Antiseizure Medications Impact Mitochondrial Ion Channels via Novel Bioenergetic and Neural Mechanisms.

Antiseizure medications (ASMs) have traditionally been characterized by their modulation of neuronal ion channels and synaptic processes; however, accumulating evidence indicates that numerous ASMs also directly modulate mitochondrial function. Specifically, several ASMs interact with ion channels located in both the inner and outer mitochondrial membranes, including the voltage-dependent anion channel (VDAC), the mitochondrial calcium uniporter (MCU), the mitochondrial Na+/Ca2+ exchanger (NCLX), the mitochondrial permeability transition pore (mPTP), and mitochondrial ATP-sensitive potassium c

Hypoglycemia Occurrence and Risk Factors in Insulin Independent Patients After Total Pancreatectomy With Islet Autotransplantation.

BACKGROUND: Total pancreatectomy with islet auto-transplantation (TPIAT) alleviates pain and preserves islet function in patients with severe pancreatitis. Although approximately one third of recipients achieve insulin independence, spontaneous hypoglycemia has been observed in this population. Its frequency, risk factors, and management remain poorly characterized. METHODS: Using prospectively collected data on insulin-independent (TPIAT) recipients at the University of Minnesota (2010-2023), we compared patients who developed hypoglycemia (defined as documented blood glucose < 70 mg/dL) to

Post-bariatric hypoglycaemia: an under-recognized complication of weight loss surgery - a case series.

Post-bariatric hypoglycaemia (PBH) has emerged as a significant, under-recognized complication following Roux-en-Y gastric bypass (RYGB) and other weight loss surgeries. The purpose of this study was to characterize the timing, clinical presentation, diagnosis and management outcomes of PBH in a diverse case series from a multi-speciality centre in Southern India owing to a predominantly high-carbohydrate diet and distinct bariatric phenotype. This was a retrospective observational case series analysis from a multidisciplinary bariatric clinic at a tertiary teaching hospital in Southern India

Eriodictyol stimulates insulin secretion through cAMP/PKA signaling pathway in mice islets.

Eriodictyol, a flavonoid isolated from Lyonia ovalifolia, was found to be the most potent insulin secretagogue in our preliminary studies. Here, we explored mechanism(s) of insulin secretory activity of eriodictyol in vitro and in vivo. Mice islets and MIN6 cells were incubated in basal and stimulatory glucose containing eriodictyol with or without agonist/antagonist. Secreted insulin and cAMP contents were measured using ELISA kits. K+- and Ca2+-channels currents were recorded with patch-clamp technique. Oral glucose tolerance test and plasma insulin was evaluated in non-diabetic and diabetic

Sulfonylurea and K(+)-channel opener sensitivity of K(ATP) channels. Functional coupling of Kir6.2 and SUR1 subunits.

The sensitivity of K(ATP) channels to high-affinity block by sulfonylureas and to stimulation by K(+) channel openers and MgADP (PCOs) is conferred by the regulatory sulfonylurea receptor (SUR) subunit, whereas ATP inhibits the channel through interaction with the inward rectifier (Kir6.2) subunit. Phosphatidylinositol 4, 5-bisphosphate (PIP(2)) profoundly antagonized ATP inhibition of K(ATP) channels expressed from cloned Kir6.2+SUR1 subunits, but also abolished high affinity tolbutamide sensitivity. By stabilizing the open state of the channel, PIP(2) drives the channel away from closed stat

Case Report: Pasireotide treatment in neonatal congenital hyperinsulinism due to a homozygous ABCC8 mutation.

Mutations in ABCC8 and KCNJ11 are associated with the most severe and diazoxide-resistant forms of congenital hyperinsulinism (CHI). Somatostatin analogues are commonly used off-label as second-line treatment. While octreotide and lanreotide are the most established options, pasireotide-a second-generation somatostatin analogue with higher affinity for somatostatin receptor subtype 5-has been hypothesized to offer improved suppression of insulin secretion. We report the off-label use of subcutaneous pasireotide in an infant with severe CHI due to a homozygous ABCC8 mutation. The patient presen

Nesidioblastosis and Insulinoma: A Rare Coexistence and a Therapeutic Challenge.

Background: Nesidioblastosis and insulinoma are disorders of the endocrine pancreas causing endogenous hyperinsulinemic hypoglycemia. Their coexistence is very unusual and treatment represents a still unresolved dilemma. Case Description: The patient was a 43-year-old Caucasian woman, with a 2-year history of repeated severe hypoglycemic events. The diagnostic work-up was strongly suggestive of insulinoma and the patient was submitted to surgical treatment carried out laparoscopically under robotic assistance. However, surgical exploration and intraoperative ultrasonography failed to detect a
