Two patients with severely displaced maxillary canines respond differently to treatment.

The patient with a severely displaced maxillary canine continues to challenge the orthodontic specialist. When the two patients in the following case reports presented, their problems seemed similar: each had a palatally displaced canine. However, unique clinical problems were soon diagnosed that led to the development of different treatment approaches. These differences are worth discussing, especially in light of newly published studies. In planning treatment for these two patients, the following questions are worth considering. 1. Is it common for other dental anomalies (small lateral incis

Alveolar bone grafting in the treatment of midline alveolar cleft and diastema in incomplete median cleft lip.

Median cleft lip is a rare congenital anomaly. The wide diastema with mesial tipping observed in these patients has been largely overlooked. A midline submucosal alveolar cleft prevents adequate treatment. The purpose of this article is to describe an alveolar bone grafting (ABG) technique used in the combined surgical-orthodontic approach to diastema treatment in patients presenting with incomplete median cleft lip. Patients treated for incomplete median cleft lip and diastema were identified in the clinic registry from 1981 to 2007. Six patients were identified; 4 underwent ABG before perman

Restoration and retention of maxillary anteriors with severe root resorption.

BACKGROUND: The authors describe the treatment of three patients who had severe apical root resorption of maxillary lateral incisors caused by ectopically erupting canines. Ectopically erupting canines derive from a normal path of eruption and may cause resorption of the adjacent teeth. CASE DESCRIPTION: The authors present the three cases of severe root resorption caused by ectopically erupting canines. They then discuss various prosthetic options; the indications, advantages and disadvantages of various treatment modalities; the long-term retention and esthetic concerns of the patients; and

Single Nucleotide Polymorphisms in COX2 Is Associated with Persistent Primary Tooth and Delayed Permanent Tooth Eruption.

Persistent primary tooth (PPT) is a prevalent clinical condition that occurs when a primary tooth is over-retained beyond the established period of its normal exfoliation time, remaining in the oral cavity. Many factors could be involved in the risk of PPT; therefore, the aim of this study was to evaluate if single nucleotide polymorphisms (SNPs) in the COX2 gene are associated with PPT. Children undergoing orthodontic treatment were screened. Orthopantomographs were assessed to evaluate PPT according to the Nolla stage of its permanent successor. The primary tooth was considered retained when

From Child to Adulthood, a Multidisciplinary Approach of Multiple Microdontia Associated with Hypodontia: Case Report Relating a 15 Year-Long Management and Follow-Up.

Oral rehabilitation of patients presenting multiple microdontia is a real therapeutic challenge. These alterations in size, often associated with other dental anomalies, have aesthetic and functional repercussions for patients and can lead to significant psycho-social consequences. We report here the case of an 11-year-old patient with bilateral sectorial microdontia and agenesis of teeth numbers 13 and 23. She also presented staturo-ponderal delay and a history of acute coronary syndrome with a lower coronary occlusion of unknown aetiology. At first, additive coronoplasties and an orthodontic

Total Mandibular Subapical Alveolar Osteotomy to Correct Class II Division I Dentofacial Deformity.

INTRODUCTION: Class II division I malocclusions are the most common dentofacial deformities seen in clinical practice. Severe cases or cases in which growth has ceased may require full correction combining orthodontic and surgical treatment. We report a case of a total mandibular subapical alveolar osteotomy, performed to correct a class II division I dentofacial deformity. CASE REPORT: A 19-year-old female patient was referred to the oral and maxillofacial surgery department at the Federal University of Paraná with chin aesthetic complaints as well as class II malocclusion. The proposed trea

Cleidocranial dysplasia case report: remodeling of teeth as aesthetic restorative treatment.

Cleidocranial dysplasia (CCD), is an autosomal dominant disorder with a prevalence of 1 in 1,000,000 individuals. It is generally characterized by orofacial manifestations, including enamel hypoplasia, retained primary teeth, and impacted permanent and supernumerary teeth. The successful treatment involving a timing intervention (orthodontic-maxillofacial surgeons-restorative) is already described. However, the restorative treatment might improve the aesthetic final result in dentistry management for patients with cleidocranial dysplasia. Objective. Therefore, this clinical report presents a c

Non-invasive, continuous oral delivery of solid levodopa-carbidopa for management of Parkinson's disease.

