Comparative evaluation of effect of N-acetyl cysteine, maleic acid, and ethylenediaminetetraacetic Acid on the depth of dentinal tubule penetration of an epoxy resin-based root canal sealer: A confocal laser scanning microscopy study.

BACKGROUND: Effective root canal irrigation removes the smear layer for optimal sealer penetration. While 17% ethylenediaminetetraacetic Acid (EDTA) is effective, concerns about dentin erosion exist. Alternatives like 7% maleic acid (MA) and 20% N-acetylcysteine (NAC) show promise with fewer adverse effects. AIM: To compare the effects of 20% NAC, 7% MA, and 17% EDTA as final irrigating solutions on the depth of sealer penetration into dentinal tubules at coronal, middle, and apical thirds of root canals using confocal laser scanning microscopy (CLSM). MATERIALS AND METHODS: Sixty-six single

The ability of thiourea to scavenge hydrogen peroxide and hydroxyl radicals during the intra-coronal bleaching of bloodstained root-filled teeth.

BACKGROUND: Hydrogen peroxide, an agent used in the intra-coronal bleaching of root-filled teeth for over a century, has been shown to diffuse from the pulp chamber to the outer root surface. Furthermore, it has been demonstrated that destructive hydroxyl radicals, the by-products of the bleaching process, have been detected on the external root surface. The control of such diffusion may be of importance in minimizing the risk of invasive cervical resorption (ICR) which has been linked to intra-coronal bleaching of discoloured root-filled teeth using hydrogen peroxide. The aims of the present

Nonsurgical management of a nonvital tooth with orthodontically induced external root resorption and extensive periapical pathology.

The purpose of this article is to report a rare case of orthodontically induced external root resorption of a maxillary left central incisor with a large periapical radiolucency; it was treated successfully with a conservative approach that included the use of calcium hydroxide. The tooth was nonvital and had grade III mobility. It was treated endodontically for 2 years with an interim filling of calcium hydroxide mixed with propylene glycol. After 2 years, the postoperative intraoral periapical radiograph showed good periapical healing with complete resolution of periapical radiolucency. Ther

Reattachment of subgingivally fractured central incisor with an open apex.

A case report of a 6-year-old girl with a fractured maxillary left central incisor with an open apex is presented. The procedure used to repair the fracture included flap surgery with an intrasulcular incision and endodontic treatment. The patient was called for 3 months regular follow-up to check the root formation. At the end of 32 months just before the root was obturated by guttaperka, she fractured the same tooth. Flap surgery was repeated and the tooth was restored. The root canal was obturated with a root filling paste and guttaperka as the apex was closed. Examination 10 months after t

Influence of Er:YAG laser irradiation on apical sealing of four different sealers.

The sealing of the root canal system is of fundamental importance for successful endodontic treatment. To obtain an adequate apical seal, many factors must be considered such as the presence of smear layer and the sealer applied. After canal preparation, this layer must be removed because it prevents close contact between the dentinal walls and the sealing material. The goal of this study was to evaluate the sealing ability of four different sealers after smear layer removal with either 17% EDTA-T irrigation or Er:YAG laser irradiation of 46 teeth. The canals were sealed with four different se

The effect of chlorhexidine gluconate as an endodontic irrigant on the apical seal: long-term results.

The purpose of this study was to determine whether chlorhexidine gluconate (0.12%), used as an endodontic irrigating solution, would affect the apical seal of three root canal cements. One hundred, extracted, human, single-canal teeth were divided into 9 experimental groups of 10 teeth each, in addition to a positive and negative control group of 5 teeth each. The teeth were decoronated at the level of the CEJ, accessed, instrumented to a Master apical file #50, irrigated with either sterile saline, 5.25% NaOCl, or 0.12% chlorhexidine gluconate, and dried using paper points. Obturation was acc

Maxillary premolar with 4 separate canals.

INTRODUCTION: Recognition of anatomic variations is a challenge for clinicians regardless of which tooth is treated. Maxillary premolars usually have 2 root canals, but the presence of 3 distinct root canals has been reported in 1%-6% of cases. METHODS AND RESULTS: This report describes the case of a maxillary right second premolar with 4 separate canals: 1 mesial, 1 palatal, and 2 distal canals. This was confirmed by using cone-beam computed tomography and was successfully treated with rotary files and obturated by using a warm vertical compaction technique. CONCLUSIONS: The clinical signif

The influence of different factors on the survival of root canal fillings: a 10-year retrospective study.

The aim of the present study was to determine the survival time of root canal fillings performed in 1990 and 1991 at the Dental School, Philipps University of Marburg, Germany. Data were collected retrospectively from the dental records and control radiographs were evaluated. The survival probability was determined by Kaplan-Meier analysis. Intergroup differences were verified with the log-rank test. The 914 evaluated root canal fillings yielded an overall 10-yr cumulative survival probability of 0.74. The factors baseline periapical condition (p < 0.001), length (p < 0.001), condensation (p <

Successful inferior alveolar nerve decompression for dysesthesia following endodontic treatment: report of 4 cases treated by mandibular sagittal osteotomy.

Endodontic overfilling involving the mandibular canal may cause an injury of the inferior alveolar nerve (IAN) resulting in disabling sensory disturbances such as pain, dysesthesia, paresthesia, hypoesthesia, or anesthesia. Two fundamental mechanisms are responsible for the injury: the chemical neurotoxicity and the mechanical compression caused by the extruded material. Although spontaneous resorption has been described for some materials, early surgical exploration with removal of the material and decompression of the IAN should be performed, irrespective of the material used, given that the

Single-session use of mineral trioxide aggregate as an apical barrier in a case of external root resorption.

External root resorption may occur as a consequence of trauma, orthodontic treatment, bacterial infection or incomplete sealing of the root canal system (bacterial re-infection), and lead to crater formation on the resorbed apex. This would deform the root apex surface, and cause loss of apical constriction. Depending on the extent of the resorptive process, different treatment regimens have been proposed. A 34-year-old male patient presented with an intra-radicular retainer and an inadequate filling on tooth #21, as well as a radiographic image suggesting periapical bone rarefaction. After ro

The unpredictability of seal after post space preparation: a fluid transport study.

A root canal filling remaining after post space preparation is commonly expected to provide adequate seal. Coronal leakage of 30 endodontically treated teeth was measured before post space preparation using a fluid transport assay. In 10 of these teeth post space was prepared, using a two-step procedure, first to a remaining filling of 6 mm and then to 3 mm, with the leakage studied after each step. In 10 teeth the removal was done in one step to a remaining length of 3 mm. The other 10 teeth, with intact root canal fillings, served as controls and were tested twice for leakage. A significant

Measuring bone density in healing periapical lesions by using cone beam computed tomography: a clinical investigation.

INTRODUCTION: Differences in bone density before and after endodontic treatment were examined in teeth with periapical lesions in Hounsfield units (HUs) by using cone beam computed tomography (CBCT). METHODS: Sixteen patients requiring endodontic treatment for periapical lesions underwent CBCT scan before and after treatment. Their bone densities were calculated in HUs. The study included 16 lesions measuring 8-10 mm in diameter. HU measurements were taken from an area of 2.25 mm(2) on the CBCT images where the bone density was lowest, before and after treatment. RESULTS: The initial and fin
