Essential drugs for ration kits in developing countries.

Since the early 1980s drug ration kits have been used to improve the supply of essential drugs to rural health facilities in developing countries. This paper evaluates some of the experiences with kit systems in Angola, Bhutan, Democratic Yemen, Guinea-Conakry, Kenya, Mozambique, Sudan, Tanzania, Uganda and Zambia in relation to the selection of drugs for the kits and their quantities and cost. Data were collected through a review of published papers, annual reports and programme evaluations, by questionnaires among field staff and interviews with key experts. In comparing the 10 programmes, 2

Plasmid evolution in carbapenemase-producing Enterobacteriaceae: a review.

Carbapenem-resistant Enterobacteriaceae (CRE) have been listed by the WHO as high-priority pathogens owing to their high association with mortalities and morbidities. Resistance to multiple β-lactams complicates effective clinical management of CRE infections. Using plasmid typing methods, a wide distribution of plasmid replicon groups has been reported in CREs around the world, including IncF, N, X, A/C, L/M, R, P, H, I, and W. We performed a literature search for English research papers, published between 2013 and 2018, reporting on plasmid-mediated carbapenem resistance. A rise in both carb

The seasonal influence of climate and environment on yellow fever transmission across Africa.

BACKGROUND: Yellow fever virus (YFV) is a vector-borne flavivirus endemic to Africa and Latin America. Ninety per cent of the global burden occurs in Africa where it is primarily transmitted by Aedes spp, with Aedes aegypti the main vector for urban yellow fever (YF). Mosquito life cycle and viral replication in the mosquito are heavily dependent on climate, particularly temperature and rainfall. We aimed to assess whether seasonal variations in climatic factors are associated with the seasonality of YF reports. METHODOLOGY/PRINCIPAL FINDINGS: We constructed a temperature suitability index fo

Co-Inheritance of alpha-thalassemia and sickle cell disease in a cohort of Angolan pediatric patients.

The aim of this study was to explore the association between alpha-thalassemia, fetal hemoglobin, hematological indices, and clinical adverse events in Angolan sickle cell disease pediatric patients. A total of 200 sickle cell disease (SCD) children were sampled in Luanda and Caxito. A venous blood sample was collected and used for hematological analyses, fetal hemoglobin quantification, and genotyping of 3.7 kb alpha-thalassemia deletion by GAP-PCR. The frequency of the 3.7 kb alpha-thalassemia deletion in homozygosity was 12.5% and in heterozygosity was 55.0%. An increase in alpha-thalassemi

Sensitivity and Specificity of a Prototype Rapid Diagnostic Test for the Detection of Trypanosoma brucei gambiense Infection: A Multi-centric Prospective Study.

BACKGROUND: A major challenge in the control of human African trypanosomiasis (HAT) is lack of reliable diagnostic tests that are rapid and easy to use in remote areas where the disease occurs. In Trypanosoma brucei gambiense HAT, the Card Agglutination Test for Trypanosomiasis (CATT) has been the reference screening test since 1978, usually on whole blood, but also in a 1/8 dilution (CATT 1/8) to enhance specificity. However, the CATT is not available in a single format, requires a cold chain for storage, and uses equipment that requires electricity. A solution to these challenges has been pr

Sickle Cell Disease: Can genetic variability influence pregnancy outcomes?

UNLABELLED: Pregnancy in Sickle Cell Disease (SCD), a severe hereditary genetic condition, highly prevalent in Sub-Saharan African countries, is associated with increased risk of complications and severe outcomes in pregnancy, like intrauterine growth restriction, low birth weight, premature birth, miscarriage, stillbirth, pre-eclampsia, and maternal mortality. Several factors have been identified as associated with the heterogeneity of SCD phenotypes, namely the hemoglobin subunit beta (HBB) haplotype and -3.7 kb α-thalassemia deletion. OBJECTIVE: This study aimed to identify pregnancy compl

A Regional Approach to Malaria Molecular Surveillance in Southern Africa: Lessons from the GenE8 Initiative.

The growing threat of artemisinin partial resistance prompted the WHO to recommend routine monitoring of drug efficacy and resistance. Advances in sequencing technologies and expanded laboratory capacity across Africa have made malaria molecular surveillance (MMS) a viable option for many countries. Despite improved access to next-generation sequencing platforms, the uptake of MMS for programmatic decision-making and the availability of antimalarial resistance marker data in southern Africa have lagged behind those in other regions. The Genomics of Malaria in the Elimination Eight (GenE8) init

Reducing transfusion utilization for children with sickle cell anemia in sub-Saharan Africa with hydroxyurea: Analysis from the phase I/II REACH trial.

