Nasal high flow reduces dead space.

UNLABELLED: Recent studies show that nasal high flow (NHF) therapy can support ventilation in patients with acute or chronic respiratory disorders. Clearance of dead space has been suggested as being the key mechanism of respiratory support with NHF therapy. The hypothesis of this study was that NHF in a dose-dependent manner can clear dead space of the upper airways from expired air and decrease rebreathing. The randomized crossover study involved 10 volunteers using scintigraphy with 81mKrypton (81mKr) gas during a breath-holding maneuver with closed mouth and in 3 nasally breathing tracheot

The effect of a peptide-containing synthetic lung surfactant on gas exchange and lung mechanics in a rabbit model of surfactant depletion.

BACKGROUND: Currently, a new generation of synthetic pulmonary surfactants is being developed that may eventually replace animal-derived surfactants used in the treatment of respiratory distress syndrome. Enlightened by this, we prepared a synthetic peptide-containing surfactant (Synsurf) consisting of phospholipids and poly-l-lysine electrostatically bonded to poly-l-glutamic acid. Our objective in this study was to investigate if bronchoalveolar lavage (BAL)-induced acute lung injury and surfactant deficiency with accompanying hypoxemia and increased alveolar and physiological dead space is

Physiological dissociation between ventilatory ratio and ventilatory efficiency in patients with ARDS.

BACKGROUND: The ventilatory ratio (VR) is frequently used as a surrogate marker of ventilatory efficiency in patients with ARDS. However, its ability to reflect changes in alveolar ventilation (V̇Talv/VT) when respiratory mechanics are modified remains unknown. This study aimed to evaluate the relationship between VR and V̇Talv/VT during sequential changes in respiratory mechanics=, tidal volume (VT), and minute ventilation (V̇E) in patients with ARDS. METHODS: This was a secondary analysis of a quasi-experimental, repeated-measures study conducted in a single-center adult ICU. Twenty-two pat

Determination of the 95% Effective Dose of Remifentanil for Moderate Sedation During Gastroscopy in Obese Patients.

INTRODUCTION: Obesity increases the risk of hypoxemia during gastroscopy. The optimal remifentanil dose for effective and safe conscious sedation in this population remained unclear. Applying the biased coin design up-and-down sequential method, we aimed to determine the 95% Effective Dose (ED95) of remifentanil required for gastroscopy in obese patients. MATERIALS AND METHODS: In this prospective interventional dose-finding study, patients aged 18-65 years with a BMI ≥ 30 kg/m2 (ASA class I-III) undergoing elective gastroscopy received remifentanil via target-controlled infusion. Patients wi

End-tidal carbon dioxide monitoring to predict hypovolaemic shock and the subsequent need for blood transfusions in adult pre-hospital trauma patients: a systematic review.

INTRODUCTION: Traumatic haemorrhage is a major cause of preventable death, and blood transfusion is a crucial component of damage control resuscitation. Several diagnostic strategies exist to anticipate hypovolaemic shock and subsequent blood transfusion requirements, but their pre-hospital utility is inconsistent. Capnography or end-tidal carbon dioxide (EtCO2) measurement has received growing recognition for prognostic capability in metabolic, respiratory and cardiac disease. This systematic review examines EtCO2 efficacy in predicting hypovolaemic shock and subsequent blood transfusion in p

Ventilation-perfusion effects of negative-pressure ventilation: insights from an experimental rat model.

BACKGROUND: Mechanical ventilation typically utilizes positive-pressure ventilation (PPV), which fundamentally differs from physiological pressure conditions. In contrast, negative-pressure ventilation (NPV) more closely mimics physiological pressure conditions; however, its impact on ventilation-perfusion matching remains unclear. Therefore, we compared PPV and NPV in terms of their effects on ventilation-perfusion matching and determined the consequences of increasing end-expiratory pressure (EEP). METHODS: Anesthetized rats (n = 9) were ventilated using PPV at a positive EEP of 0, 3, 6, an

Oxygen Saturation Thresholds for Opioid-induced Respiratory Depression: A Systematic Review.

