Otological symptoms and audiometric findings in patients with temporomandibular disorders: Costen's syndrome revisited.

OBJECTIVE: Otological symptoms (otalgia, subjective hearing loss, blocked ear sensation, tinnitus and vertigo) associated with temporomandibular disorders are documented features of Costen's syndrome. However, the origin of these symptoms and the causes of hearing loss are unknown. This study aimed to characterise hearing loss in a large number of patients with temporomandibular disorders. The causes of these symptoms were explored in patients with otological symptoms and normal audiometric findings. METHODS: A prospective case study and literature review were performed. The audiometric featu

Initial Investigation of the relation between extended computer use and temporomandibular joint disorders.

AIM: To determine if temporomandibular joint disorders (TMDs) are associated with extended computer use. MATERIALS AND METHODS: People with chronic pain and extensive computer use were recruited by means of a newspaper advertisement. Those who responded to the ad were asked to complete an online survey, which included questions on computer use, medical history, pain symptoms, lifestyle and mood. RESULTS: Ninety-two people completed the online survey, but none of them responded to all questions in the survey. Of the 88 respondents who reported their sex, 49 (56%) were female. Most of the resp

Efficiency of occlusal appliance therapy in secondary otalgia and temporomandibular disorders.

In clinical practice, it is commonly assumed that occlusal splints have therapeutic value in the treatment of temporomandibular disorders CTMD), but the evidence based on randomized controlled trials is scarce. This study evaluated the short-term (10-week) efficacy of a stabilization splint in subjects with recurrent secondary otalgia and active TMD treatment need using a randomized, controlled, double-blind design. Thirty-six subjects were randomly allocated to the two treatment groups: the stabilization splint and the control splint group. After 10 weeks' treatment, the intensity of secondar

Tinnitus in patients with temporomandibular joint internal derangement.

Tinnitus has commonly been reported in patients with temporomandibular joint (TMJ) disorders. The aim of this study was to determine if there was any correlation between arthographically verified disk displacement of the TMJ and ear symptoms related to tinnitus. Fifty-three patients with unilateral tinnitus and disk displacement and 82 individuals with disk displacement, but no tinnitus, were analyzed. Disk displacement was found to be present in the ipsilateral joint in all 53 patients with unilateral tinnitus, while the contralateral joint was asymptomatic in 50 (94.3%) of them. Patients wit

Transmastoid and Transtemporal Drainage of Petrous Apicitis with Otitis Media.

BACKGROUND: Petrous apicitis (PA) is a serious infection involving the apical portion of the petrous temporal bone. The classic triad of purulent otorrhea, ipsilateral abducens nerve palsy and retroorbital pain is rarely seen due to early detection and widespread use of antibiotics. Medical management is the primary treatment modality with surgery reserved for cases of recalcitrant petrous apex abscess. METHODS AND RESULTS: We presented a case of PA with previously untreated otitis media. After multidisciplinary evaluation, the patient was initially treated with intravenous antibiotics follow

Pediatricians' attitudes in management of acute otitis media and ear pain in Turkey.

OBJECTIVES: Acute otitis media (AOM) is predominantly a disease of childhood and one of the common reasons for prescribing antibiotics. Ear pain is the main symptom of AOM, with the result that parents frequently seek immediate medical assistance for their children. Antibiotic therapy for AOM does not provide symptomatic relief in the first 24 hours, and analgesics are commonly recommended for relieving the pain associated with AOM. The aims of the present study were to assess pediatricians' attitudes toward AOM and ear pain management in Turkey. METHODS: This multicenter descriptive question

Ear pain in patients with oropharynx carcinoma: how MRI contributes to the explanation of a prognostic and predictive symptom.

Reflex otalgia is a predictive and prognostic parameter for local control in patients with oropharynx carcinoma. Can a morphologic correlate of this important symptom be detected by MRI? Thirty-six patients were prospectively evaluated by MRI before radical radiotherapy. Sixteen patients had reflex otalgia; 20 did not. The oropharynx and adjacent regions were analyzed. Alteration was defined as effacement of anatomical structures, signal alteration or enhancement after contrast medium administration. The chi(2)-test was used to compare categorical parameters. In patients with reflex otalgia, a

Prospective study of the risk of not using prophylactic antibiotics in nasal packing for epistaxis.

BACKGROUND: There is wide variation in UK prescribing practice regarding prophylactic antibiotics for nasal packing in spontaneous epistaxis. There are few published cases of infective complications in such patients. METHOD: This prospective study examined 149 consecutive patients admitted to a tertiary otorhinolaryngology centre with spontaneous epistaxis, who underwent nasal packing, over a six-month period. In the first three-month period, 78 patients were routinely prescribed prophylactic antibiotics; in the second three months, 71 patients were not routinely prescribed antibiotics. Exclu

Hoarse voice in adults: an evidence-based approach to the 12 minute consultation.

BACKGROUND: The hoarse voice is a common presentation in the adult ENT clinic. It is estimated that otolaryngology/voice clinics receive over 50 000 patients with dysphonia each year. Good vocal function is estimated to be required for around 1/3 of the labour force to fulfil their job requirements. The assessment and management of the patient with a hoarse voice is potentially a complex and protracted process as the aetiology is often multi-factorial. This article provides a guide for the clinician in the general ENT clinic to make a concise, thorough assessment of the hoarse patient and enga

Acquired external auditory canal stenosis: clinical characteristics, surgical strategies and prognostic analysis.

BACKGROUND: Acquired external auditory canal stenosis (EACS) is a major cause of conductive hearing loss in children and adults, with multiple etiologies including otological surgical procedures, blunt trauma, and chronic inflammatory diseases, which can lead to persistent hearing loss and other adverse outcomes. Due to children's unique anatomical fragility and strong tissue proliferative capacity, the clinical management of EACS remains challenging. This study aims to explore the clinical characteristics, optimal surgical approaches and prognostic factors of traumatic EACS in children, so as
