Totally laparoscopic sigmoid colectomy: a simple and safe technique for intracorporeal anastomosis.

BACKGROUND: Current techniques of laparoscopic colectomy require an abdominal incision for extraction of the specimen. Although this incision is smaller than that for open laparotomy incision, it may reduce the advantages of laparoscopic surgery. In totally laparoscopic sigmoid colectomy, intracorporeal anastomosis is technically difficult. A safe and simple technique for circularly stapled intracorporeal anastomosis is described. METHODS: After mobilization of the colon and division of the mesentery, a semicircumferential colotomy is made at the anterior colonic wall just proximal to the tra

Evaluation of quality of life and function at 1 year after transanal endoscopic microsurgery.

AIM: Transanal endoscopic microsurgery (TEM) enables organ preservation after rectal tumour surgery. Its application is being expanded using adjuvant and neoadjuvant treatments. Our objective was to evaluate the changes over time in anorectal function, urinary symptoms and quality of life (QoL) in patients who had TEM surgery for a rectal tumour. METHOD: Between September 2009 and October 2012, a consecutive series of 102 patients underwent TEM at a single institution. Patients were asked to fill out standardized questionnaires at baseline and then at 6, 12, 26 and 52 weeks after surgery. The

Single-port robotic-assisted colorectal surgery using the da Vinci SP system: a preliminary noncomparative exploratory cohort study.

BACKGROUND: Minimally invasive surgery, particularly robotic-assisted platforms like the da Vinci systems, has revolutionized colorectal surgery by improving precision and recovery. The novel da Vinci Single-Port (SP) system enables procedures through a single incision, offering potential advantages over multiport systems. Following its 2023 clinical authorization in China via Hainan's special policy, this study evaluates the application of SP system in colorectal surgery within the Chinese population. OBJECTIVE: To assess the perioperative safety, feasibility, and short-term oncological outc

Short- and mid-term outcomes of transanal/transperineal minimally invasive surgery for locally recurrent rectal cancer.

BACKGROUND: Surgical resection, the primary treatment for locally recurrent rectal cancer (LRRC), is technically challenging. Transanal/transperineal minimally invasive surgery (Ta/Tp MIS) improves visualization and access through undisturbed planes. This study compared the short- and mid-term outcomes of Ta/Tp MIS with those of conventional non-Ta/Tp MIS for LRRC. METHODS: This retrospective observational study involved 98 patients who underwent curative-intent surgery for LRRC at a single tertiary cancer center between April 2008 and March 2022. Patients were classified into Ta/Tp MIS (n =

Laparoscopic delayed coloanal anastomosis without diverting ileostomy for low rectal cancer surgery: 85 consecutive patients from a single institution.

BACKGROUND: Surgical treatment for low rectal cancer septic complications often requires an ileostomy for fecal diversion. Delayed coloanal anastomosis (CAA) has been performed for several years to reduce septic complications and to avoid ileostomy. The aim of this study was to report the technical, functional and oncological results of delayed CAA in patients operated on for low rectal cancer focusing on pelvic septic complications. METHODS: All consecutive patients operated on for low rectal cancer suitable for total mesorectal excision and two-step delayed CAA at a single institution betwe

Single incision laparoscopic surgery - trans anal endoscopic microsurgery: A technological innovation.

Trans anal endoscopic microsurgery (TEM) first burst upon the scene several decades ago and then underwent a period of immersion. We have herein reported our experience in two cases who underwent TEM using laparoscopic techniques. The advent of single incision laparoscopic surgery (SILS) has made great inroads into various fields of general and gastrointestinal (GI) surgery. We decided to make use of the same technique in TEM for two patients who had large sessile villous adenomas of the rectum. We used this port and fixed it transanally to the edge of the anus. Carbon dioxide used for insuffl

Novel technique for anastomotic salvage using transanal minimally invasive surgery: A case report.

BACKGROUND: Anastomotic leak (AL) after low anterior resection (LAR) can be a highly morbid complication. The incidence of AL ranges from 5% to 20% depending on patient characteristics and the distance of the anastomosis from the anal verge. Low anastomoses and leaks pose technical challenges for endoscopic treatment. The aim of this report was to describe the use of a commercially available laparoscopic energy device through a transanal minimally invasive surgery (TAMIS) port for the management of a symptomatic leak not requiring relaparotomy (grade B) after a LAR with diverting loop ileostom

Sphincter preservation for distal rectal cancer--a goal worth achieving at all costs?

To assess the merits of currently available treatment options in the management of patients with low rectal cancer, a review of the medical literature pertaining to the operative and non-operative management of low rectal cancer was performed, with particular emphasis on sphincter preservation, oncological outcome, functional outcome, morbidity, quality of life, and patient preference. Low anterior resection (AR) is technically feasible in an increasing proportion of patients with low rectal cancer. The cost of sphincter preservation is the risk of morbidity and poor functional outcome in a si

The role of transanal endoscopic microsurgery for rectal tumors.

PURPOSE: The management of rectal tumors is complex, because of the balance between preserving rectoanal function and curing the patient. Transanal endoscopic microsurgery (TEM) is both an effective treatment for benign rectal tumors and early cancers, and a diagnostic tool for determining tumor depth, or for residual tumors of post endoscopic mucosal resection. In the present study, we evaluated the role of TEM in the management of rectal tumors. METHODS: Twenty-six patients with rectal tumors underwent TEM from December 2000 through March 2005 in our department. The operations were performe

The Role of MRI in the Preoperative Staging of Rectal Cancer: Ten-year Experience from a Single Tertiary Center.

BACKGROUND/AIM: Accurate preoperative staging of rectal cancer (CRC) guides treatment decisions, particularly regarding neoadjuvant chemoradiotherapy. Although magnetic resonance imaging (MRI) is recommended for local staging, questions persist regarding whether T stage or circumferential resection margin (CRM) involvement is the stronger predictor of treatment necessity. This study evaluated ten years of institutional experience to determine whether MRI-based CRM assessment provides superior prognostic guidance compared to tumor-node-metastasis (TNM) staging alone. PATIENTS AND METHODS: Betw
