Extended donor criteria for lung transplantation--a clinical reality.

OBJECTIVE: Standard lung donor criteria have been established on opinions and individual experiences rather than on existing evidence. Since the scarcity of donor organs is one of the major limitations to lung transplantation, extension of donor lung criteria might considerably increase the donor pool. This study therefore evaluates the outcome, achieved with the use of extended donors versus standard donors and aims to redefine lung donor criteria. METHODS: We performed a retrospective analysis of 98 consecutive primary lung transplantations from 94 donors from 1/2001 to 12/2002. Donors were

Parenteral buprenorphine-naloxone abuse is a major cause of fatal buprenorphine-related poisoning.

Buprenorphine (BPN) medication for opioid maintenance treatment in Finland consists predominantly of buprenorphine-naloxone (BNX). Both BPN and BNX are associated with diversion, abuse and non-medically supervised use worldwide. Our purpose was to estimate the proportion of BNX to all BPN-related fatalities. The material consisted of 225 deceased drug abusers in Finland from January 2010 to June 2011 with a positive BPN and/or norbuprenorphine (NOR) and/or naloxone (NX) finding in urine. The data were divided into three groups based on the urine NX and BPN concentrations. The "Parenteral BNX"

Confirmation of gestational exposure to alprazolam by analysis of biological matrices in a newborn with neonatal sepsis.

BACKGROUND: Different biological matrices are suitable for drug testing in newborns presenting with an acute withdrawal syndrome. CASE REPORT: The newborn of a mother reporting alprazolam use during pregnancy presented with respiratory distress and clinical features consistent with neonatal withdrawal syndrome or neonatal sepsis of vertical transmission. Alprazolam and its main metabolite (alpha-hydroxyalprazolam) were detected in cord serum, neonatal urine and also in neonatal hair, meconium and placenta, accounting for both acute and chronic exposure to this benzodiazepine during intrauteri

Concurrent crack and powder cocaine users from Sao Paulo: do they represent a different group?

BACKGROUND: Cocaine abuse is a serious and socially damaging illegal drug problem. Different routes of administration are associated with a specific progression of use, different degrees of abuse liability, propensity for dependence and treatment response. There have been relatively few studies comparing different cocaine users groups and no studies into the characterization of the group of individuals reporting concurrent use of powder cocaine and crack cocaine. METHODS: Six hundred and ninety-nine cocaine users were assessed during the period August 1997 to October 1998 in one outpatient an

Assessment of nicotine pharmacokinetics and abuse liability in randomized, crossover studies of Vuse Alto electronic nicotine delivery systems.

INTRODUCTION: Understanding the neuropharmacological effects and abuse liability (AL) of electronic nicotine delivery systems (ENDS) is essential for evaluating their potential role in tobacco harm reduction (THR). Nicotine delivered via ENDS engages central nervous system (CNS) pathways involved in reinforcement, craving, and dependence. This research characterized nicotine pharmacokinetics (PK) and AL of Vuse Alto ENDS across multiple nicotine concentrations and flavor and contextualized these findings relative to combustible cigarettes and nicotine replacement therapy (NRT) gum. METHODS: T

Qualitative and quantitative in silico toxicity profiling of "angel dust": phencyclidine (PCP) analogues as new psychoactive substances (3-HO-PCP, 3-MeO-PCP, 4-MeO-PCP, 3-HO-PCE, 3-MeO-PCE, 4-MeO-PCE).

Phencyclidine (PCP), historically known as "angel dust," and its analogues (3-HO-PCP, 3-MeO-PCP, 4-MeO-PCP, 3-HO-PCE, 3-MeO-PCE, 4-MeO-PCE) are dissociative new psychoactive substances with high abuse potential and limited experimental safety data. An integrated in silico workflow (STopTox, admetSAR 3.0, ADMETlab 3.0, ACD/Labs Percepta, Toxtree, ProTox 3.0, OCHEM, TEST, VEGA QSAR) was applied to profile acute toxicity and key hazard domains. Across platforms, rat oral LD50 values for PCP-type analogues were consistently in the ~ 200-630 mg/kg range (Percepta 210-800 mg/kg, TEST 197-628 mg/kg,

Drugs for mental illnesses in ancient greek medicine.

Drugs with psychedelic, hypnotic, narcotic, analgesic, suppressive, euphoric and stimulating effects were used in various ways during the ancient times. We can classify drug use in ancient times into two main categories. On the one hand, ancient physicians prescribed drugs based on their pharmaceutical-therapeutic actions. On the other hand, people not pertaining to medicine and not relating to its accurate implementation often abused drugs to achieve an euphoric state of mind, either for personal gratification or as an indispensable element of religion and mythology. Ancient Greek medical lit

Increased ethanol drinking after repeated chronic ethanol exposure and withdrawal experience in C57BL/6 mice.

BACKGROUND: The development of dependence may have significant motivational consequences regarding continued use and abuse of ethanol. We have developed a mouse model of ethanol dependence and repeated withdrawals that demonstrates sensitization of seizures and other symptoms of withdrawal. It is unclear whether such experience influences ethanol drinking behavior. The present series of experiments were designed to examine whether repeated cycles of chronic ethanol exposure and withdrawal has an impact on subsequent motivation to voluntarily self-administer ethanol. METHODS: With the use of a

Prevalence of Methamphetamine (Mkpurummiri) use in south east Nigeria: a community-based cross-sectional study.

