Learning disabilities in children with sex chromosome anomalies.

Studies of clinical populations have suggested that genetic factors may be involved in the etiology of learning disabilities. The present study included 44 children (ages 7-16) with sex chromosome anomalies (SCA) who were identified in a 10-year sex chromosome screening of all newborns in 2 large hospitals and thus represents an unbiased sample of children with a genetic etiology. 17 chromosomally normal siblings are included as controls. All subjects were given IQ and achievement tests, and extensive, repeated school histories were taken from parents and school personnel. Results demonstrate

Primary writing tremor: motor cortex reorganisation and disinhibition.

BACKGROUND: Primary writing tremor (PWT) is a task-specific tremor of uncertain origin. There has been debate as to whether PWT represents a variant of essential tremor or a tremulous form of focal dystonia related to writer's cramp. In writer's cramp there is evidence of changes in intracortical inhibition (ICI), as well as cortical motor reorganisation. OBJECTIVE: To study corticomotor organisation and short-latency ICI in a patient with typical task-specific PWT. METHODS: Transcranial magnetic stimulation mapping of the corticomotor representation of the hand and studies of ICI using pair

Micrographia secondary to lenticular lesions.

Four patients with a stroke developed micrographia. In two patients, the condition was pure and in the two other patients it was associated with signs of writer's cramp. We conclude that infarct of the left lenticular nucleus could either mimic pure micrographia similar to that of Parkinson's disease or micrographia associated with dystonia.

Anticholinergics in adult-onset focal dystonia.

Anticholinergics are one of the most common treatments used in adult-onset focal dystonias although their general efficacy has not been proven. We have investigated the effects of intravenous administration of atropine, benztropine and chlorpheniramine, in comparison to normal saline, in 20 patients with adult-onset focal dystonias (spasmodic torticollis (9), cranial dystonia (6), writer's cramp (5), and have retrospectively reviewed the notes of 78 patients (spasmodic torticollis (38), cranial dystonia (25), writer's cramp (15), who had received chronic oral anticholinergic therapy in varying

Developmental Dysgraphia in Child and Adolescent Psychiatric-Mental Health Nursing: An Integrative Review.

BACKGROUND: Developmental dysgraphia affects 5% to 20% of school-age children and reaches 59% among children with ADHD or autism. The condition produces task avoidance, eroded self-esteem, and behavioral presentations that frequently result in psychiatric referral for anxiety, depression, school refusal, and behavioral disturbance. More than 37% of children with learning disabilities reach clinical thresholds for affective and ADHD problems. Despite this clinical footprint, dysgraphia remains nosologically contested and rarely identified within psychiatric evaluation. OBJECTIVES: This integra

Pick's Disease Presenting as Tremulous Parkinsonism with Limited Levodopa Response-A Rare Cause of Corticobasal Syndrome.

BACKGROUND: Corticobasal syndrome is a clinical diagnosis and common pathological causes are corticobasal degeneration, progressive supranuclear palsy and Alzheimer's disease. OBJECTIVES: We would like to highlight a rare but important differential of corticobasal syndrome. METHODS: A 78-year-old female had a 4-year history of predominantly right-hand rest tremor, worsening of handwriting but no change in cognition. The clinical examination showed right upper limb postural and kinetic tremor, mild wrist rigidity and reduced amplitude of right-sided finger tapping. She was initially diagnosed

Modulation of reflex responses in hand muscles during rhythmical finger tasks in a subject with writer's cramp.

The objective of this study was to examine phase- and task-dependent modulation of stretch reflexes during repetitive finger movements in writer's cramp, and compare them with normal controls from our previous study. A subject with writer's cramp conducted two rhythmic tasks, index finger abduction (RFA) and a pen-squeezing (RPS) task akin to handwriting. Stretch reflexes were evoked by mechanical perturbations at random phases of each task. Surface electromyograms (EMG) were recorded from two hand muscles, first dorsal interosseous (FDI) and flexor digitorum superficialis (FDS). The reflex re

Progressive agraphia can be a harbinger of degenerative dementia.

By investigating three patients with progressive agraphia, we explored the possibility that this entity is an early sign of degenerative dementia. Initially, these patients complained primarily of difficulties writing Kanji (Japanese morphograms) while other language and cognitive impairments were relatively milder. Impairments in writing Kana (Japanese syllabograms), verbal language, executive function, visuo- and visuospatial cognition and memory were identified by neuropsychological testing. The agraphia was compatible with a peripheral type, based on deficits at the interface between the c

The corpus callosum, interhemisphere interactions, and the function of the right hemisphere of the brain.

