Fragile and Brittle Bone Disease or Osteogenesis Imperfecta: A Case Report.

AIM AND OBJECTIVE: The aim and objective of this report is to describe the dental management of 11-year-old patient with type III osteogenesis imperfecta (OI). BACKGROUND: Osteogenesis imperfecta or brittle bone disease is caused by mutations in the collegen type I gene which is a heterogeneous rare connective tissue disorder. Dentinogenesis imperfecta, hearing impairment, scoliosis, sclera is blue, hyperlaxity of ligaments, and fragile skin are other common features. Individuals having positive family history suggest a straightforward diagnosis of OI but can be difficult in the absence of af

Osteogenesis imperfecta type VI: a form of brittle bone disease with a mineralization defect.

Osteogenesis imperfecta (OI) is a heritable disease of bone in which the hallmark is bone fragility. Usually, the disorder is divided into four groups on clinical grounds. We previously described a group of patients initially classified with OI type IV who had a discrete phenotype including hyperplastic callus formation without evidence of mutations in type I collagen. We called that disease entity OI type V. In this study, we describe another group of 8 patients initially diagnosed with OI type IV who share unique, common characteristics. We propose to name this disorder "OI type VI." Fractur

Wnt-Responsive Odontoblasts Secrete New Dentin after Superficial Tooth Injury.

The objective of our experiments was to identify new therapeutic strategies to stimulate dentin formation in an adult tooth. To address this objective, we evaluated dentin production in 2 acute trauma models: one involving a pulp exposure and the other involving a superficial dentin injury. Molecular, cellular, and histologic analyses revealed that in response to a severe injury, where the pulp is exposed to the oral cavity, cell death is rampant and the repair response initiates from surviving pulp cells and, to a lesser extent, surviving odontoblasts. When an injury is superficial, as in the

Expression of amelogenins in developing embryonic and neonatal rat teeth.

OBJECTIVE: Tooth enamel is formed by ameloblasts, which are derived from epitheliums and secrete an extracellular matrix containing a complex arrangement of protein components. The epithelial component, referred to as the enamel organ, contains a layer of cells that secrete an organic matrix that biomineralizes to become tooth enamel. Adjacent ectomesenchyme cells differentiate to become dentinproducing odontoblasts. These two mineralized matrices form the crown of the vertebrate tooth. Therefore, amelogenins play a critical role in tooth enamel formation. We have examined the expression patte

Osteogenesis imperfecta: practical treatment guidelines.

Osteogenesis imperfecta (OI), an inherited connective tissue disorder of remarkable clinical variability, is caused by a quantitative or qualitative defect in collagen synthesis and is characterised by bone fragility. The number of fractures and deformities, and the age at which they begin greatly influence the prognosis and the achievement of walking and autonomy. A multidisciplinary team approach is essential for diagnosis, for communication with patient and parents, and to tailor treatment needs to the severity of the disease and the age of the patient. Three types of treatment are availabl

[Pathologic anatomy of osteogenesis imperfecta. Light and electron microscopic studies of supportive tissue and skin].

Osteogenesis imperfecta (OI) is characterised by an abnormal fragility of the bones, and more or less marked blue sclerae. The mode of inheritance is both autosomal dominant and recessive. Facultatively, dentinogenesis imperfecta, abnormal laxness of joint ligaments, and, in early adulthood, deafness, may also be associated. In numerous cases, dwarfism, which may be very marked, and kyphoscoliosis of varying degrees, may also be found. The presumed cause of OI is a disturbance of collagen metabolism, which is associated with an insufficiency state of the osteoblasts; this leads to inadequate f

Regulation of Reactionary Dentine Formation.

During the treatment of dental caries that has not penetrated the tooth pulp, maintenance of as much unaffected dentine as possible is a major goal during the physical removal of decayed mineral. Damage to dentine leads to release of fossilized factors (transforming growth factor-β [TGF-β] and bone morphogenic protein [BMP]) in the dentine that are believed to stimulate odontoblasts to secrete new "tertiary" dentine (reactionary dentine). This is formed on the pulpal surface of existing dentine and rethickens the dentine. We have previously shown that activation of Wnt/β-catenin signaling is p

Effects of protein deficiency induced by raw soy with and without sucrose on dentine formation and dentinal caries in young rats.

