Evaluation of scrotal and testicular radiation doses for heterotopic ossification prophylaxis.

The majority of patients with heterotopic ossification are males with traumatic injuries in the hip/femur region. The testes, given their proximity, are exposed to scatter radiation, which has the potential to alter sperm count and morphology. In a prospective study, patients were treated with an 800-cGy dose of radiation without direct exposure of the testes/scrotum but with a testicular shield. Thermoluminescent dosimeters were placed inside and outside the shield. Mean dose inside and outside the shield was 10.2 and 20.2 cGy, respectively (sperm abnormalities have been reported with 15 cGy)

Partridge osteosynthesis: a prospective clinical study on the use of nylon cerclage bands and plates in the treatment of periprosthetic femoral shaft fractures.

OBJECTIVE: To evaluate the clinical use of the Partridge osteosynthesis in periprosthetic femoral fractures. DESIGN AND SETTING: Prospective nonrandomized clinical study. PATIENTS: Over a ten-year period, 222 patients presenting with femoral fractures near the tip of a hip prosthesis were treated with the Partridge system, which employs elevated cerclage nylon bands and flexible elevated nylon plates. Sixty-five fractures were located cranial to the tip of the prosthesis (Whittaker Type I), 116 at the tip (type II), and forty-one distal to the tip of the prosthesis (Type III). The population

Nail-to-bicondylar ratio, but not CPAK classification, is associated with arthrosis and deformity progression after retrograde fixator-assisted nailing for distal femoral deformity in metabolic bone disease.

BACKGROUND: Distal femoral deformities are common in metabolic bone disease and often require surgical correction. Retrograde Fixator-Assisted Nailing(r-FAN) stabilizes the femoral axis, yet the influence of nail-to-bicondylar ratio (NBR) and coronal-plane knee alignment patterns(CPAK) on deformity and osteoarthritis(OA) progression remains unclear. METHODS: We retrospectively evaluated 30 limbs from 17 patients with distal femoral deformities treated with r-FAN, with a mean follow-up of 10.5 ± 4.6 years. Pre- and postoperative radiographs were analyzed for mechanical lateral distal femoral a

Tibiotalocalcaneal arthrodesis using retrograde nailing (TTCAN) in post-traumatic conditions with high septic risk. Clinical experience.

PURPOSE: To evaluate the efficacy of tibiotalocalcaneal arthrodesis using retrograde nailing (TTCAN) in post-traumatic conditions with high septic risk. We hypothesized that this minimally invasive technique would achieve bone union and satisfactory functional recovery without increasing septic risk. METHODS: A prospective single-centre observational study included 20 patients who underwent TTCAN between January 2020 and December 2023. The primary outcome was the complete joint fusion rate at six months. Secondary outcomes included pain assessment, complications, functional evaluation using t

Cost-Effectiveness of Titanium Elastic Nail (TEN) in the Treatment of Forearm Fractures in Children.

Background and objectives: There are various methods in the management of forearm fractures in children. Elastic stable intramedullary nailing using Titanium Elastic Nail (TEN) is nowadays employed in diaphysis fractures of children, with clear benefits over other treatment options. However, in the case of TEN versus other treatment methods of forearm fractures in children, cost is an important issue. This report will focus on the cost assessment of using TEN versus other therapeutic means in the treatment of forearm fractures in children. Materials and Methods: We performed a retrospective lo

Rotator cuff integrity and shoulder function after intra-medullary humerus nailing.

INTRODUCTION: Antegrade percutaneous intra-medullary nailing (IMN) has a poor reputation in the treatment of humerus fractures. The aim of the present study was to assess rotator cuff integrity and shoulder function after IMN in humerus fracture. HYPOTHESIS: Third-generation humeral nails (straight, small diameter, with locked screws) conserve rotator cuff tendon integrity and avoid the shoulder stiffness and pain incurred by 1st generation (large diameter, without self-blocking screw) and 2nd generation nails (curved, penetrating the supraspinatus insertion on the greater tuberosity). Defining reference levels for intra-operative radiation exposure in orthopaedic trauma: A retrospective multicentre study.

There is currently limited data to define reference levels for the use of ionising radiation in orthopaedic trauma surgery. In this multicentre study, we utilise methodology employed by the Health Protection Agency in establishing reference levels for diagnostic investigations in order to define analogous levels for common and reproducible orthopaedic trauma procedures. Four hundred ninety-five procedures were identified across four Greater London hospitals over a 1-year period. Exposure was defined in terms of both time and dose area product (DAP). Third quartile mean values for either parame

Pseudoaneurysm of anterior tibial artery following tibial nailing: a case report.

Interlocking nailing is a widely accepted and performed treatment for tibial shaft fractures. The addition of percutaneously placed transfixation screws increases the stabilisation provided by intramedullary nailing; however, the technical complexity associated with the procedure has introduced new potential complications. We report a pseudoaneurysm of the anterior tibial artery caused by a proximal interlocking screw after intramedullary nailing surgery to repair a tibial shaft fracture. The patient experienced complete relief of symptoms following removal of the nail and the screws, excision

Late vascular injury following intertrochanteric fracture reduction with sliding hip screw.

A case involving late injuries to pelvic vessels caused by a sliding hip screw penetrating through the acetabulum has not been previously reported. We present the case of an 88-year-old man who presented with a retroperitoneal and extraperitoneal hematoma 4 months after open reduction and internal fixation of an intertrochanteric hip fracture. Surgical exploration showed a laceration of the left external iliac artery, which was repaired. However, the sliding hip screw was left in place. The patient's condition deteriorated, and he expired 2 days later. Postmortem examination revealed a lacerat

Intraarticular heterotopic ossification in the knee following intramedullary nailing of the fractured femur using a retrograde method.

The cases of a forty-five-year-old woman and a twenty-year-old man who developed severe intraarticular and periarticular heterotopic ossification around the knee following intramedullary nailing of a femur fracture using a retrograde technique. The association of musculoskeletal heterotopic ossification with closed head injuries seems well established and can occur in and around the knee following retrograde intramedullary nailing. This complication may occur more often than has been reported.

The use of femoral intramedullary nailing as an interim or salvage technique during complicated total hip replacement.

When performing a revision total hip replacement complicated by infection, severe osteolysis, comminuted periprosthetic fracture, and/or extensive bone loss, a single-stage procedure may not be feasible. This study reports four cases of femoral intramedullary nailing as an interim or salvage technique during complicated total hip replacement. This reconstruction provides axial and rotational stability of the femur while maintaining femoral alignment. Furthermore, this reconstruction facilitates early mobilization and rehabilitation of the patient. This interim reconstruction can be converted t

Precise nail tip positioning after tibial intramedullary nailing prevents anterior knee pain.

PURPOSE: Anterior knee pain (AKP) is a common complication following intramedullary nailing of tibial shaft fractures. Our aim was, by analysing the postoperative lateral knee X-rays and clinical status (VAS score), to find the best intramedullary tip position of a non protruded nail that will provide the best postoperative outcome avoiding AKP. METHODS: We evaluated the postoperative outcome of 221 patients, from the last four years, with healed fractures initially treated with intramedullary reamed nails with two or three interlocking screws proximally and distally through a medial paratend
