Infectious complications in patients receiving autologous CD34-selected hematopoietic stem cell transplantation for severe autoimmune diseases.

Long-term analysis of infectious complication after high-dose immunosuppressive therapy with CD34-selected autologous hematopoietic stem cell transplantation for patients with severe autoimmune diseases (AD) was performed. Theoretically, CD34 selection can reduce the risk of reinfusion of autoreactive lymphocytes. However, it is also associated with a significant reduction in T cells, natural killer cells, and monocytes, which in turn may compromise immune reconstitution, thereby increasing the risk of infection. Moreover, AD compromises host immunity and causes organ damage resulting in dysfu

Native Aortic Valve Endocarditis Caused by Gut Bacterial Translocation Following Gastroenteritis.

Native valve infective endocarditis (IE) refers to infection resulting from the seeding of bacteria onto the heart valves, usually in patients with one or more risk factors such as intravenous drug use, poor dentition or recent dental surgery, prior structural or valvular heart disease, or indwelling cardiac devices. Although viridans group streptococci (VGS) are a common cause of aortic valve endocarditis, it is uncommon for the infection to result from bacterial translocation across the intestinal mucosa during gastroenteritis. This case involves a 64-year-old female who presented with an ac

Pneumonia surveillance with culture-independent metatranscriptomics in HIV-positive adults in Uganda: a cross-sectional study.

BACKGROUND: Pneumonia is a leading cause of death worldwide and is a major health-care challenge in people living with HIV. Despite this, the causes of pneumonia in this population remain poorly understood. We aimed to assess the feasibility of metatranscriptomics for epidemiological surveillance of pneumonia in patients with HIV in Uganda. METHODS: We performed a retrospective observational study in patients with HIV who were admitted to Mulago Hospital, Kampala, Uganda between Oct 1, 2009, and Dec 31, 2011. Inclusion criteria were age 18 years or older, HIV-positivity, and clinically diagno

[Replacement of the aortic valve with a pulmonary autograft: experience at the University of Verona].

BACKGROUND: Early and long term results after surgical replacement of the aortic valve depend to a large degree on the type of valve substitute used. Pulmonary autograft offers many theoretical advantages, particularly in young patients. METHODS: Between July 1994 and August 1996, 15 patients (12 male) with a mean age of 28 +/- 6 years (range 14 to 36 years) were selected for aortic valve replacement with a pulmonary autograft. The aortic valve disease was isolated insufficiency in 7 cases (47%), stenosis in 3 (20%) and mixed lesion in 5 (33%). One patient in the last group had bacterial endo

[Endocarditis caused by Cardiobacterium hominis].

The incidence of endocarditis due to Cardiobacterium hominis is probably underestimated because clinical presentations vary greatly and culture of this Gram negative germ is difficult. A 48-year-old man with a past history of post-streptococcic aortic regurgitation was hospitalized twice within 1 week for fever (38 degrees C) and junctional tachycardia which responded to amiodarone. Subsequently, infero-apical necrosis was documented. Based on the result of the laboratory tests, coronary embolism was suspected although 12 blood cultures were negative. The patient recovered well with a standard

[Undetected hypopharyngeal perforation with deep neck abscess and mediastinitis due to transesophageal echocardiography].

Hypopharyngeal perforation is a rare but dangerous complication caused by diagnostic procedures. If there is any suspicion of perforation of the upper airways and/or upper digestive tract, immediate diagnostic and therapeutic procedures are indicated. Possible complications of a hypopharyngeal perforation are deep neck infection with subsequent mediastinitis and haemorrhage from major cervical vessels, both of which have high mortality rates. We present the case of a 65 year old male patient with perforation of the hypopharynx after transesophageal echocardiography which was unrecognized for m

Impact of sex on the development of bacteremia in critically ill burn patients: A retrospective cohort study.

BACKGROUND: Burn injuries are associated with significant morbidity and mortality, with infections being a major factor influencing outcomes. Given evidence of sex-related differences in infection risk, we investigated whether such disparities extend to bacteremia in critically ill burn patients. METHODS: We conducted a single-centre cohort study of critically ill patients with severe burn injuries admitted to the Burn Center at the University Hospital Zurich between January 2017 and December 2021. The primary outcome was the association between sex and the development of a first bacteremia e

Do we need to reconsider our antibiotic regimen for preterm premature rupture of membranes? Pathogens and perinatal outcome in PPROM.

BACKGROUND: The antibiotic regimen currently recommended in most guidelines in case of preterm premature rupture of membranes (PPROM) with an aminopenicillin and a macrolide is mainly based on studies performed more than 25 years ago. In view of the changing pathogens in neonatal and perinatal infectious morbidity and their increasing aminopenicillin-resistance in recent decades, the question arises as to whether these data are still applicable to our current situation and whether an adjustment of our antibiotic standard regimen with amoxicillin and azithromycin is necessary. METHODS: We retr

Risk of bacteremia in bleeding and nonbleeding gastric varices after endoscopic injection of cyanoacrylate.