Children with sickle cell anemia (SCA) in Africa frequently require transfusions for SCA complications. Despite limited blood supplies, strategies to reduce their transfusion needs have not been widely evaluated or implemented. We analyzed transfusion utilization in children with SCA before and during hydroxyurea treatment. REACH (Realizing Effectiveness Across Continents with Hydroxyurea, NCT01966731) is a longitudinal Phase I/II trial of hydroxyurea in children with SCA from Angola, Democratic Republic of Congo, Kenya, and Uganda. After enrollment, children had a two-month pre-treatment scre

Clinical Study on the Melarsoprol-Related Encephalopathic Syndrome: Risk Factors and HLA Association.

Melarsoprol administration for the treatment of late-stage human African trypanosomiasis (HAT) is associated with the development of an unpredictable and badly characterized encephalopathic syndrome (ES), probably of immune origin, that kills approximately 50% of those affected. We investigated the characteristics and clinical risk factors for ES, as well as the association between the Human Leukocyte Antigen (HLA) complex and the risk for ES in a case-control study. Late-stage Gambiense HAT patients treated with melarsoprol and developing ES (69 cases) were compared to patients not suffering

Progress toward interruption of wild poliovirus transmission--worldwide, 2008.

Since 1988, when the Global Polio Eradication Initiative was established, the incidence of polio has decreased from an estimated 350,000 cases annually to 1,655 reported in 2008. Cases of wild poliovirus (WPV) type 2 were last reported in October 1999, and indigenous WPV types 1 and 3 (WPV1 and WPV3) have been eliminated from all but four countries worldwide (Afghanistan, India, Nigeria, and Pakistan). This report updates previous reports and describes overall progress toward global eradication in 2008. Despite accelerated efforts, polio cases increased 26%, from 1,315 cases in 2007 to 1,655 i

Sleep structure: a new diagnostic tool for stage determination in sleeping sickness.

Human African trypanosomiasis (HAT), due to the transmission of Trypanosoma brucei (T. b.) gambiense and T. b. rhodesiense by tsetse flies, is re-emerging in inter-tropical Africa. It evolves from the hemolymphatic Stage I to the meningo-encephalitic Stage II. The latter is generally treated with melarsoprol, an arseniate provoking often a deadly encephalopathy. A precise determination of the HAT evolution stage is therefore crucial. Stage II patients show: (i) a deregulation of the 24-h distribution of the sleep-wake alternation; (ii) an alteration of the sleep structure, with frequent sleep

Social determinants of health and migrant access to public healthcare in Brazil during the COVID-19 pandemic.

BACKGROUND: In recent decades, Brazil has emerged as a significant destination for migrants and refugees, contributing to a Global South-South migration trend. This trend is particularly notable for its influx from South and Central American countries, as well as African countries. Healthcare access in Brazil is universal through the Brazilian Health System (SUS). Although the existence of a social protection system is relevant, there are challenges to the law’s actual applicability in the country. Given the profound social and territorial inequalities existing in Brazil, the consequences of t

Assessing the ecological resilience of Ebola virus in Africa and potential influencing factors based on a synthesized model.

BACKGROUND: The Ebola epidemic has persisted in Africa since it was firstly identified in 1976. However, few studies have focused on spatiotemporally assessing the ecological adaptability of this virus and the influence of multiple factors on outbreaks. This study quantitatively explores the ecological adaptability of Ebola virus and its response to different potential natural and anthropogenic factors from a spatiotemporal perspective. METHODOLOGY: Based on historical Ebola cases and relevant environmental factors collected from 2014 to 2022 in Africa, the spatiotemporal distribution of Ebol

Prevalence of elephantiasis, an overlooked disease in Southern Africa: a comprehensive review.

Elephantiasis, also known as lymphatic filariasis (LF), is a debilitating condition characterized by the thickening of the skin and muscles, primarily affecting the limbs, genitalia, and female breasts. Lymphatic filariasis is a major global health concern, affecting approximately 120 million people worldwide and having a significant impact on people's quality of life, mobility, and socio-economic status. Although LF is endemic in many parts of the world, including Africa, it is a neglected issue in Southern Africa, with little information available. According to the World Health Organisation,
