[The use of hydroxyurea in severe forms of sickle cell disease: study of 47 Tunisian paediatric cases].

INTRODUCTION: The Sickle Cell Disease (SCD) is a serious illness considering its complications. For the children seriously affected, three therapeutic options are currently validated: transfusion therapy, hydroxyurea and bone-marrow transplantation. OBJECTIVES: To see the contribution of hydroxyurea therapy on severe forms of SCD in affected Tunisian children. MATERIAL AND METHODS: This investigative study lasted over 6 years and 9 months, (September 2000-May 2007), enrolling 47 patients including 27 homozygous SCD and 20 double heterozygote SCD-S/beta thalassemia. The median age was 12 year

Blood rheology biomarkers in sickle cell disease.

UNLABELLED: Sickle cell disease (SCD) is the most common inherited blood disorder, affecting approximately 100,000 patients in the U.S. and millions more worldwide. Patients with SCD experience a wide range of clinical complications, including frequent pain crises, stroke, and early mortality, all originating from a single-point mutation in the β-globin subunit. The RBC changes resulting from the sickle mutation lead to a host of rheological abnormalities that diminish microvascular blood flow, and produce severe anemia due to RBC hemolysis, and ischemia from vaso-occlusion initiated by sticky

Beneficial effects of SGLT1/2 and SGLT2 inhibitors on vaso-occlusive events and organ damage in sickle cell disease mice.

AIMS: Sodium-glucose co-transporter 2 inhibitors are widely used to treat patients with type 2 diabetes and exhibit beneficial cardiovascular effects beyond glucose lowering. In this study, we investigated their potential to alleviate vaso-occlusive events and organ damage in sickle cell disease (SCD) mice. METHODS AND RESULTS: Intravital and immunofluorescence microscopy reveal that a 4-day oral administration of dapagliflozin (DAPA) or sotagliflozin (SOTA) significantly reduces neutrophil adhesion and transmigration in cremaster venules, with SOTA showing greater inhibition, and down-regula

CRISPR-Cas12a Gene Editing of HBG1 and HBG2 Promoters to Treat Sickle Cell Disease.

BACKGROUND: Renizgamglogene autogedtemcel (reni-cel) is an investigational clustered regularly interspaced short palindromic repeats (CRISPR)-Cas12a gene-edited autologous hematopoietic stem-cell therapy. The therapy was designed to disrupt the BCL11A binding sites in the HBG1 and HBG2 promoters to reactivate fetal hemoglobin production for the treatment of sickle cell disease. METHODS: We conducted a phase 1-2, multicenter, open-label, single-group study involving patients with severe sickle cell disease who were 12 to 50 years of age and had had at least two severe vaso-occlusive events per

Vitamin D Deficiency and Avascular Necrosis in Patients With Sickle Cell Disease: A Retrospective Cohort Study.

Introduction Sickle cell disease (SCD) is an inherited hemoglobinopathy with complex multi-systemic involvement. Avascular necrosis (AVN) is a devastating complication among patients with SCD. Vitamin D deficiency (VDD) is a common finding in SCD patients. However, there is a paucity of research that evaluates the potential role of VDD as a risk factor for AVN in patients with SCD. This study aimed to assess the association between VDD and AVN and determine the patterns of presentation, risk factors, diagnosis, and treatment of AVN in patients with SCD. Methods We conducted a retrospective coh

The Red Blood Cell-Inflammation Vicious Circle in Sickle Cell Disease.

Sickle cell disease (SCD) is a genetic disease caused by a single mutation in the β-globin gene, leading to the production of an abnormal hemoglobin called hemoglobin S (HbS), which polymerizes under deoxygenation, and induces the sickling of red blood cells (RBCs). Sickled RBCs are very fragile and rigid, and patients consequently become anemic and develop frequent and recurrent vaso-occlusive crises. However, it is now evident that SCD is not only a RBC rheological disease. Accumulating evidence shows that SCD is also characterized by the presence of chronic inflammation and oxidative stress

Pentoxifylline. A review of its pharmacodynamic and pharmacokinetic properties, and its therapeutic efficacy.

