Anatomy of the medial femoral circumflex artery and its surgical implications.

The primary source for the blood supply of the head of the femur is the deep branch of the medial femoral circumflex artery (MFCA). In posterior approaches to the hip and pelvis the short external rotators are often divided. This can damage the deep branch and interfere with perfusion of the head. We describe the anatomy of the MFCA and its branches based on dissections of 24 cadaver hips after injection of neoprene-latex into the femoral or internal iliac arteries. The course of the deep branch of the MFCA was constant in its extracapsular segment. In all cases there was a trochanteric branch

Visual outcome and rehabilitation in a case of bilateral acanthamoeba sclerokeratitis.

PURPOSE: To report a rare and complicated case of bilateral Acanthamoeba sclerokeratitis (ASK). METHODS: Case report of a 42-year-old patient who was referred to the Massachusetts Eye and Ear Infirmary 7 months after a history of culture-confirmed bilateral Acanthamoeba polyphaga sclerokeratitis. At the time of referral to the MEEI, he was noted to have iris neovascularization and chronic mydriasis in both eyes. The anterior chambers were shallow, and there were mature cortical cataracts that obscured the posterior pole in both eyes. His visual acuity in both eyes was limited to hand motions.

[Arthrofibrosis].

Arthrofibrosis represents a severe complication in joints after trauma and surgery, with loss of motion due to an excessive fibrotic response in the repair process. Patients with primary arthrofibrosis suffer from a general fibrotic healing response after injury or surgery, while patients with secondary arthrofibrosis exhibit loss of motion due to a local fibrotic healing response. Nonisometric positioning of cruciate ligament grafts, notch impingement, or hardware problems may lead to secondary arthrofibrosis. In contrast, the etiology of primary arthrofibrosis is still unknown. There are alt

Vogt-Koyanagi-Harada disease recurrence after laser treatment: A case report.

This study reports two cases of Vogt-Koyanagi-Harada disease recurrence after laser treatment. Case 1 involved a woman in her late 70s with Vogt-Koyanagi-Harada disease that was controlled by adalimumab and steroid eye solution therapy. She presented with uveitis recurrence 4 weeks after undergoing neodymium:yttrium aluminum garnet laser posterior capsulotomy in both eyes. She was immediately treated with sub-Tenon triamcinolone acetonide injection in both eyes, which provided palliative relief. During the following year, she experienced further recurrences every 3-5 months. Case 2 involved a

How do intraocular lens materials influence the outcome of cataract surgery?

PURPOSE OF REVIEW: This review summarizes the evidence on the effect of intraocular lens (IOL) material on the outcomes of cataract surgery, as well as on the surgical procedure itself. RECENT FINDINGS: Differences in capsular biocompatibility between IOL materials lead to variations in capsular stability and posterior capsule opacification (PCO), while differences in uveal biocompatibility affect postoperative inflammatory response. SUMMARY: Refractive outcomes are affected by both incision size and the rotational stability of toric IOLs. Small incision sizes favour hydrophilic IOLs. Rotati

Corneal Opacification and Spontaneous Recovery following Injection of Healon5 into the Corneal Stroma during Intervention for Postoperative Hypotony.

We report the self-limited nature of corneal opacification after accidental injection of Healon5 into the corneal stroma. A 52-year-old male with a new diagnosis of severe stage, primary open-angle glaucoma underwent successful trabeculectomy OS, which was complicated by ocular hypotony and shallow anterior chamber (AC) on postoperative day 1. Healon5, a hyaluronic acid-containing viscoelastic device, was accidentally introduced into the corneal stroma during attempted injection into the AC. The cornea hydrodissected and opacified, leading to precipitous loss of best-corrected visual acuity (B

Guided growth for correction of knee flexion deformity: a series of four cases.

Fixed knee flexion deformity can present as an insidious and significant problem in diverse etiologies, most commonly in cerebral palsy. Traditional surgical intervention has included posterior capsulotomy and supracondylar femoral osteotomy, both of which carry significant associated morbidity and risks. In the skeletally immature patient, guided growth may be used to correct or substantially diminish the deformity. We are presenting our early experience encompassing four subjects who completed instrumented gait analysis both prior to and after distal femoral anterior guided growth (hemiepiph

[The synthesis of cytokines by human lens epithelial cells--interleukin 1 (IL-1), tumor necrosis factor (TNF) interleukin 6 (IL-6), and epidermal growth factor (EGF)].

We proposed the hypothesis that pseudophakic inflammation, including the fibrin reaction, may be caused by cytokines and/or prostaglandins, synthesized by residual lens epithelial cells (LEC). To test our hypothesis, we measured IL-1 alpha, TNF-alpha, IL-6 and EGF in the culture media of human LEC, obtained by capsulotomy during cataract surgery, by ELISA. IL-1 alpha was detected in one of the two pools of 2-week cultures (20.7 pg/10(5) cells), in two of the three pools of 3-week cultures (12.0 pg/10(5) cells and 13.9 pg/10(5) cells), and in one pool of 4-week cultures (11.1 pg/10(5) cells). Current uses of ophthalmic lasers.

Current laser treatments are quick, relatively painless, and well tolerated. Some ophthalmic techniques can be performed only by laser while others have a lower morbidity than alternative treatments. Peripheral retinal photocoagulation and focal photocoagulation now offer greatly improved visual prognosis for diabetic patients with proliferative diabetic retinopathy or diabetic macular disease. Selected cases of macular degeneration may be treated by focal laser photocoagulation. The role of lasers in treating sub-retinal neovascular membranes is limited by the extent and location of the membr

Extension deficit after ACL reconstruction: Is open posterior release a safe and efficient procedure?

BACKGROUND: Extension loss is a potentially devastating consequence of anterior cruciate ligament reconstruction (ACLR). It can often be treated by anterior arthroscopic release. In rare cases, a chronic flexion contracture requires an additional posterior open release. To our knowledge, no study analysing the results of both posteromedial and posterolateral approaches has been performed so far. The purposes of this study were (1) to analyse the midterm outcome and complications of a combined procedure, anterior arthroscopic debridement and posterior open capsulotomy - for the treatment of chr

Quantitative-MRI analysis of the effects of retrograde nailing on vascularity of the distal femur: A cadaveric study.

BACKGROUND: Distal femur fractures remain treatment challenges with a considerable postoperative non-union rate. Concern remains that surgery may compromise osseous vascularity. This study aimed to determine effects of retrograde femoral intramedullary nailing (RFIN) on distal femur vascularity, and the locations of the middle genicular artery terminal branches in relation to the standard RFIN entry point. METHODS: Five lower limb cadaveric pairs were obtained (ten specimens). Experimental limbs were randomly assigned, and contralateral limbs served as controls. An 11 mm femoral nail was impl
