[Neuroendocrine colorectal tumors. Surgical and endoscopic treatment].

The incidence of colorectal neuroendocrine tumors (NETs) is rising in developed countries primarily as a result of increased incidental detection by endoscopy and probably also due to a more adequate diagnosis according to the WHO classification. Less than 1% of colorectal NETs produce serotonin so that such tumors are practically never associated with a hormonal carcinoid syndrome. An exact clinico-pathological staging is of paramount importance for the therapeutic strategy and comprises the classification of the tumor type (well or poorly differentiated) and the assessment of established pro

[Removal of rectal adenoma with the TEM technique].

The paper presents the operation of local excision of a rectal polyp using TEM technique. The operation was performed at our Department on the 12th of April 1996 with the instruments for transanal endoscopic microsurgery made by Wolf Company according to professor Buess. The patient was 49 year old woman, in whom rectoscopy revealed at 8 cm above the dantate line a cauliflower like lesion, 3 cm in diameter, bleeding on touch and not fixed to the underlying tissues. Transrectal sonography showed the tumor was confined to the mucosa with the absence of muscularis propria infiltration. During the

Development and validation of an interpretable machine learning model for predicting postoperative complications after transanal total mesorectal excision: a multicenter study.

OBJECTIVE: This study aimed to develop and externally validate an interpretable machine learning (ML) model for predicting postoperative complications after transanal total mesorectal excision (taTME). METHODS: We conducted a multicenter case-control study of 245 consecutive patients undergoing taTME at three centers in China. Patients from one center formed the development cohort (n = 185); two independent centers provided an external validation cohort (n = 60). The primary endpoint was any 30-day complication (Clavien-Dindo). Candidate variables were screened with least absolute shrinkage a

Transanal Total Mesorectal Excision (taTME) for Rectal Cancer: A Case Series Report of a Natural Orifice Surgical Technique.

In the treatment of colorectal cancer, total mesorectal excision (TME) has risen as the gold standard in the surgical treatment of this disease in order to obtain negative distal and circumferential radial margins. Since introduction in 2010, transanal TME has aimed to decrease the rate of positive margins and improve the quality of the dissection while decreasing the complications associated with a transabdominal low pelvic dissection. We retrospectively reviewed 25 cases of transanal TME completed between December 2014 and August 2017. Most of the patients in our case series were male (60%)

Transanal Endoscopic Microsurgery for Patients with Rare Rectal Tumors.

BACKGROUND: Transanal endoscopic microsurgery (TEM) has been accepted worldwide for the treatment of local rectal lesions. Rare rectal tumors consist of several different types of malignant or benign tumors. Surgical management is considered to be the only curative option. The aim of this study is to investigate the role of TEM in the treatment of rare rectal tumors. MATERIALS AND METHODS: A total of 147 patients with rare rectal tumors underwent TEM in our center from April 2006 to May 2017. Clinical data were collected and a retrospective accurate database was constructed. Demographic chara

Anal function after endoluminal locoregional resection by transanal endoscopic microsurgery and radiotherapy for rectal cancer.

AIM: In patients with rectal cancer, surgery and chemoradiotherapy may affect anal sphincter function. Few studies have evaluated anorectal function after neoadjuvant chemoradiotherapy (n-CRT) and/or transanal endoscopic microsurgery (TEM). The aim of this study was to evaluate the effects of n-CRT and TEM on anorectal function. METHOD: Thirty-seven patients with rectal cancer underwent anorectal manometry and Wexner scoring for faecal incontinence at baseline, after n-CRT (cT2-T3N0 cancer) and at 4 and 12 months after surgery. Water-perfused manometry measured anal tone at rest and during sq

Long-term results of patients with pT2 rectal cancer treated with radiotherapy and transanal endoscopic microsurgical excision.

Anterior resection and abdomino-perineal resection are the surgical techniques used most frequently in the treatment of rectal cancer. Local recurrence rates of 10% to 14% are described after these conventional procedures. Preoperative neoadjuvant radiotherapy reduces local failure. Because local excision techniques can be applied to treat early rectal cancer in selected patients, we evaluated the results of preoperative high-dose radiotherapy and transanal endoscopic microsurgical excision (TEM) in patients with T2 rectal cancer. All patients underwent preoperative irradiation with 5,040 cGy,

Compression anastomosis revisited: prospective audit of short- and medium-term outcomes in 62 rectal anastomoses.

BACKGROUND: The ColonRing™ represents a further advancement in compression anastomosis by using superelastic materials. In contrast to other compression devices, its field of application is identical to circular staplers. Yet, limited data are available and there are no reports on the use of this compression device for anastomoses in the lower rectum and in diverted cases. METHODS: Between November 2008 and March 2010 a nonrandomized prospective study of patients undergoing colorectal resection was done. RESULTS: Sixty-two patients (30 female) with a median age of 67 years (range = 23-87) un

Pneumovaginal Resection of a Vaginal Septum in Obstructed Hemi-Vagina and Ipsilateral Renal Anomaly Syndrome.