When short plasma half-life drugs act only briefly, they require frequent or continuous administration. We report the engineering of a non-invasive oral drug delivery system for long-term, continuous administration of these drugs. Their non-invasive, long-term, continuous administration at daily doses exceeding 100 mg has, for many years, been considered an insurmountable challenge. We show that over 1200 mg/day of 4:1 levodopa-carbidopa (LD-CD) can be non-invasively and continuously extruded when formulated as a semisolid paste, loaded with 63%w/w of the solid drugs. The drug delivery system

Restorative dentistry clinical decision-making for hypodontia: peg and missing lateral incisor teeth.

Peg-shaped and missing lateral incisor teeth are common features for patients affected by hypodontia. While improvements in dental appearance may be a strong motivating factor for these patients, providing dental treatment to improve the clinical condition and achieve an acceptable and stable outcome can be complex and lengthy. For patients affected by hypodontia, discussion and consideration of various approaches to their individual treatment are best achieved in a multidisciplinary team environment. This allows debate of options and joint agreement between at least orthodontic and restorative

Single-session use of mineral trioxide aggregate as an apical barrier in a case of external root resorption.

External root resorption may occur as a consequence of trauma, orthodontic treatment, bacterial infection or incomplete sealing of the root canal system (bacterial re-infection), and lead to crater formation on the resorbed apex. This would deform the root apex surface, and cause loss of apical constriction. Depending on the extent of the resorptive process, different treatment regimens have been proposed. A 34-year-old male patient presented with an intra-radicular retainer and an inadequate filling on tooth #21, as well as a radiographic image suggesting periapical bone rarefaction. After ro

Fluoride release from a light-cured bonding material in openbite orthodontic patients.

This study was undertaken to clarify whether the salivary fluoride concentration on the anterior and premolar teeth, released from an orthodontic bonding material, differed between openbite and non-openbite patients. Using fluoride-releasing bonding materials, brackets were bonded to the teeth in openbite and in non-openbite patients. Saliva films on the tooth surfaces near the brackets were collected with filter paper and fluoride concentrations in the saliva were determined by using a fluoride electrode technique. Saliva was collected and analyzed at different periods after the bonding. Fluo

Extraction as a treatment alternative follows repeated trauma in a severely handicapped patient.

Handicapped patients with protruding maxillary incisors are prone to repeated dental trauma. A 13-year-old girl with cerebral palsy, severe mental retardation and seizure disorder was referred to our department for restoring the traumatized anterior teeth. Despite drug combination, the frequency of seizure attack was around 10 times a month. The oral examination showed multiple caries, gingival hyperplasia, class II malocclusion with 14 mm overjet and deep overbite. During the first 3 years of a 7-year follow-up period, six episodes of anterior tooth trauma due to seizure attack occurred. The

Endosseous titanium implants during and after orthodontic load--an experimental study in the dog.

An experiment was carried out to determine whether endosseous titanium implants (Brånemark) retain their clinical stability throughout a 26-week period of continuously applied force (2 N) and what kind of marginal peri-implant bone changes occur in the process. For this purpose 6 premolars (P3: mandible/maxilla and P4: mandible) were extracted from each of 2 foxhounds, and 12 implants (6 per dog) were positioned in the edentulous maxillary and mandibular areas after the alveolar healing period. Following a 25-week implant healing period, 8 fixtures (P3 areas) were used as anchoring elements fo

The displaced maxillary canine--a retrospective study.

OBJECTIVE: The re-alignment of retained maxillary canines was studied in relation to the degree of displacement in panoramic radiographs, mesiodistal root deviations, length of treatment and side-effects. The aim was then to determine whether the decision between surgical removal and orthodontic re-alignment can be made on the basis of panoramic radiographs alone. MATERIALS AND METHODS: Forty-seven panoramic radiographs with 59 displaced canines were used to determine their distance and inclination to the occlusal plane (similar to the method described by Dausch-Neumann [8]), the position of