BACKGROUND AND STUDY AIM: Bacteremia is common in cirrhosis with gastrointestinal bleeding, including variceal bleeding. Elective esophageal sclerotherapy and banding have been reported to cause bacteremia. The risk associated with therapeutic endoscopy in patients with gastric varices has not yet been reported. This study was conducted to compare the risk of bacteremia between patients with active gastric variceal bleeding and those with gastric varices that were not actively bleeding who underwent N-butyl-2-cyanoacrylate injection. PATIENTS AND METHODS: Patients were categorized into three

[Therapeutic impact of streptococcal and enterococcal bacteremia in hematology patients].

From January 1999 to May 2000 (17 months), 21 strains of streptococci and four strains of enterococci have been isolated from 74 blood cultures in 25 infectious episodes in hematologic patients. They concerned 21 patients, of 21 to 77 years old. These patients suffered from acute leukaemia (14 cases), chronic lymphoid leukaemia (two cases), non-Hodgkin's lymphoma (two cases) or myeloma (three cases). Seventeen patients displayed a single streptococcal or enterococcal episode, two had two episodes in the course of a single stay in the hospital, two others in the course of two different stays. D

Influence of Early Feeding Practices on Oral Microbiota Composition During Infancy and Potential Implications for Early Childhood Caries: A Systematic Review.

BACKGROUND: Early feeding practices are among the most influential determinants of the infant oral microbiota during the first years of life. Breastfeeding provides bioactive components-immunoglobulins, human milk oligosaccharides (HMOs), and commensal bacteria-that may shape microbial colonisation patterns with long-term implications for oral health. However, the nature, magnitude, and clinical relevance of these effects remain poorly characterised, particularly with regard to early childhood caries (ECC) risk. OBJECTIVES: The primary objective was to evaluate the association between early f

The Selective Interaction of Pistacia lentiscus Oil vs. Human Streptococci, an Old Functional Food Revisited with New Tools.

Pistacia lentiscus berry oil (LBO) represents a typical vegetal product of the Mediterranean basin that has been formally used in traditional cuisine for 100s of years. In addition to its interesting alimentary properties, this product could represent an interesting candidate in the field of research on the study of new anti-infective agents. In fact, in Mediterranean countries, lentisk oil still continues to be widely used in folk medicine for oral and skin affections, in particular, acute gingivitis, pediatric skin infections such as impetigo and foot plaques, and biofilm related infections

Identification of Salivary Microbiota and Its Association With Host Inflammatory Mediators in Periodontitis.

Periodontitis is a microbial-induced chronic inflammatory disease, which may not only result in tooth loss, but can also contribute to the development of various systemic diseases. The transition from healthy to diseased periodontium depends on microbial dysbiosis and impaired host immune response. Although periodontitis is a common disease as well as associated with various systemic inflammatory conditions, the taxonomic profiling of the salivary microbiota in periodontitis and its association with host immune and inflammatory mediators has not been reported. Therefore, the aim of this study

Redo mitral valve replacement with annular reconstruction of left atrial dissection following mitral valve replacement for infective endocarditis: a case report.

BACKGROUND: Left atrial dissection is a rare and occasionally fatal complication of cardiac surgery and is defined as the creation of a false chamber through a tear in the mitral valve annulus extending into the left atrial wall. Some patients are asymptomatic, while others present with various symptoms, such as chest pain, dyspnea, and even cardiac arrest. Although there is no established management for left atrial dissection, surgery should be considered in patients with hemodynamic disruption. Herein, we report a case of left atrial dissection managed using redo mitral valve replacement (MV

Relationship between dental fluorosis, dental caries and salivary levels of Streptococcus mutans.

BACKGROUND AND OBJECTIVE: Worldwide, dental caries is an endemic infection and a significant public health problem. Fluoride reduces caries by helping to prevent demineralisation, by remineralising early carious lesions, and by decreasing the activity and growth of Streptococcus mutans (S. mutans). Fluoride intake above the optimal levels leads to a condition known as dental fluorosis, which causes hypo-mineralisation of the tooth. Some studies have revealed that the severity of fluorosis is directly proportional to caries, but some showed opposite results. Hence, with these contradicting resu

Retropharyngeal and parapharyngeal abscesses in children - a 9.5-year retrospective single-center analysis followed by literature review.

Introduction: Deep neck infections are severe complications of the inflammatory processes in the upper respiratory tract. They can be divided into retropharyngeal, parapharyngeal, peritonsillar, and submandibular abscesses, depending on the affected region. Deep neck infections are considered acute bacterial complications with a progressive course and life-threatening nature, and that is why they require an immediate, broad-spectrum, intravenous antibiotic therapy. Aim: The aim of our retrospective analysis was to characterize the population of children affected by retro- and/or parapharyngeal