INTRODUCTION: Hypoventilation and hypercapnia are the primary physiological indicators of opioid-induced respiratory depression (OIRD), while hypoxia is typically a later manifestation. However, use of pulse oximetry to measure oxygen saturation (SpO2) level remains the most widely available and commonly used monitoring modality in both clinical and community settings, whereas capnography is not routinely accessible. Given this reality, it is important to evaluate hypoxia thresholds reported in the literature to inform practical detection and intervention strategies. Oxygen saturation monitori

Oliceridine for postoperative pain and opioid-related complications: The intravenous oliceridine and opioid-related complications (VOLITION) prospective cohort study.

BACKGROUND: Oliceridine is a G protein-selective μ-opioid receptor agonist with reduced β-arrestin activation that may produce fewer opioid-related adverse effects (ORAEs) than traditional opioids. The VOLITION prospective cohort study evaluated oliceridine for management of postoperative pain and evaluated ORAEs. METHODS: We enrolled 204 patients scheduled for major noncardiac surgery. Our primary aim was to quantify the proportion of patients having respiratory compromise over the initial two postoperative days. Cardiorespiratory function was continuously monitored with pulse oximetry, capn

Complete Endotracheal Tube Obstruction Due to Highly Viscous Secretions Immediately After Tracheal Intubation: A Case Report.

Obstruction of the endotracheal tube (ETT) from secretions can cause severe respiratory distress and potentially life-threatening complications; therefore, prompt decisions and management are needed. Cases of ETT obstruction have been observed in patients who have been ventilated for extended periods but are rare in patients immediately after endotracheal intubation. Herein, we present a rare case of ETT obstruction after endotracheal intubation. A 90-year-old male was scheduled for open reduction and internal fixation (ORIF) under general anesthesia for a femoral fracture. The patient had a h

One-year aerobic exercise altered cerebral vasomotor reactivity in mild cognitive impairment.

The purpose of this study was to test the hypothesis that changes in cerebral vasomotor reactivity (CVMR) after 1-yr aerobic exercise training (AET) are associated with cognitive performances in individuals with amnestic mild cognitive impairment (MCI). Seventy sedentary patients with amnestic MCI were randomized to 1-yr moderate-to-vigorous intensity AET or stretching and toning (SAT) interventions. Cerebral blood flow velocity (CBFV) with transcranial Doppler, mean arterial pressure (MAP) with finapres plethysmograph, and EtCO2 with capnography were measured during hyperventilation (hypocapn

COPD phenotypes on computed tomography and its correlation with selected lung function variables in severe patients.

BACKGROUND: Computed tomography (CT) phenotypic characterization helps in understanding the clinical diversity of chronic obstructive pulmonary disease (COPD) patients, but its clinical relevance and its relationship with functional features are not clarified. Volumetric capnography (VC) uses the principle of gas washout and analyzes the pattern of CO2 elimination as a function of expired volume. The main variables analyzed were end-tidal concentration of carbon dioxide (ETCO2), Slope of phase 2 (Slp2), and Slope of phase 3 (Slp3) of capnogram, the curve which represents the total amount of CO

Correlation between minute carbon dioxide elimination and pulmonary blood flow in single-ventricle patients after stage 1 palliation and 2-ventricle patients with intracardiac shunts: A pilot study.

BACKGROUND: Assessment of pulmonary blood flow and cardiac output is critical in the postoperative management of patients with single-ventricle physiology or 2-ventricle physiology with intracardiac shunting. Currently, such hemodynamic data are only obtainable by invasive procedures, such as cardiac catheterization or the use of a pulmonary artery catheter. Ready availability of such information, especially if attainable noninvasively, could be a valuable addition to postoperative management. AIMS: The aim of this study was to assess the correlation between volume of CO2 elimination obtained
