We had to revise 14 total elbow arthroplasties, three for infection, six for aseptic loosening, four for instability, and one for a failed bearing mechanism. All of the infected elbows were converted to resection arthroplasty with elimination of infection but with poor function; in one acceptable motion was achieved after a fascial arthroplasty. Those that failed aseptically were salvaged with reimplantation of other devices, either a custom implant or a semiconstrained device, which was also used in those nonconstrained elbows that failed because of dislocation. All of the noninfected elbows

OBJECTIVES: To determine outcomes of radial head replacement (RHR) for acute fractures using 3 different implant designs with or without cement fixation.
DESIGN: Retrospective.
SETTING: Tertiary referral hospital.
PATIENTS/PARTICIPANTS: One hundred fourteen elbows underwent RHR for an acute radial head fracture using either (1) a nonanatomic design and smooth stem (n = 60), (2) a nonanatomic design with a grit-blasted, ingrowth, curved stem (n = 21), or (3) an anatomic design with a grit-blasted ingrowth straight stem (n = 33). Cemented (25%) or uncemented (75%) fixation was used at the dis

BACKGROUND: Radial head hemiarthroplasty is commonly used to manage comminuted displaced fractures. Regarding implant fixation, current designs vary, with some prostheses aiming to achieve a tight "fixed" fit and others using a smooth stem with an over-reamed "loose" fit. The purpose of this study was to evaluate the effect of radial head hemiarthroplasty stem fit on radiocapitellar contact using a finite element model that simulated both fixed (size-for-size) and loose (1-, 2-, and 3-mm over-reamed) stem fits. It was hypothesized that a loose stem fit would improve radiocapitellar contact mec

BACKGROUND: Radial head arthroplasty (RHA) is a popular method of treatment for complex fractures of the radial head. The purpose of this study was to investigate patient outcomes and radiographic findings associated with a single anatomic monopolar press-fit radial head system commonly used for the treatment of radial head fractures.
METHODS: A retrospective review of prospectively collected data was performed for a consecutive series of patients treated with a press-fit anatomically designed RHA between November 2007 and April 2014. The most recent radiographs were evaluated for loosening,

Radial head fractures are often secondary to a direct axial force, such as that involved in motor vehicle accidents and falls on an outstretched hand. The Hotchkiss-modified Mason classification is an excellent assessment tool in that it provides commonly accepted direction regarding treatment. For more unstable, comminuted displaced radial head fractures that cannot be reconstructed, replacement of the radial head is warranted. The surgeon should attempt open reduction and internal fixation with restoration of the radial head in anatomical alignment for most type II and some type III fracture

This study reports our experience with total elbow replacement for fused elbows. Between 1982 and 2004, 13 patients with spontaneously ankylosed elbows were treated with a linked semi-constrained non-custom total elbow implant. The mean age at operation was 54 years (24 to 80). The stiffness was a result of trauma in ten elbows, juvenile rheumatoid arthritis in one, and rheumatoid arthritis in two. The patients were followed for a mean of 12 years (2 to 26) and were evaluated clinically using the Mayo Elbow Performance Score, as well as radiologically. A mean arc from 37 degrees of extension t

Medium-term results of the Discovery elbow replacement are presented. We reviewed 51 consecutive primary Discovery total elbow replacements (TERs) implanted in 48 patients. The mean age of the patients was 69.2 years (49 to 92), there were 19 males and 32 females (37%:63%) The mean follow-up was 40.6 months (24 to 69). A total of six patients were lost to follow-up. Statistically significant improvements in range movement and Oxford Elbow Score were found (p < 0.001). Radiolucent lines were much more common in, and aseptic loosening was exclusive to, the humeral component. Kaplan-Meier survivo

Automation of wrist rotations in upper limb prostheses allows simplification of the human-machine interface, reducing the user's mental load and avoiding compensatory movements. This study explored the possibility of predicting wrist rotations in pick-and-place tasks based on kinematic information from the other arm joints. To do this, the position and orientation of the hand, forearm, arm, and back were recorded from five subjects during transport of a cylindrical and a spherical object between four different locations on a vertical shelf. The rotation angles in the arm joints were obtained f

