Late results of two hundred seventeen femoropopliteal bypasses to isolated popliteal artery segments.

A 10-year-experience of 217 femoropopliteal bypasses to isolated popliteal artery segments in 207 patients is reported. Thirty-three femoropopliteal bypasses (15%) were performed with reversed saphenous vein and 184 (85%) with polytetrafluoroethylene grafts. Operative indications were gangrene in 121 (56%), nonhealing ulceration in 40 (18%), ischemic rest pain in 51 (24%), and claudication in 5 (2%) cases. The 5-year primary graft patency rate of these bypasses was 59% (reversed saphenous vein, 74%; polytetrafluoroethylene, 55%; p less than 0.05), the secondary 5-year graft patency rate was 61

Development and Validation of an Evidence-Based Algorithm for Diagnosing Periprosthetic Joint Infection.

BACKGROUND: The guidelines for diagnosis of periprosthetic joint infection (PJI) introduced by the American Academy of Orthopaedic Surgeons served the orthopedic community well. However, they have never been validated and do not account for newer diagnostic modalities. Our aim was to update current guidelines and develop an evidence-based and validated diagnostic algorithm. METHODS: This multi-institutional study examined total joint arthroplasty patients from 3 institutions. Patients fulfilling major criteria for infection as defined by Musculoskeletal Infection Society were considered infec

The Impact of α-Defensin Test in Diagnosing Periprosthetic Infection After Total Ankle Arthroplasty.

Periprosthetic joint infection (PJI) after total ankle arthroplasty (TAA) is a serious complication, and a reliable diagnostic test to identify PJI is needed. The purpose of this study was to investigate the use of synovial α-defensin levels in identifying PJI of the ankle. Data from 33 patients were retrospectively collected between September 2015 and May 2018. Patients who had pain or suspected loosening after TAA and who had undergone joint aspiration were included in the study. Aspiration was performed in a semisterile theatre. Synovial fluid was processed in descending order for microbiol

The effects of hydroxyapatite coating and bone allograft on fixation of loaded experimental primary and revision implants.

We used our established experimental model of revision joint replacement to examine the roles of hydroxyapatite coating and bone graft in improving the fixation of revision implants. The revision protocol uses the Søballe micromotion device in a preliminary 8-week period of implant instability for the presence of particulate polyethylene. During this procedure, a sclerotic endosteal bone rim forms, and a dense fibrous membrane is engendered, having macrophages with ingested polyethylene and high levels of inflammatory cytokines. At the time of revision after 8 weeks, the cavity is revised with

Interleukin-4 inhibits the gene expression and biosyntheis of cytosolic phospholipase A2 in lipopolysaccharide stimulated U937 macrophage cell line and freshly prepared adherent rheumatoid synovial cells.

We recently reported that interleukin-4 (IL-4) inhibited prostanoid synthesis through inhibiting cyclooxygenase 2 biosynthesis. In the present study, we examined the effect of IL-4 on the expression of cytosolic phospholipase A2 (cPLA2). The amounts of protein and mRNA of cPLA2 were determined by western blotting and reverse transcription polymerase chain reaction (RT-PCR), respectively. Although interleukin-1alpha (IL-1alpha) and tumor necrosis factor alpha (TNFalpha) had little effect on the biosynthesis of cPLA2 in phorbol myristate acetate (PMA)-differentiated U937 cells, lipopolysaccharid

Malignant Melanoma Bone Metastasis With Unknown Primary Site: A Case Report and Literature Review.

IntroductionCases of malignant melanoma first identified in bone are relatively uncommon. Approximately 4.3% of the patients will have metastasis, among which 23% will have metastasis to the bone.Case presentationThis article presents a case involving malignant melanoma with a metastatic lesion located at the distal femur, which was the initial symptom observed. The patient was a 21-year-old male who was admitted to the hospital due to intermittent pain in his lower left thigh. Imaging studies indicated bone destruction in the distal left femur, and pathological and immunohistochemical confirm

Periprosthetic joint infections at a teaching hospital in 1990-2007.

BACKGROUND: Periprosthetic joint infections (PJIs) are major complications associated with high costs and substantial morbidity. We sought to evaluate hip and knee arthroplasty infection rates at our hospital, compare them in periods before and after implementation of measures to reduce PJIs (1990-2002 and 2003-2007) and identify associated risk factors. METHODS: We retrospectively reviewed records of patients who received primary hip or knee total joint prostheses at our centre between Jan. 1, 1990, and Dec. 31, 2007, and were readmitted for the treatment of infection related to their surger

Total hip arthroplasty using a cementless dual-mobility cup provides increased stability and favorable gait parameters at five years follow-up.

BACKGROUND: Rates of dislocation following primary total hip arthroplasty (THA) vary from 0.5 to 10%. Dual-mobility cups in THA demonstrate increased stability. Clinical outcomes following THA with dual-mobility cups have been reported, but gait has not been assessed. Therefore we performed a retrospective case control study to answer: (1) is gait better in patients following THA with a dual-mobility cup than in frail, elderly patients of the same age? (2) Are clinical outcomes better in patients following THA with a dual-mobility cup than in frail, elderly patients? (3) What is the dislocatio

Comparative outcome analysis of malpositioned and properly positioned fixation groups after hamstring autograft ACL reconstruction with femoral cross-pin fixation.

Anterior cruciate ligament (ACL) reconstruction using soft tissue grafts has been gaining in popularity. Cross-pin fixation is used frequently, and its principle of fixation is an expansion mechanism. However, some malpositioned cases have been reported, and these malpositions may cause poor outcomes. Our aim was to compare the stability and clinical results between malpositioned and properly positioned cases. From November 2003 to December 2007, 41 patients were enrolled in this study. We divided these patients into malpositioned and properly positioned subgroups. The malpositioned group was

Rapidly progressive neuropathic arthropathy of the knee in possible association with a huge extruded cervical intervertebral disc herniation.

A 58-year-old woman with an insignificant stage of diabetes mellitus presented with rapidly progressive osteoarthritis of her right knee exhibiting painless but significant antero-posterior and lateral instability. In association, she had a huge extruded left-sided cervical intervertebral disc at the C4-5 level. Neurological examination of the upper limbs was normal but, she had diminished pinprick sensation on the right leg. Radiographic findings were consistent with neuropathic arthropathy of the knee. Anterior decompression with fusion of C4-5 and total knee arthroplasty using a semi-constr
