Enhanced swallowing function and quality of life in post-stroke dysphagia: superior efficacy of tongue-nape acupuncture combined with tDCS.

BACKGROUND: This study evaluated the combined effects of Tongue-Nape Acupuncture (TNA) and Transcranial Direct Current Stimulation (tDCS) on 85 post-stroke dysphagia (PSD) patients. METHODS: Participants were assigned to either a tDCS-only group (n = 40) or a TNA + tDCS group (n = 45). Swallowing function (Water Swallowing Test, Standard Swallowing Assessment) and Quality of Life (QoL) were assessed. Serum levels of neural injury markers (S-100β, NSE, MMP-9, Hcy, hs-CRP) were measured, alongside brain activity (near-infrared imaging) and muscle function (EMG). RESULTS: The TNA + tDCS group a

The Barriers and Facilitators to the Application of Non-Invasive Brain Stimulation for Injury Rehabilitation and Performance Enhancement: A Qualitative Study.

INTRODUCTION: Despite clinical evidence for efficacy, there has been minimal uptake of transcranial direct current stimulation (tDCS) for musculoskeletal conditions. Thus, our objective was to explore the perceptions and experiences of people living with lower-limb musculoskeletal injury as well as healthy physically active populations and relate this to the usage of tDCS and key aspects of tDCS design that would improve the capacity for implementation. METHODS: We conducted a qualitative descriptive study of 16 participants (44% women) using semi-structured focus groups to identify the descr

tDCS-induced enhancement of cognitive flexibility in autism: role of frontal lobe and associated neural circuits.

BACKGROUND: Autism spectrum disorder (ASD) is a neurodevelopmental disorder characterized by impaired social interaction and restricted, repetitive behaviors (RRBs). These symptoms may stem from cognitive flexibility deficits, with dysfunction in the prefrontal cortex (PFC) and related neural circuits proposed as underlying mechanisms. OBJECTIVES: This study examined whether transcranial direct current stimulation (tDCS) could enhance PFC activity and functional connectivity, thereby improving cognitive flexibility in a valproic acid (VPA)-induced ASD rat model. METHODS: Pregnant Sprague-Daw

Toward Improving Functional Recovery in AIDS-associated Progressive Multifocal Leukoencephalopathy: A Single Case Pilot Study on a Novel Neuromodulation Approach.

Patients with progressive multifocal leukoencephalopathy (PML) in the context of human immunodeficiency virus-acquired immunodeficiency syndrome (HIV-AIDS) show a partial improvement following rehabilitation; however, this improvement is rapidly lost if the patient is not provided with intensive rehabilitation. A 42-year-old patient affected by HIV-AIDS had a clinical worsening within a few months following PML onset, despite being treated with antiretroviral drugs and conventional rehabilitation. He developed severe paraparesis and significant dependency in the activities of daily life. A fir

Beyond Reading Modulation: Temporo-Parietal tDCS Alters Visuo-Spatial Attention and Motion Perception in Dyslexia.

Dyslexia is a neurodevelopmental disorder with an atypical activation of posterior left-hemisphere brain reading networks (i.e., temporo-occipital and temporo-parietal regions) and multiple neuropsychological deficits. Transcranial direct current stimulation (tDCS) is a tool for manipulating neural activity and, in turn, neurocognitive processes. While studies have demonstrated the significant effects of tDCS on reading, neurocognitive changes beyond reading modulation have been poorly investigated. The present study aimed at examining whether tDCS on temporo-parietal regions affected not only

Combined TMS and FMRI reveal dissociable cortical pathways for dynamic and static face perception.

Faces contain structural information, for identifying individuals, as well as changeable information, which can convey emotion and direct attention. Neuroimaging studies reveal brain regions that exhibit preferential responses to invariant [1, 2] or changeable [3-5] facial aspects but the functional connections between these regions are unknown. We addressed this issue by causally disrupting two face-selective regions with thetaburst transcranial magnetic stimulation (TBS) and measuring the effects of this disruption in local and remote face-selective regions with functional magnetic resonance

TMS-evoked EEG potentials demonstrate altered cortical excitability in migraine with aura.

Migraine is associated with altered sensory processing, that may be evident as changes in cortical responsivity due to altered excitability, especially in migraine with aura. Cortical excitability can be directly assessed by combining transcranial magnetic stimulation with electroencephalography (TMS-EEG). We measured TMS evoked potential (TEP) amplitude and response consistency as these measures have been linked to cortical excitability but were not yet reported in migraine. We recorded 64-channel EEG during single-pulse TMS on the vertex interictally in 10 people with migraine with aura and 1

Effectiveness of a Five-Day Multimodal Intensive Treatment Program for Obsessive-Compulsive Disorder: A Prospective Observational Study.

INTRODUCTION: Obsessive-compulsive disorder (OCD) is a chronic psychiatric disorder that frequently requires a combination of pharmacological and behavioral interventions. Intensive treatment programs integrating optimized pharmacotherapy, exposure and response prevention (ERP), and neuromodulation have emerged as promising approaches for accelerating symptom improvement. However, evidence regarding the effectiveness of complete adherence to such multimodal interventions in routine clinical practice remains limited. This study evaluated the clinical effectiveness and safety of a five-day multi

Negation and social avoidance in language recruits the right inferior frontal gyrus: a tDCS study.