Pentoxifylline (oxpentifylline) is an orally active haemorheological agent for the treatment of peripheral vascular disease, cerebrovascular disease and a number of other conditions involving a defective regional microcirculation. Pentoxifylline acts primarily by increasing red blood cell deformability, by reducing blood viscosity and by decreasing the potential for platelet aggregation and thrombus formation. Extensive open and placebo-controlled studies have shown that pentoxifylline 600 to 1200 mg/day for at least 6 weeks is associated with subjective and objective improvements in 60 to 100

Temporal trends and determinants of ED visits in sickle cell disease: a multicenter EHR study from Saudi Arabia (2016-2021).

BACKGROUND: Sickle cell disease (SCD) is a major chronic hematologic disorder in Saudi Arabia associated with recurrent vaso-occlusive crises (VOCs), acute pain episodes, and heavy reliance on emergency department (ED) visits. Despite improvements in survival, longitudinal data on ED utilization patterns and their determinants remain limited in the region. Therefore, the aim of this study is to assess temporal trends and predictors of ED utilization among patients with SCD across multiple tertiary centers in Saudi Arabia between 2016 and 2021. METHODS: This multicenter retrospective cohort st

Mineral bone disorders and kidney disease in hospitalized children with sickle cell anemia.

BACKGROUND: Mineral bone disorders (MBD) are common in sickle cell anemia (SCA). Frequent vaso-occlusive crises (VOC) further impact MBD in children with SCA. We evaluated the prevalence of markers of SCA-related MBD (sMBD) in hospitalized children and assessed the relationship between sMBD and individual mineral abnormalities with kidney disease. METHODS: We prospectively recruited 185 children with SCA hospitalized with a VOC. Serum measures of mineral bone metabolism (calcium, phosphate, parathyroid hormone, 25-hydroxy vitamin D, FGF23, osteopontin) were measured at enrollment. The primary

Mechanism of action of hydroxyurea in the management of sickle cell anemia in adults.

In a randomized, placebo-controlled clinical trial, treatment with hydroxyurea (HU) reduced crisis rates in adult patients with severe sickle cell anemia. No serious acute toxicity was seen, but the safety of long-term therapy could not be evaluated. The rationale for the use of HU was based on its ability to increase fetal hemoglobin (HbF) synthesis and the inhibitory effect of HbF on polymerization of sickle cell hemoglobin. Surprisingly, final HbF levels in patients assigned to HU did not differ markedly from their pretreatment levels (5% v 9%). As Steinberg et al showed, when HU patients w

Indications for Blood Transfusion and Exchange Transfusion in Sickle Cell Disease: A Single Center Experience.

Background Sickle cell disease (SCD) is a prevalent inherited hemoglobinopathy characterized by chronic hemolytic anemia, vaso-occlusive crises (VOC), and multi-organ complications. Blood transfusion plays a critical role in the management of both acute and chronic complications associated with SCD, including stroke, acute chest syndrome (ACS), and splenic sequestration. However, the variability in transfusion practices and associated risks, such as alloimmunization and iron overload, necessitates further investigation into optimizing transfusion strategies in pediatric SCD patients. Objective

PHEDRE trial protocol - observational study of the prevalence of problematic use of Equimolar Mixture of Oxygen and Nitrous Oxide (EMONO) and analgesics in the French sickle-cell disease population.

BACKGROUND: The use of analgesics can lead to cases of drug abuse and dependence. It can also cause pseudo-addiction in patients suffering from pain. What is the actual situation in patients suffering from severe sickle-cell disease, exposed to acute pain during vaso-occlusive crises? Evaluation of the use of analgesics, on the basis of Diagnostic and Statistical Manual of Mental Disorders criteria for substance abuse and dependence, makes it possible to differentiate the symptoms occurring only in a context of pain, in the aim of managing the pain, and thus describing pseudo-addiction, from s

Confronting Cholelithiasis: A Case Series of Patients With Sickle Cell Disease and Gallstones.

Sickle cell disease (SCD) is a prevalent hemoglobinopathy that leads to various complications, including hepatobiliary diseases, which are often underrecognized. Cholelithiasis and choledocholithiasis, resulting from accelerated pigment gallstone formation due to increased hemolysis, are common in SCD patients. This case series aims to raise awareness of the heightened risk of biliary disease in SCD and emphasize the need for early screening and management strategies. We present four cases of adult SCD patients who developed gallstones, each requiring medical intervention for biliary complicat
