Cannabis use during treatment for alcohol use disorders predicts alcohol treatment outcomes.

AIMS: To compare post-treatment alcohol use between those who use cannabis and those who abstain during treatment for alcohol use disorders (AUD); and to examine potential cannabis use thresholds by comparing post-treatment alcohol use between four frequency groups of cannabis users relative to abstainers. DESIGN: Secondary analyses of the Combined Pharmacotherapies and Behavioral Interventions (COMBINE) Study, a randomized control trial of AUD treatments. The current study compares longitudinal drinking data between those who used cannabis versus those who abstained during COMBINE treatment.

Brain Correlates of the Alcohol Use Disorder Pharmacotherapy Response: A Systematic Review of Neuroimaging Studies.

BACKGROUND: Although Alcohol Use Disorder (AUD) is highly prevalent worldwide, treating this condition remains challenging. Further, potential treatments for AUD do not fully address alcohol-induced neuroadaptive changes. Understanding the effects of pharmacotherapies for AUD on the human brain may lead to tailored, more effective treatments, and improved individual clinical outcomes. OBJECTIVES: We systematically reviewed the literature for studies investigating pharmacotherapies for AUD that included neuroimaging-based treatment outcomes. We searched the PubMed, Scielo, and PsycINFO databas

Incidence and Progression of Alcohol-Associated Liver Disease After Medical Therapy for Alcohol Use Disorder.

IMPORTANCE: Alcohol-associated liver disease (ALD) is one of the most devastating complications of alcohol use disorder (AUD), an increasingly prevalent condition. Medical addiction therapy for AUD may play a role in protecting against the development and progression of ALD. OBJECTIVE: To ascertain whether medical addiction therapy was associated with an altered risk of developing ALD in patients with AUD. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study used the Mass General Brigham Biobank, an ongoing research initiative that had recruited 127 480 patients between its sta

[Maintaining abstinence after alcohol detoxification].

"To quit drinking" is not the panacea of alcohol dependence treatment; it is only its first step. Abstinence should be considered more as a mean than a purpose of the after-withdrawal cares. The frequent resistance of the alcoholic patient to undertake in a long term abstinence can be by-passed by suggesting to fix himself renewable terms for periods during which he feels rather confident to raise the bet of a "most accomplished possible" abstinence. To facilitate the realization and the preservation of this abstinence in the best conditions (potentiation of the profits and minimization of the

Medicaid Managed Care Plan Alignment With State Substance Use Disorder Treatment Coverage Requirements.

UNLABELLED: Policy Points States contract with Medicaid managed care plans to administer benefits for roughly 70 million Medicaid enrollees, yet little is known about how plan benefit policies for substance use disorder (SUD) treatment medications align with state requirements. In this study, we found that among the population of 167 Medicaid managed care plans responsible for SUD pharmacy benefits in 2021, many did not align with state requirements to cover SUD treatment medications. Many plans imposed prior authorization requirements on these medications, even when prohibited from doing so.

Opioid antagonists for pharmacological treatment of alcohol dependence - a critical review.

Alcohol dependence is a widespread psychiatric disorder. While relapse prevention therapy in alcoholism was exclusively dominated by social and psychological treatments for many years, in the last decades the benefits of pharmacological agents for the rehabilitation treatment in alcoholism have become increasingly evident. Naltrexone, an opiate receptor antagonist, blocks the pleasant and reinforcing effects of alcohol by preventing the stimulation of opioid receptors and the reduction of dopamine release in the ventral tegmental area (VTA). Clinical evidence about the effectiveness of the sub

Optimizing outpatient pharmacologic treatment for veterans with alcohol use disorder using an academic detailing approach for primary care providers in the United States Department of Veterans Affairs: A retrospective quasi-experimental study.

BACKGROUND AND AIMS: Although evidence-based medications for alcohol use disorder (AUD) exist, they are alarmingly under-prescribed, particularly for United States (US) veterans. Only 9.5% of veterans who had an alcohol-related overdose death were prescribed a medication for alcohol use disorder (MAUD) in the year prior. Academic detailing (AD), an educational outreach targeting primary care providers (physicians, nurse practitioners and physician assistants), was implemented at the US Department of Veterans Affairs (VA) to improve MAUD prescribing rates among veterans with AUD. This study eva

Cloninger's typology and treatment outcome in alcohol-dependent subjects during pharmacotherapy with naltrexone.

Naltrexone is an opiate receptor antagonist mainly at the micro-receptor that is thought to reduce the positively reinforcing, pleasurable effects of alcohol and to reduce craving. An increase in time to first relapse to heavy drinking has been the most consistent finding obtained with naltrexone, although not all trials including two of the largest have been positive. Inconsistent outcome data suggest that effectiveness varies among different subgroups of patients. This paper re-evaluates recent data on the effectiveness of naltrexone in subjects differentiated according to Cloninger Type I a

Assessment and treatment of insomnia in adult patients with alcohol use disorders.

Insomnia in patients with alcohol dependence has increasingly become a target of treatment due to its prevalence, persistence, and associations with relapse and suicidal thoughts, as well as randomized controlled studies demonstrating efficacy with behavior therapies and non-addictive medications. This article focuses on assessing and treating insomnia that persists despite 4 or more weeks of sobriety in alcohol-dependent adults. Selecting among the various options for treatment follows a comprehensive assessment of insomnia and its multifactorial causes. In addition to chronic, heavy alcohol

[Impact of functional social support for abstinence after inpatient detoxification].

OBJECTIVE: Several studies discovered an association between social support and therapy outcomes in patients suffering from alcoholism. However, less is known about the influence of functional social support (fss) on these outcomes. In this prospective study, we examined the impact of fss on different alcohol-dependence related variables in 132 alcohol-dependent patients. METHODS: The fss was measured with the Medical Outcome Study (MOS) Social Support Survey at baseline and at the end of the study. RESULTS: We found significantly higher fss levels in patients with a current partnership. How

Combined Pharmacotherapy and Cognitive Behavioral Therapy for Adults With Alcohol or Substance Use Disorders: A Systematic Review and Meta-analysis.

IMPORTANCE: Substance use disorders (SUDs) represent a pressing public health concern. Combined behavioral and pharmacological interventions are considered best practices for addiction. Cognitive behavioral therapy (CBT) is a first-line intervention, yet the superiority of CBT compared with other behavioral treatments when combined with pharmacotherapy remains unclear. An understanding of the effects of combined CBT and pharmacotherapy will inform best-practice guidelines for treatment of SUD. OBJECTIVE: To conduct a meta-analysis of the published literature on combined CBT and pharmacotherap

Predictors of abstinence from heavy drinking during treatment in COMBINE and external validation in PREDICT.

BACKGROUND: The goal of the current study was to use tree-based methods (Zhang and Singer, 2010, Recursive Partitioning and Applications, 2nd ed. Springer, New York) to identify predictors of abstinence from heavy drinking in COMBINE (Anton et al. JAMA 2006; 295:2003), the largest study of pharmacotherapy for alcoholism in the United States to date, and to validate these results in PREDICT (Mann et al. Addict Biol 2012; 18:937), a parallel study conducted in Germany. METHODS: We compared a classification tree constructed according to purely statistical criteria to a tree constructed according