OBJECTIVE: To demonstrate surgical excision of a high obstructing vaginal septum using a pneumovaginal approach with the GelPOINT Path Transanal Access Platform. SETTING: OHVIRA Syndrome is defined by a Müllerian anomaly with a unilateral obstructed hemivagina and associated renal anomaly [1,2]. Resection of a high vaginal septum can be technically challenging from the transvaginal approach, limited by instrument length and introital size in recently menarchal patients, which can result in poor outcomes such as incomplete septum resection and stenosis. Pneumovaginal endoscopic surgery has bee

Complete resection of recurrent anal canal cancer using endoscopic submucosal dissection and transanal resection: A case report.

BACKGROUND: Early anal canal cancer is frequently treated with endoscopic submucosal dissection (ESD) to preserve anal function. However, if the lesion is in the anal canal, then significant difficulties such as bleeding and challenges associated with scope manipulation can arise. CASE SUMMARY: A 70-year-old woman undergoing follow-up after transverse colon cancer surgery was diagnosed with anal canal cancer extending to the dentate line. The patient underwent a combination of ESD and transanal resection (TAR). The specimen was excised in pieces, which resulted in difficulty performing the pa

Transrectal natural orifice specimen extraction in left hemicolectomy for tumours around the splenic flexure: Old wine in new bottles.

AIM: Laparoscopic anterior resection with natural orifice specimen extraction (NOSE) has favourable short-term outcomes. However, NOSE is rarely adopted for left hemicolectomy procedures. This study aimed to review the feasibility, safety and short-term outcomes of transrectal NOSE in patients undergoing laparoscopic left hemicolectomy. METHOD: All consecutive patients who underwent laparoscopic left hemicolectomy surgery with transrectal NOSE in a single institution between January 2018 and December 2020 were reviewed. Transrectal NOSE was performed with an enterotomy at the upper rectum. Th

The transanal endoscopic microsurgery procedure: standards and extended indications.

Transanal endoscopic microsurgery (TEM) was developed in the early 1980s as a minimally invasive technique allowing the resection of benign rectal adenomas. For this indication, TEM was reported to be safe and effective and even exceeded the results compared to classical local excision. Unsurprisingly, the indication expanded to small rectal cancer. There is still much debate, though, whether it is oncologically safe to perform TEM for rectal cancer. Much has been published about the need for proper patient selection, i.e. patients presenting a low-risk T1 rectal cancer seem to be the most ade

Early-Stage Rectal Mixed Neuroendocrine-Non-neuroendocrine Neoplasm Treated With Local Excision in a Frail Elderly Patient: A Multidisciplinary Management Dilemma.

Mixed neuroendocrine-non-neuroendocrine neoplasms (MiNENs) of the rectum are rare tumors that typically demonstrate aggressive behavior, largely driven by high-grade neuroendocrine carcinoma components. Evidence guiding management is limited, particularly for early-stage lesions and for patients unable to tolerate standard oncologic surgery. We report the case of an elderly man with extensive comorbidities who was found to have a 1.3 cm rectal MiNEN following transanal endoscopic microsurgery (TEMS) performed for rectal bleeding. Histopathology revealed a moderately differentiated adenocarcino

Severe Rectal Stenosis as the First Clinical Appearance of a Metastasis Originating from the Bladder: A Case Report and Literature Review.

While locally advanced rectal cancer is the first clinical suspicion for severe rectal stenosis, in extremely unusual cases a lower bowel obstruction may be related to bladder metastasis. We present the case of a 64-year-old male who was admitted for occlusive rectal tumor (4 cm from the anal verge), for which an emergency loop-colostomy was performed. After two inconclusive endoscopic biopsies, a transanal rectal tru-cut biopsy allowed for the detection of high-grade urothelial carcinoma with signet ring cells. Furthermore, primary origin was detected in a small bladder tumor. In imaging reas

Paradigm-shifting new evidence for treatment of rectal cancer.

BACKGROUND: Treatment of rectal cancer has dramatically evolved during the last three decades shifting toward a tailored approach based on preoperative staging and response to neoadjuvant combined modality therapy (CMT). METHODS: A literature search was performed using PubMed/Medline electronic databases. RESULTS: Selected patients with T1 N0 rectal cancer are best treated with local excision by transanal endoscopic microsurgery (TEM). Satisfactory results have been reported after CMT and TEM for the treatment of highly selected T2 N0 rectal cancers. CMT followed by rectal resection and tota

Transanal total mesorectal excision (taTME) for rectal cancer: beyond the learning curve.

BACKGROUND: Transanal total mesorectal excision (taTME) is a surgical approach for low rectal cancer with a learning curve estimated at 40-50 cases. The experience among taTME surgeons beyond their learning curve is limited. METHODS: A retrospective analysis of all taTME cases performed for rectal cancer at two tertiary care hospitals from 2014 to 2019 was conducted. Transanal surgeons had previously performed > 50 taTME cases. Demographic, perioperative, and short-term outcomes were analyzed. RESULTS: Among 54 taTME patients, 74.1% were male and 27.8% had a BMI ≥ 30. Tumors were stage I (8)