A complex clinical-neuropsychological study was performed by the Luriya method before and after surgery in 36 patients with arteriovenous malformations (AVM) of the corpus callosum. The symptoms of local lesions to the various parts of the corpus callosum are described. Symptoms of partial lesioning of the corpus callosum were found to be modality-specific, though only relatively. The symptoms of dyscopia or dysgraphia could appear in isolation fro each other. Combined lesions of the medial parts of the brain (cingulate cortex, frontal lobes) and the corpus callosum significantly increased the

Phonological agraphia following a focal anterior insulo-opercular infarction.

Following a unique infarction, restricted to the left anterior insula and the adjacent part of the intrasylvian frontal opercular cortex, an 83-year-old right-handed patient acutely developed a severe speech disorder that evolved into mere mutism within a few hours. After rapid recovery from mutism, oral language was characterized by severe apraxia of speech. In-depth language investigations further disclosed an isolated, highly selective disturbance of the spelling system (phonological agraphia) which resolved rapidly. One year after onset of neurological symptoms, the apraxia of speech had a

Tiapride as treatment for certain patients with idiopathic torsion dystonia.

Five female patients with idiopathic torsion dystonia (ITD) responded to treatment with tiapride, a selective D-2 dopamine antagonist. Previous treatments with various drugs, including anticholinergics, were ineffective. It is suggested that the previously reported genetic subgroups of ITD respond to different drug regimens. The autosomal dominant group responded to anticholinergics, while the autosomal recessive group responded best to a selective D-2 dopamine antagonist.

[Agraphia and preservation of music writing in a bilingual piano teacher].

A bilingual virtuoso piano teacher developed aphasia and amusia, probably due to cerebral embolism. The perfectly demarcated and unique lesion was located in the left posterior temporoparietal region. Language examinations in French and Italian demonstrated entirely comparable difficulties in both languages. The linguistic course was favorable after a period of auditory agnosia and global aphasia. Language became fluent again 3 months after the onset, with a marked vocabulary loss and phonemic paraphasias with attempts at self-correction. Repetition was altered markedly with a deficit in audit

[Determination of language dominance using EEG spectral analysis versus the Wada test in temporal epilepsy (right-handed subjects)].

Spectral analysis of the EEG alpha rhythm was studied in nine temporal epileptic right-handed patients in order to predict localization of the speech area. We studied the variation of the spectral power of the alpha rhythm during an activation paradigm previously validated in normal right-handed subjects. Significant alpha power decreases in the left hemisphere during writing with the right hand (as compared to resting) and/or significant alpha power decreases in the right hemisphere during left-hand recognition and classification of cardboard objects (as compared to resting) were considered a

The Rehapiano-Detecting, Measuring, and Analyzing Action Tremor Using Strain Gauges.

We have developed a device, the Rehapiano, for the fast and quantitative assessment of action tremor. It uses strain gauges to measure force exerted by individual fingers. This article verifies the device's capability to measure and monitor the development of upper limb tremor. The Rehapiano uses a precision, 24-bit, analog-to-digital converter and an Arduino microcomputer to transfer raw data via a USB interface to a computer for processing, database storage, and evaluation. First, our experiments validated the device by measuring simulated tremors with known frequencies. Second, we created a

Error-enhancing robot therapy to induce motor control improvement in childhood onset primary dystonia.

BACKGROUND: Robot-generated deviating forces during multijoint reaching movements have been applied to investigate motor control and to tune neuromotor adaptation. Can the application of force to limbs improve motor learning? In this framework, the response to altered dynamic environments of children affected by primary dystonia has never been studied. METHODS: As preliminary pilot study, eleven children with primary dystonia and eleven age-matched healthy control subjects were asked to perform upper limb movements, triangle-reaching (three directions) and circle-writing, using a haptic robot

Oscillatory coupling in writing and writer's cramp.

Writing is a highly skilled and overlearned movement. In patients suffering from writer's cramp, a focal task-induced dystonia, writing is impaired or even impossible due to involuntary muscle contractions and abnormal posture, which occur as soon as the person picks up a pen or within writing a few words. The underlying pathophysiological mechanisms of this movement disorder are not fully understood up to now. The aim of the present study was to unravel the oscillatory network underlying physiological writing in healthy subjects and dystonic writing in writer's cramp patients. Using whole-hea

Movement-related electroencephalographic desynchronization in patients with hand cramps: evidence for motor cortical involvement in focal dystonia.

We studied the dynamic changes in the amplitude of scalp electroencephalographic (EEG) oscillations to self-paced simple index finger abduction movements in patients with writer's cramp and compared them with those of normal aged-matched controls. The changes in EEG oscillations were measured in predefined frequency bands (8-10, 10-12, 12-20, and 20-30 Hz) by using the event-related desynchronization technique. Movements of the affected and unaffected hand in patients with writer's cramp showed significantly less reduction in 20- to 30-Hz power compared with controls. The differences in moveme
