HMGB1-mediated neuroinflammation: molecular mechanisms and emerging therapeutic approaches.

High mobility group box 1 (HMGB1) has emerged as a central inflammatory mediator linking cellular stress and tissue injury to sustained neuroinflammation in the central nervous system. Although originally characterized as a nuclear chromatin-binding protein, HMGB1 acquires potent pro-inflammatory activity following cytoplasmic translocation and extracellular release, where it functions as a damage-associated molecular pattern. The inflammatory actions of HMGB1 are regulated by its redox state and post-translational modifications, which determine receptor engagement and downstream signaling. Ex

Dispensary Staff Perceptions About the Benefits, Risks, and Safety of Cannabis for Medical Purposes.

BACKGROUND: Therapeutic use of cannabis is common in the United States (up to 18.7% of Americans aged ≥12), and dispensaries in the US are proliferating rapidly. However, the efficacy profile of medical cannabis is unclear, and customers often rely on dispensary staff for purchasing decisions. The objective was to describe cannabis dispensary staff perceptions of medical cannabis benefits and risks, as well as its safety in high-risk populations. METHODS: Online Survey study conducted using Qualtrics from February 13, 2020 to October 2, 2020 with a national sample of dispensary staff who repo

Epilepsy in adult X-linked adrenoleucodystrophy due to the deletion c.1415-1416delAG in exon 5 of the ABCD1-gene.

Seizures in cerebral X-linked adrenoleucodystrophy (X-ALD) more frequently occur in the early-onset compared to the late-onset form. Here we describe an adult in whom X-ALD deteriorated after head trauma and who developed epilepsy with progression of X-ALD. In a 50 year-old Caucasian male, cerebral X-ALD was diagnosed upon progressive gait disturbance, intellectual decline, elevated very-long chain fatty acids in the serum or leucocytes, cerebral MRI, showing extensive, symmetric, homogenous demyelination in the parieto-occipital areas, the splenium corporis callosum, the thalamus, the crura c

Prevalence of psychopathology in childhood epilepsy: categorical and dimensional measures.

Few studies have utilized both categorical and dimensional measures of psychopathology in children with epilepsy. We evaluated 173 children (88 males, 85 females; mean age 11.7y [SD 1.8]; range 9-14y) who had epilepsy (generalized 36%, partial 61%) for at least 6 months. The primary caregiver completed a dimensional measure, the Child Behavior Checklist (CBCL), and a categorical measure, either the Child Symptom Inventory (CSI) or the Adolescent Symptom Inventory (ASI). Correlation coefficients were computed between the CBCL scores and CSI/ASI symptom scores. For all children, diagnostic risk

Parietal Lobe Reorganization and Widespread Functional Connectivity Integration in Upper-Limb Amputees: A rs-fMRI Study.

The right parietal lobe plays an important role in body image, and disorders of body image emerge after lesions in the parietal lobe or with parietal lobe epilepsy. Body image disorder also often accompanies upper-limb amputation, in which the patient misperceives that their missing limb is still part of their body. Cortical reorganization is known to occur after upper-limb amputation, but it is not clear how widespread and to what degree functional connectivity (FC) is reorganized post-amputation, nor whether such changes might be related to misperceptions of body image. Twenty-four subjects

Effect of diet on serotonergic neurotransmission in depression.

Depression is characterized by sadness, purposelessness, irritability, and impaired body functions. Depression causes severe symptoms for several weeks, and dysthymia, which may cause chronic, low-grade symptoms. Treatment of depression involves psychotherapy, medications, or phototherapy. Clinical and experimental evidence indicates that an appropriate diet can reduce symptoms of depression. The neurotransmitter, serotonin (5-HT), synthesized in the brain, plays an important role in mood alleviation, satiety, and sleep regulation. Although certain fruits and vegetables are rich in 5-HT, it is

[Recent developments in the modeling and psychological management of persecutory ideation].

Persecutory ideas are a major clinical problem and are associated with impaired functioning, reduced compliance with medication and increased risk of hospitalization. Persecutory ideation is defined as the false conviction that others are threatening or conspiring against one. Although persecutory delusions are mainly described and experienced in schizophrenia spectrum disorders, they also occur in other neurological and psychiatric diagnoses including Alzheimer disease, epilepsy, depression, mania, dementia and post-traumatic stress disorder. Moreover, epidemiological data from general and cl

Mental health and psychosocial support in conflict: children's protection concerns and intervention outcomes in Syria.

Child protection and mental health during conflict intersects with a variety of adverse conflict-related factors, and intervention outcomes in the field are often difficult to predict. Using the casefiles of 376 school children registered in a Mental Health and Psychosocial Support (MHPSS) project in the Northwest governorate of Idleb in Syria, this study aimed to determine (i) the rates of various protection concerns (potential mental health conditions, psychosocial deprivation issues, and social, behavioural and emotional issues) for students enrolled in this project, (ii) whether the rates

Exogenous IL-33 mitigates inflammatory responses by suppressing the NF-Κb signaling pathway, promoting blood-spinal cord barrier repair and neural functional recovery.