BACKGROUND: Methamphetamine (MA) (Mkpurummiri) is seen as the most common, accessible and new drug of use/abuse in south east Nigeria. Despite the overwhelming evidence of the dire consequences of this drug to the users, there is lack of empirical research to determine its prevalence in this area where use is assumed to be common. This study aimed to determine the prevalence of MA use, characteristics of the users and the control measures. METHODS: A descriptive cross-sectional survey research design was conducted from May through November 2023 among adolescents/adults at the age bracket of 1

Varied Presentations of Pediatric Patients With Positive Cannabinoid Tests.

BACKGROUND: Cannabis (marijuana) is one of the most abused drugs worldwide. The increasing availability of cannabis has been associated with increased emergency department (ED) visits. There is a wide range of presentations documented in the recent literature, and the full scope of symptoms in young children is not fully known. OBJECTIVE: The primary objectives were to gather information regarding the characteristics in the presentation of the children with positive cannabinoid urine drug screen (UDS) results in the ED and to determine if there are certain common presentations with cannabinoi

Violent language in the environment of street children singer-beggars.

BACKGROUND: Street children singer-beggars are differently treated psychologically and socially by many people on the street since they are considered to disturb the environment. They grow up with different perspectives from children of their age and social interactions unsuitable for their growth. Abusive language, repeated acts of violence, and antisocial attitudes have shaped their different personalities and characteristics in accordance with their age. OBJECTIVE: This study was conducted to observe the lives of street children singer-beggars through their understanding of adult communica

Substance use among inmates at the Eldoret prison in Western Kenya.

BACKGROUND: Criminal activity and social problems are recognized as important outcomes of substance use and abuse. Little research has been carried out on substance use among prison inmates in Kenya. General population surveys that have examined drug use usually omit this 'hidden' population which may offer insight into drug related morbidity and invaluable preventive measures. This study is set out to determine the lifetime prevalence and factors associated with substance use, including the most frequently used substances, among inmates at a government prison in Western Kenya. DESIGN: A cros

Patterns of concurrent substance use among adolescent nonmedical ADHD stimulant users.

OBJECTIVES: There are growing concerns about nonmedical use of ADHD stimulants among adolescents; yet, little is known whether there exist heterogeneous subgroups among adolescents with nonmedical ADHD stimulant use according to their concurrent substance use. METHODS: We used latent class analysis (LCA) to examine patterns of past-year problematic substance use (meeting any criteria for abuse or dependence) in a sample of 2203 adolescent participants from the National Surveys on Drug Use and Health 2006-2011 who reported past-year nonmedical use of ADHD stimulants. Tolerance and cross-tolerance to neurocognitive effects of THC and alcohol in heavy cannabis users.

INTRODUCTION: Previous research has shown that heavy cannabis users develop tolerance to the impairing effects of Δ9-tetrahydrocannabinol (THC) on neurocognitive functions. Animal studies suggest that chronic cannabis consumption may also produce cross-tolerance for the impairing effects of alcohol, but supportive data in humans is scarce. PURPOSE: The present study was designed to assess tolerance and cross-tolerance to the neurocognitive effects of THC and alcohol in heavy cannabis users. METHODS: Twenty-one heavy cannabis users participated in a double-blind, placebo-controlled, three-way

An extremely infrequent episode of nasopharyngeal actinomycosis associated with squamous adenocarcinoma occurred in an HIV-infected male patient with a previous diagnosis of AIDS, treated with combined antiretroviral therapy taken with insufficient adherence, such that a satisfactory immune system recovery (as expressed by a CD4 lymphocyte count persistently above 400 cells/mcl), contrasted with a low-level persistence of detectable HIV viraemia, and enlarged genotypic resistance mutations. Interestingly, a number of local and specific risk factors for both infectious and neoplastic disorders

The consumption of khat and other drugs in Somali combatants: a cross-sectional study.

BACKGROUND: For more than a decade, most parts of Somalia have not been under the control of any type of government. This "failure of state" is complete in the central and southern regions and most apparent in Mogadishu, which had been for a long period in the hands of warlords deploying their private militias in a battle for resources. In contrast, the northern part of Somalia has had relatively stable control under regional administrations, which are, however, not internationally recognized. The present study provides information about drug abuse among active security personnel and militia w

Assessment of Abuse Liability and Nicotine Pharmacokinetics of glo Heated Tobacco Products in a Randomized, Crossover Study.

BACKGROUND AND OBJECTIVE: Heated tobacco products (HTPs) are a class of non-combustible, inhaled tobacco products with the potential to reduce the harm associated with cigarette smoking due to reduced cigarette smoke toxicant exposure. Subjective and nicotine pharmacokinetics measures taken over the course of product use provide a framework for abuse liability (AL) assessment of tobacco and nicotine products as well as information on adoption potential for a new tobacco product, which are important aspects for premarket tobacco product authorization by the US Food and Drug Administration.