INTRODUCTION: In the process of comprehension, linguistic negation induces inhibition of negated scenarios. Numerous studies have highlighted the role of the right Inferior Frontal Gyrus (rIFG) - a key component of the inhibitory network - in negation processing. Social avoidance can be linguistically portrayed using attitudinal verbs such as "exclude" vs. "include", which inherently carry negative connotations. Consequently, we hypothesize that the interplay between explicit negation and the implicit negativity of avoidance verbs can be modulated via transcranial direct current stimulation (t

Clinical outcomes and neural correlates of 20 sessions of repetitive transcranial magnetic stimulation in severe and enduring anorexia nervosa (the TIARA study): study protocol for a randomised controlled feasibility trial.

BACKGROUND: Anorexia nervosa (AN) is a serious mental disorder with multiple comorbidities and complications. In those with a severe and enduring form of the illness (SEED-AN), treatment responsivity is poor and the evidence base limited. Thus, there is a need for novel treatment strategies. This paper describes the theoretical background and protocol of a feasibility randomised controlled trial (RCT) of real versus sham (placebo) therapeutic repetitive transcranial magnetic stimulation (rTMS) in SEED-AN. The aim of this trial is to obtain information that will guide decision making and protoc

Perceptual and cognitive effects of focal tDCS of auditory cortex in tinnitus.

OBJECTIVES: Noninvasive brain stimulation continues to grow as an effective, low-risk way of improving the symptoms of brain conditions. Transcranial direct current stimulation (tDCS) is particularly well-tolerated, with benefits including low cost and potential portability. Nevertheless, continued study of perceptual and cognitive side effects is warranted, given the complexity of functional brain organization. This paper describes the results of a brief battery of tablet-based tasks used in a recent pilot study of auditory-cortex tDCS in people with chronic tinnitus. METHODS: Volunteers wit

Cross-Frequency Magnetic Modulation of Hippocampal Synaptic Plasticity: From Cellular Mechanisms to System-Level Adaptation.

Magnetic stimulation modulates hippocampal synaptic plasticity in a parameter-dependent manner with effects shaped by stimulation frequency, intensity, waveform, and exposure conditions. Low-intensity magnetic fields generate induced electric fields that can influence neuronal membrane excitability and neural network activity. Hippocampal long-term potentiation (LTP) and long-term depression (LTD) are widely used as important readouts for evaluating the neurobiological effects of magnetic stimulation. Previous studies have shown that extremely low-frequency magnetic fields (ELF-MFs) may enhanc

Effects of non-target leg activation, TMS coil orientation, and limb dominance on lower limb motor cortex excitability.

Transcranial magnetic stimulation (TMS) is used to examine corticospinal tract integrity after stroke, however, generating motor-evoked potentials (MEPs) in the lower limb (LL) can be difficult. Previous studies have used activation of the target leg to facilitate MEPs in the LL but this may not be possible after stroke due to hemiplegia. The dominance of the target limb may also be important, however the neurophysiological effects of LL dominance are not known. We investigated whether voluntary activation of the non-target leg combined with optimal TMS coil orientation increases corticomotor

Task-dependent cortical responses to peripheral and central neuromodulation during water and saliva swallowing after stroke: an exploratory fNIRS pilot study.

BACKGROUND: Post-stroke dysphagia is a common and serious complication. Neurorehabilitation strategies include peripheral neuromuscular electrical stimulation (NMES) and central transcranial direct current stimulation (tDCS), which operate via distinct bottom-up and top-down mechanisms, respectively. However, their differential effects on cortical hemodynamic responses during functionally distinct swallowing tasks remain poorly understood. This pilot study aimed to investigate the immediate modulatory effects of NMES and tDCS on cortical hemodynamics during different swallowing tasks in post-s

Personalized transcranial alternating current stimulation (tACS) and physical therapy to treat motor and cognitive symptoms in Parkinson's disease: A randomized cross-over trial.

Abnormal cortical oscillations are markers of Parkinson's Disease (PD). Transcranial alternating current stimulation (tACS) can modulate brain oscillations and possibly impact on behaviour. Mapping of cortical activity (prevalent oscillatory frequency and topographic scalp distribution) may provide a personalized neurotherapeutic target and guide non-invasive brain stimulation. This is a cross-over, double blinded, randomized trial. Electroencephalogram (EEG) from participants with PD referred to Specialist Clinic, University Hospital, were recorded. TACS frequency and electrode position were

Direct cortical stimulation with cylindrical depth electrodes in the interhemispheric fissure for leg motor evoked potential monitoring.

OBJECTIVE: To evaluate cylindrical depth electrodes in the interhemispheric fissure as an alternative to subdural strip electrodes for direct cortical stimulation (DCS) leg motor evoked potential (MEP) monitoring. METHODS: A cylindrical depth electrode was positioned in the interhemispheric fissure of 37 patients who underwent supratentorial brain surgery. Leg sensory and motor cortices were localized by highest tibial nerve somatosensory evoked potential amplitude and lowest DCS leg MEP threshold; the lowest-threshold electrode was then used for DCS leg MEP monitoring. RESULTS: Intraoperati

Brain age prediction in stroke patients: Highly reliable but limited sensitivity to cognitive performance and response to cognitive training.

Cognitive deficits are important predictors for outcome, independence and quality of life after stroke, but often remain unnoticed and unattended because other impairments are more evident. Computerized cognitive training (CCT) is among the candidate interventions that may alleviate cognitive difficulties, but the evidence supporting its feasibility and effectiveness is scarce, partly due to the lack of tools for outcome prediction and monitoring. Magnetic resonance imaging (MRI) provides candidate markers for disease monitoring and outcome prediction. By integrating information not only about