BACKGROUND: Neuroinflammation following spinal cord injury (SCI) is a critical contributor to secondary damage. This inflammatory response disrupts the integrity of the blood-spinal cord barrier (BSCB), leading to neural tissue edema and impaired neural function. Interleukin-33 (IL-33), a tissue-derived cytokine belonging to the IL-1 family, is detectable in both tissues and blood following neural injury. While the neuroprotective effects of exogenous IL-33 have been demonstrated in models of traumatic brain injury (TBI) and epilepsy, its role in spinal cord injury remains unexplored. Therefor

Decoding Causal Associations Between Neuropsychiatric Disorders and Rotator Cuff Tendinopathy: A Two-Sample Mendelian Randomization Study.

BACKGROUND: Rotator cuff tendinopathy (RCT) is a major contributor to over 30 million surgeries which are conducted to treat shoulder overuse injuries worldwide annually. Accumulating evidence indicates that neuropsychiatric disorders (ND) share pathogenic pathways with tendinopathy. However, unclarified causal relationships between these two disease spectra undermine the individualized design of treatment strategies benefiting patients with ND while minimizing the RCT risk. We aimed to unveil whether ND were genetically associated with increased RCT occurrence by conducting a two-sample Mende

Training and Practices of Cannabis Dispensary Staff.

Introduction: The proliferation of cannabis dispensaries within the United States has emerged from patient demand for the legalization of cannabis as an alternative treatment for a number of conditions and symptoms. Unfortunately, nothing is known about the practices of dispensary staff with respect to recommendation of cannabis strains/concentrations for specific patient ailments. To address this limitation, the present study assessed the training and practices of cannabis dispensary staff. Materials and Methods: Medical and nonmedical dispensary staff (n=55) were recruited via e-mail and soc

Modulatory role of the ketogenic diet in glial scar formation after traumatic brain injury: A Fourier transform infrared, Raman, and X-ray fluorescence microscopy study.

Traumatic brain injuries (TBI) represent a significant clinical challenge, causing not only direct damage to neural tissue but also triggering secondary pathophysiological processes, including glial scar formation. While the glial scar serves a protective role, it simultaneously generates profound biochemical and elemental disturbances that may contribute to secondary neurodegeneration and the development of epilepsy. In recent years, the ketogenic diet (KD), recognized for its anticonvulsant properties, has emerged as a potential therapeutic strategy for TBI. However, despite growing interest

Neutrophil-to-lymphocyte, platelet-to-lymphocyte ratios and systemic immune-inflammation index in patients with post-traumatic stress disorder.

BACKGROUND: Low-grade systemic inflammation has been reported in many psychiatric diseases and is described as a non-severe state of the inflammatory response. Post-traumatic stress disorder (PTSD) is a chronic psychiatric disorder characterized by symptoms of avoidance, re-experiencing and hyperarousal that develop secondary to a serious traumatic event. The trauma itself creates psychological and biological changes in the individual, apart from PTSD. This complex situation has still not been clarified and researchers have tended to research on inflammatory processes. Systemic immune inflamma

NKCC1 up-regulation contributes to early post-traumatic seizures and increased post-traumatic seizure susceptibility.

Traumatic brain injury (TBI) is not only a leading cause for morbidity and mortality in young adults (Bruns and Hauser, Epilepsia 44(Suppl 10):210, 2003), but also a leading cause of seizures. Understanding the seizure-inducing mechanisms of TBI is of the utmost importance, because these seizures are often resistant to traditional first- and second-line anti-seizure treatments. The early post-traumatic seizures, in turn, are a contributing factor to ongoing neuropathology, and it is critically important to control these seizures. Many of the available anti-seizure drugs target gamma-aminobutyr

Causal effect of air pollution on the risk of brain health and potential mediation by gut microbiota.

OBJECTIVE: Epidemiologic investigations have examined the correlation between air pollution and neurologic disorders and neuroanatomic structures. Increasing evidence underscores the profound influence of the gut microbiota on brain health. However, the existing evidence is equivocal, and a causal link remains uncertain. This study aimed: to determine if there is a causal connection between four key air pollutants, and 42 neurologic diseases, and 1325 distinct brain structures; and to explore the potential role of the gut microbiota in mediating these associations. METHODS: Univariable Mendel

Cervical Sympathetic Blockade for Psychogenic Nonepileptic Seizures Associated With Post-traumatic Stress Disorder: A Case Report and Neurobiological Framework.

Psychogenic nonepileptic seizures (PNES), also termed functional seizures, are episodes that resemble epileptic seizures but occur without the abnormal ictal electrical activity characteristic of epilepsy. PNES is frequently associated with trauma exposure, post-traumatic stress disorder (PTSD), and abnormalities in autonomic regulation. Emerging evidence suggests that altered interactions among limbic, autonomic, and motor networks may contribute to symptom generation. Cervical sympathetic blockade (CSB), including stellate ganglion block techniques, has been investigated as an intervention f

Decision-Making in Meningoencephalic Herniation in the temporal bone: A report of 95 cases.

INTRODUCTION: Meningoencephalic herniation (MEH) of the temporal bone is a rare but potentially life-threatening condition that may lead to meningitis, encephalitis, cerebral abscess, or epilepsy. Its incidence appears to be increasing, particularly in spontaneous cases, which are frequently multifocal and associated with obesity and benign intracranial hypertension. MATERIALS AND METHODS: A retrospective review was performed of 88 patients (95 cases) surgically treated for MEH at a tertiary referral center between January 2000 and February 2023. Inclusion criteria were intraoperatively confi
