Should open Kasai portoenterostomy be performed for biliary atresia in the era of laparoscopy?

Kasai portoenterostomy has been the treatment of choice for neonates with biliary atresia since its introduction. With the advance in laparoscopic techniques, a few centers have reported the feasibility of performing laparoscopic Kasai portoenterostomy. However, the outcome of this new technique is not known. Here, we aim to evaluate, as the only referral center for liver transplantation, our experience with patients referred for transplantation after failed Kasai portoenterostomy. A retrospective study was carried out between October 1996 and September 2005. The records of all patients with t

Pancreas transplantation. A new program.

Sixteen pancreatico-duodenal transplants were performed on 15 insulin-dependent diabetics, aged 25-46, during a 20-month period beginning May 1, 1988. Fourteen patients received a combined cadaveric pancreas/renal transplant with bladder drainage. One patient received a second pancreas transplant 24 hours after the first pancreas graft failed due to portal vein thrombosis. One patient received a pancreas graft 3 years after kidney transplantation. Complications included five cases of hematuria, two bladder leaks, two wound infections, one cytomegalovirus pneumonia, three cases of graft pancrea

Closing/Closed Gastroschisis (CGS): Antenatal Predictors and Surgical Strategies in Cases of Unique Anatomy from a Case Series.

Background: Closing/closed gastroschisis (CGS) accounts for approximately 6% of gastroschisis cases globally. Currently, no consensus exists regarding: antenatal predictors of CGS types, optimal antenatal management (ultrasound screening frequency, indications for early delivery), or standardized surgical strategies tailored to CGS type (staging/timing of procedures, enterostomy necessity/level). Methods: Five neonates with CGS were enrolled and classified according to Perrone's classification: two patients with type B (40%), one with type C (20%), one with type D (20%), one patient was classi

Different results with early single-stage and staged repair in infants with ileal atresia: a two-case report on the effect of timing and treatment approach.

INTRODUCTION AND IMPORTANCE: Ileal atresia represents a major cause of newborn intestinal obstruction, requiring prompt surgical correction. Although a preferred procedure is the primary anastomosis in stable patients, the timing and surgical procedure can play a pivotal role in affecting morbidity, nutritional state, and the course of hospitalization. This article highlights the importance of these factors. CASE PRESENTATION: We describe two neonates diagnosed with isolated ileal atresia and treated using two different techniques. In case 1, early primary anastomosis was performed on day 5 o

Rare Presentation of Spontaneous Transomental Hernia in an Elderly Oncology Patient: Diagnostic Vigilance and Surgical Management.

Источник пометил публикацию как отозванную

BACKGROUND: Transomental hernia (TOH) is a rare internal hernia, representing approximately 1-4% of all internal hernia cases. The spontaneous form, occurring in patients without a history of abdominal surgery or trauma, is exceptionally uncommon and presents a significant diagnostic challenge due to its nonspecific clinical features. TOH typically involves herniation of small bowel loops through a defect in the greater omentum. CASE PRESENTATION: We report a case of spontaneous TOH in a 76-year-old male undergoing chemotherapy for synchronous primary malignancies - left lung adenocarcinoma a

Laparoscopic cyst enucleation is a promising technique for treatment of gastrointestinal tract duplications in children.

BACKGROUND: Laparoscopic cyst enucleation has its advantages and limitations in treatment of gastrointestinal tract (GIT) duplications. It allows to avoid bowel resection in such locations as ileocecal valve. We introduced laparoscopic cyst enucleation with further bowel plasty using cyst muscle layer and without monopolar coagulation, for cyst dissection we used straight scissors. MATERIALS AND METHODS: Our study included 20 children with GIT duplications, who underwent cyst enucleation from 2018 to 2021. Laparoscopy was performed in all cases with various cyst locations (stomach - 2, duoden

The International Study Group of Pancreatic Surgery definition of delayed gastric emptying and the effects of various surgical modifications on the occurrence of delayed gastric emptying after pancreatoduodenectomy.

BACKGROUND: A number of definitions have been used for delayed gastric emptying (DGE) after pancreatoduodenectomy and the reported rates varied widely. The International Study Group of Pancreatic Surgery (ISGPS) definition is the current standard but it is not used universally. In this comprehensive review, we aimed to determine the acceptance rate of ISGPS definition of DGE, the incidence of DGE after pancreatoduodenectomy and the effect of various technical modifications on its incidence. DATA SOURCE: We searched PubMed for studies regarding DGE after pancreatoduodenectomy that were publish

Intensive Nutrition Management in a Patient with Short Bowel Syndrome Who Underwent Bariatric Surgery.

Many individuals with short bowel syndrome (SBS) require long-term parenteral nutrition (PN) to maintain adequate nutritional status. Herein, we report a successful intestinal adaptation of a patient with SBS through 13 times intensive nutritional support team (NST) managements. A thirty-five-year-old woman who could not eat due to intestinal discontinuity visited Seoul National University Hospital for reconstruction of the bowel. She received laparoscopic Roux-en-Y gastric bypass (RYGB) due to morbid obesity in Jan 2013 at a certain hospital and successfully reduced her weight from 110 kg to

One-stage laparoscopy-assisted endorectal pull-through for late presented Hirschsprung's disease-Case series.

INTRODUCTION: Children with late-presenting Hirschsprung's disease (HD) are classically treated by a staged operation with enterostomy. An alternative may be one-stage laparoscopy-assisted endorectal pull-through, which has cosmetic advantages. This case-series report describes the outcomes of children with late-presenting HD who underwent this procedure. PRESENTATION OF CASES: Eight older (>3 years) children (five males, three females) underwent one-stage laparoscopy-assisted endorectal pull-through in 2010-2012. A retrospective review revealed their median age was 9.9 (range, 3.4-14) years.

Braun Enteroenterostomy in Pancreaticoduodenectomy - Association With Delayed Gastric Emptying in the Era of Enhanced Recovery.

INTRODUCTION: Pancreaticoduodenectomy (PD) is a complex and high-risk surgical procedure commonly performed to treat pancreatic and periampullary cancers. Despite its curative potential, it carries a high rate of complications, particularly delayed gastric emptying (DGE), which prolongs hospital stays, delays adjuvant therapy, increases costs, and impairs quality of life. Although enhanced recovery after surgery protocols improve outcomes, the impact of Braun enteroenterostomy (BEE) on reducing DGE within these protocols remains unclear. This study evaluated the relationship between BEE and DG

Closure of enterotomy after side-to-side ileocolic anastomosis with two barbed sutures in totally laparoscopic colectomy for right-sided colon cancer.

Good short-term outcomes of intracorporeal ileocolic anastomosis (IIA) in totally laparoscopic colectomy for right-sided colon cancer (TLRC) have been shown in many reports, but no standardized technique for enterotomy closure after stapled side-to-side ileocolic anastomosis has so far been established. We retrospectively compared the short-term outcomes between 13 consecutive patients receiving either TLRC with IIA by conventional enterotomy closure (n = 6) or closure of the enterotomy using two barbed sutures (CEBAS) (n = 7) from July 2019 to April 2020.