OBJECTIVES: The effects of protein deficiency and sucrose on formation and mineralization of dentine and dental caries in the molars of young rats were investigated. MATERIALS AND METHODS: Two groups of weaning Wistar rats received raw soy flour to induce protein deficiency with sucrose or starch as the carbohydrate source; the other two groups received skim milk powder as the protein source with sucrose or starch as the carbohydrate source. At the onset, tetracycline was injected to mark the dentine formed at that moment. After 6 weeks, lower molars were sectioned sagittally, and the areas o

Is hard tissue formation in the dental pulp after the death of the primary odontoblasts a regenerative or a reparative process?

OBJECTIVES: Conceptually, two types of tertiary dentine may be produced in response to caries and environmental irritations: "reactionary dentine" that is secreted by existing primary odontoblasts and "reparative dentine", formed after the death of the odontoblasts by proliferation and differentiation of progenitor cells into odontoblast-like cells. Because histologic evidence for tubular dentine generated by newly differentiated odontoblast-like cells is lacking in human teeth, the present study examined pulpal cellular changes associated with caries/restorations, in the presence or absence o

Epigenetic modulation of odontoblast differentiation: implications for regenerative endodontics.

OBJECTIVE: Conventional restoration of dentine loss relies on the use of inert or bioactive materials that fail to replicate the resilience of natural tissue. Odontoblasts are essential for dentine formation and its structural integrity. Hence, understanding the molecular signals that control odontoblastic differentiation is essential for developing regenerative endodontic strategies. Epigenetic factors such as histone variants are important regulators of cell differentiation. This study investigated the role of the histone variant H2A.Z.1 in regulating odontoblast differentiation. METHODS: H

Ameloblasts express type I collagen during amelogenesis.

Enamel and enameloid, the highly mineralized tooth-covering tissues in living vertebrates, are different in their matrix composition. Enamel, a unique product of ameloblasts, principally contains enamel matrix proteins (EMPs), while enameloid possesses collagen fibrils and probably receives contributions from both odontoblasts and ameloblasts. Here we focused on type I collagen (COL1A1) and amelogenin (AMEL) gene expression during enameloid and enamel formation throughout ontogeny in the caudate amphibian, Pleurodeles waltl. In this model, pre-metamorphic teeth possess enameloid and enamel, wh

Evaluation of stomatognathic problems in children with osteogenesis imperfecta (osteogenesis imperfecta - oi) - preliminary study.

UNLABELLED: According to epidemiological data, muscular dysfunctions of the masticatory system occur in 15-23% of the population. Preventive examinations of functional disorders of the stomatognathic system are, therefore, of particular importance. A distinct group of patients exposed to dysfunctions in the area of the masticatory organ locomotor apparatus comprises those with genetic diseases characterised by disorders in collagen formation. One of such diseases is osteogenesis imperfecta (OI) and dentinogenesis imperfecta that usually goes together with the former. AIM: The objective of thi

Evaluation of oral tissue response and blood levels of mercury released from dental amalgam in rats.

UNLABELLED: Dental amalgam is the most common restorative material used in dentistry. It was reported that amalgam might constitute potential toxic hazards to pregnant patients and foetuses through mercury release and absorption. The present study aimed to investigate the vital tissue response in contact to dental amalgam plus determination of blood mercury levels in mother and offspring Wistar strain albino rats. Pregnant mothers were divided into two main groups each had dental amalgam implanted into either an oral mucosa incision or a bony socket following extraction. Third and fourth group

Dental diagnostic problems of potential genetic significance: the dental practitioner as geneticist in the developmental disabilities clinic.

A high incidence of aberrant dental formation can be anticipated in developmentally disabled children, particularly where the etiologic factors for developmental retardation are presumably of organic origin. The dentist, practicing in the developmental clinic, must frequently determine the developmental significance of such dental variation. In this capacity, his familiarity with the dental features of genetic disease can contribute significantly to more precise diagnosis.

Inflammation-Regeneration Axis of Dental Pulp Stem Cells: Wnt/NF-κB Crosstalk.

BACKGROUND: Inflammation critically determines dental pulp regenerative outcomes, with dental pulp stem cells (DPSCs) orchestrating tissue homeostasis through differentiation, self-renewal and immunomodulation processes dynamically regulated by Wnt/β-catenin and NF-κB signaling crosstalk. Given the rising therapeutic potential of Wnt-targeted interventions in dental tissue engineering, elucidating these molecular interactions under pathological conditions is essential for developing regenerative therapeutics capable of simultaneously promoting reparative dentinogenesis while resolving inflamma
