometer. Ten replications per test condition

A technique of occlusal adjustment for food impaction in the presence of tight proximal contacts.

Food impaction occasionally occurs in interproximal sites even though the contacts are tight. In this study of 14 patients with food impaction involving tight contacts, the lack of adequate food escape grooves was common to all 14 sites. Uneven marginal ridges and prominent opposing cusps were less common and, together, made up slightly more than half of the contact sites. An occlusal adjustment technique to create adequate food escape grooves, as well as reduce prominent opposing cusps and correct uneven marginal ridges, completely eliminated food impaction in all but one site. This site achi

The effectiveness of four-cavity treatment systems in sealing amalgam restorations.

Amalgam does not bond to tooth tissue; therefore, restorations using such material are prone to leakage despite the deposition of corrosion products. This study evaluated the effectiveness of four cavity treatment systems placed in vivo in sealing restorations of amalgam. Four cavity treatment systems were investigated in this study: Cervitec, Gluma One Bond, Panavia 21 and Copaliner Dentin Varnish and Sealant. No cavity treatment was placed in an additional group to serve as a control. The teeth were extracted within 15 minutes of restoration placement. The specimens were thermocycled (5-55 +

Pulpal inflammatory responses following non-carious class V restorations.

The effects of inflammatory activity following surgical intervention can injure pulp tissues; in severe cases it can lead to pulpal complications. With this article, the authors report on the effects of cavity preparation and restoration events and how they can interact together to reduce or increase the severity of pulpal inflammatory activity in 202 restored Class V cavities. Although some inflammatory activity was observed in the absence of bacteria, the severity of pulpal inflammatory activity was increased when cavity restorations became infected. Temperature transmission of high-output light-curing units through dentin.

Light-curing units used for polymerizing restorative resins produce heat during operation. Newer curing units with concentrating light guides or different light sources may require shorter curing times, however, the effect of such modifications on temperature transfer to the pulp is unknown. This study examined the effect of high output light-curing units on temperature transfer through resin composite and dentin. Temperature rise was measured for 40 seconds for one curing light (Optilux 401 Curing Light) with either a standard 8 mm light guide tip or a light-concentrating tip (Turbo Light Gui

Influence of different transitional restorations on the fracture resistance of premolar teeth.

Controversy exists over the most favorable material and type of restoration to be used to transitionally restore teeth destined to be crowned. This in vitro study uses fracture resistance testing to compare eight different transitional restorations in maxillary premolars. Ninety sound maxillary premolars were randomly selected and allocated to nine groups, each comprising 10 teeth. One group remained unrestored and was used as the control. Teeth in the remaining groups were prepared to a standard cavity form using: a copy milling process removing the palatal cusp. Restorations were placed usin

Foreign body gingivitis associated with a new crown: EDX analysis and review of the literature.

Gingival inflammation associated with foreign bodies in connective tissue is termed Foreign Body Gingivitis (FBG). It is not commonly recognized by clinicians and has recently been described fully in the literature. It is more common in females, and the incidence by age follows a normal distribution, unlike bacterially-induced gingivitis. Most frequently, a red or red-and-white painful, chronic lesion, it has usually been present for less than one year and does not resolve with optimization of oral hygiene. It may be clinically confused with lichen planus. There is no gingival site predilectio