BACKGROUND: Adult-onset Still's disease (AOSD) is an uncommon systemic inflammatory disease, causing spiking fever, skin rash, and arthritis. Pericarditis and myocarditis are the most common cardiac manifestation of AOSD but valvular involvement is rarely reported.
CASE SUMMARY: An 18-year-old boy presented with gradually worsening shortness of breath for 6 months. There was a history of low-grade intermittent fever and polyarthralgia affecting ankles, knees, and elbows. He was in heart failure with cardiogenic and septic shock. He was managed initially with antibiotics, inotropes, and diuret

Concomitant repair of the subscapularis with reverse shoulder arthroplasty (rTSA) is controversial. To evaluate the biomechanical impact of subscapularis repair with rTSA, a cadaveric shoulder controller quantified the muscle forces required to elevate the arm during scapular abduction with the elbow flexed at 90°. The results of this study demonstrate that concomitant subscapularis repair with rTSA creates a biomechanically unfavorable condition during arm elevation. Specifically, repair of the subscapularis significantly increased the force required by the deltoid and posterior rotator cuff

BACKGROUND: We report a retrospective review of surface replacement hemiarthroplasty of the shoulder with biologic resurfacing of the glenoid, for relatively young patients suffering from advanced glenohumeral arthritis. It was hoped that the resurfacing fascia would prevent glenoid erosion for a period of time.
METHODS: This surgery was performed by two surgeons between 1996 and 2005. The indications for the arthroplasty were severe pain and limitation of function, in relatively young patients with advanced glenohumeral arthritis but an intact rotator cuff.
RESULTS: Eighteen shoulders, with

OBJECTIVE: To evaluate the clinical results and perioperative management of primary total knee arthroplasty (TKA) in hemophilic patients.
METHODS: From February 1997 to February 2006, the data of 6 total knee arthroplasty performed in 4 hemophilic patients was reviewed retrospectively. The values of coagulation factor were maintained at suitable level by monitoring the activity of the factors and their inhibitors during perioperative period. The mean follow-up time was 4.4 years, knee society score and the last postoperative radiographs were recorded.
RESULTS: After TKA, the hemophilic patie

At the International Hemophilia Center, Malmö, Sweden, which serves a large proportion of the Swedish hemophilia population, 98 orthopaedic surgical procedures were performed from 1970 to 1989 in 66 patients ranging in age from 6 to 71 years. The most common procedures were knee synovectomy, elbow synovectomy in combination with resection of the radial head, and total hip replacement. Comparing the 2 decades of the period, 3 differences were observed: a decreasing need of surgery, an increasing average age of the patients, and a change in the kinds of operations performed. Knee synovectomy and

Upper-limb amputees face tremendous difficulty in operating dexterous powered prostheses. Previous work has shown that aspects of prosthetic hand, wrist, or elbow control can be improved through "intelligent" control, by combining movement-based or gaze-based intent estimation with low-level robotic autonomy. However, no such solutions exist for whole-arm control. Moreover, hardware platforms for advanced prosthetic control are expensive, and existing simulation platforms are not well-designed for integration with robotics software frameworks. We present the Prosthetic Arm Control Testbed (Pro

Twenty-one total elbow arthroplasties using the Capitello-Condylar design of Ewald were performed at the Denver Orthopedic Clinic from 1977 to 1986. Indications for operation included intractable pain unrelieved by medical therapy as well as late state III or IV rheumatoid arthritis. The average total elbow score at 48 months after operation was 89.5 points, including excellent pain and function ratings. Modest gains in motion occurred with lesser gains in patients with juvenile rheumatoid arthritis than those of the adult variety. Forty percent of the elbows suffered some type of complication

BACKGROUND AND AIM OF THE WORK: Medial epicondyle fractures of the humerus account for 11-20% of all elbow injuries in children and in 30-55% of cases they are associated with an elbow dislocation. Undisplaced fractures are usually treated conservatively but literature is controversial regarding the treatment of displaced fractures (≥5mm) in paediatric fractures. In recent years, there is an emerging consensus that such patients may benefit more from open reduction and internal fixation. Authors report a case of a 15 years old nonprofessional soccer player who suffered of an elbow dislocation

Modifications in implant design and improvements in surgical technique have expanded the applications of total elbow arthroplasty. Complications associated with reconstructive elbow surgery persist, however, often leading to profound and sometimes nonsalvageable disability. The most recognized complications include implant loosening, periprosthetic fracture, implant failure, infection, triceps insufficiency, and nerve palsy. Although far fewer elbow arthroplasties than lower extremity arthroplasties are performed, the proportion of complications is greater with elbow arthroplasty, and the outc
