Marrow stromal stem cell autologous transplantation in denervated fracture healing: an experimental study in rats.

OBJECTIVE: To investigate the influence of bone marrow stromal stem cell (BMSCs) transplantation on healing of fractures combined with central nerve injuries in rats. METHODS: Forty-eight healthy adult SD male rats were randomly divided into the following three groups (16 rats in each group): group A, simple (left) tibial fracture; group B, tibial fracture combined with T10 spinal cord transection (SCT); group C, tibial fracture combined with T10 SCT and BMSCs transplantation. The tibial fractures were stabilized with modular intramedullary nails and all operated hind limbs were further immob

Successful salvage of venous thrombosis of a free flap by creation of a venocutaneous fistula.

This report describes the creation of a venocutaneous fistula to salvage a free fibular osteocutaneous flap compromised by extensive venous thrombosis. This technique has previously been described for salvage of digital replants, but this is the first report of a venocutaneous fistula being used to salvage a free flap. A 21-year-old woman underwent a 9-cm resection of the distal left tibia for an aneurysmal bone cyst. A contralateral right fibular osteocutaneous free flap was used for reconstruction. On postoperative day 2, the skin paddle showed evidence of venous congestion. Reexploration de

Charcot Neuroarthropathy in Patients With Diabetes: An Updated Systematic Review of Surgical Management.

Charcot neuroarthropathy (CN) of the foot and ankle is a demanding clinical dilemma, and surgical management can be very complicated. Historically, the evidence guiding surgical management of CN has been small retrospective case series and expert opinions. The purpose of the present report was to provide a systematic review of studies published from 2009 to 2014 and to review the indications for surgery. A Medline search was performed, and a systematic review of studies discussing the surgical management of CN was undertaken. Thirty reports fit the inclusion criteria for our study, including 8

Pre-requisites for optimum centering of a tibiotalocalcaneal arthrodesis nail.

BACKGROUND: Purpose of study was to compare nail alignment, incidence of stress riser and fracture with straight and curved nails of different lengths. METHODS: We conducted a retrospective review of consecutive, initially 13 straight and subsequently 15 curved nails implanted in 17 men and 11 women. Angles of incidence and reflection subtended by nail tip with inner tibial cortex were measured. Cortical hypertrophy was assessed on follow-up radiographs. RESULTS: Angles of incidence and reflection were greater with short straight nails, less with long straight nails and stayed close to zero

Complications in the use of the Bailey-Dubow extensible nail.

The results of 40 extensible intramedullary nailing procedures in 15 children who had osteogenesis imperfecta were reviewed to identify risk factors leading to complications of this method of treatment. There were 40 complications, 17 major and 23 minor. The 17 major complications led to 15 additional procedures, 10 of which were to remove or replace the implant. The prevalence of major complications was highest in patients who were younger than 5 years when the nail was inserted. Nails placed in the tibia tended to produce a higher incidence of major complications than did those placed in the

The floating knee: Ipsilateral fractures of the tibia and femur.

Fifty-one extremities with ipsilateral tibial and femoral fractures are reviewed. Some children are included in the study. Most of the 47 patients are young men who have multiple injuries with high velocity trauma. The involved extremities are classified according to the injury site: Type 1 fractures involve both shafts; type 2-A fractures involve the knee joint; type 2-B fractures involve hip or ankle joints. Complications are frequent. Delayed union or nonunion occurred in 44.6% of the total number of bones involved. Sixty to seventy percent of the adult patients showed significant permanent

The "Bergamo Approach" for Pediatric and Adolescent Polytrauma-A One-Center Experience.

Introduction. Pediatric polytrauma (PPT) and major trauma in pediatric patients (PMT) present unique challenges compared to adult trauma care due to distinct anatomical and physiological differences. PPT/PMT remains the leading cause of death in children, responsible for over 50% of pediatric deaths and 15% of pediatric hospital admissions due to its long-term effects. This single-institution study focuses on the initial management of PPT/PMT from an orthopedics and traumatology point of view. Material and Methods. In the present study, data of PPT/PMT managed in one single institution, an aca

[Experiences with a helical femoral nail (LFN®). A multicenter study].

INTRODUCTION: Antegrade intramedullary nailing is the method of choice in most femoral shaft fractures. The trochanteric entry portal of classic femoral nails is in close proximity to the piriformis tendon, the gluteus minimus tendon, the obturator tendons, and the medial femoral circumflex artery. Nail insertion lateral to the tip of the greater trochanter may be more favorable but needs the use of a helical implant. MATERIAL AND METHODS: Measurement of the reamer pathway through an entry point lateral to the superior trochanteric border was performed with a three-dimensional motion tracking

Tibial Lengthening with a Motorized Intramedullary Lengthening Nail.

BACKGROUND: Tibial lengthening with use of a motorized intramedullary lengthening nail (MILN) provides precise deformity correction and distraction osteogenesis without requiring external fixation1. A MILN can be utilized for many etiologies requiring lengthening, such as congenital (e.g., short stature, limb deficiency) or acquired (e.g., post-infectious, traumatic)2. The present video article describes the surgical technique for MILN use in the tibia in a skeletally mature patient with use of the Precice System (Globus Medical). DESCRIPTION: The preoperative patient evaluation starts with a
