[Identification of the most common problems and mistakes in the medical certification of inability to work for disability pension provision based on the expertise carried out by the Out-Patient Clinic of Occupational Diseases, Nofer Institute of Occupational Medicine, Łódź, 2005-2007].

BACKGROUND: The definition of "inability to work" and principles of medical certification of disabilities, determining the entitlement to an ill-health pension provision are regulated by law. However, the term ,,"inability to work" is not precisely defined and the standards of its assessment are not accurately formulated. This refers not only to the term "loosing ability to work to considerable degree", but also to the criteria of occupational qualifications. OBJECTIVES: The aim of this study was to identify the most common problems and mistakes in the medical certification that entitles to d

Clinical forensic medicine and its main fields of activity from the foundation of the German Society of Legal Medicine until today.

The fields mainly covered by clinical forensic medicine are subject to time-related changes which are described on the basis of the German literature of the 20th century. Some fields of forensic sexual medicine (diagnosis of virginity, proof of criminal abortion, potentia coeundi, potentia generandi, potentia concipiendi) have become less important in the daily work of medicolegal institutes, whereas victims of rape and sexual abuse continue to form a major part of the forensic examination material in the German-speaking countries. The evaluation of suspected physical child abuse has grown in

Longitudinal study on all-cause and suicide mortality among individuals with attention deficit hyperactivity disorder.

BACKGROUND: Previous research has linked attention deficit hyperactivity disorder (ADHD) with an increased risk of all-cause mortality, primarily owing to unnatural causes such as accidents and suicides. This increase may be attributable to the co-occurrence of major psychiatric disorders, including schizophrenia (SCZ), bipolar disorder (BD), major depressive disorder (MDD), autism spectrum disorder (ASD), anxiety disorders, substance use disorders (SUDs), and personality disorders (PDs). This study examined the all-cause and specific-cause mortality rates in individuals with ADHD and the infl

Estimating cobalt exposure in respirable dust from cobalt in inhalable dust.

Cobalt is a commonly used element in metal industry. Exposure to workers occurs mainly by inhalation of cobalt-containing dust. For the evaluation of cobalt exposure, risk assessment and investigations on occupational diseases, measurements of cobalt in respirable dust are needed. Up to now, often only data for cobalt in inhalable dust are available, which is due to the earlier classification of the limit value in this fraction. Therefore, a possibility to convert cobalt concentrations mathematically from inhalable into respirable concentrations is desirable. In this study, 639 parallel measur

Choosing Wisely: the lack of validity of ultrasound scans in the investigation of shoulder instability.

AIM: To test the clinical validity and financial implications of the proposed Choosing Wisely statement: "Using ultrasound as a screening test for shoulder instability is inappropriate in people under 30 years of age, unless there is clinical suspicion of a rotator cuff tear." METHOD: A retrospective chart review from a specialist shoulder surgeon's practice over a two-year period recorded 124 patients under the age of 30 referred with shoulder instability. Of these, forty-one had already had ultrasound scans performed prior to specialist review. The scan results and patient files were review

Medically graded reduced capacity to work (RCW) following significant injury as explained by patient related outcome measures (PROMs): A prospective trauma center evaluation.

BACKGROUND: Data is limited how well patient-reported longer-term outcome measures (PROMs) explain medically graded reduced capacity for work (RCW). OBJECTIVE: This prospective study investigates the association of RCW with several PROMs in addition to demographic and injury-related variables in significantly injured of working age. METHODS: Patients (New Injury Severity Score, NISS ≥ 8) were recruited consecutively. RCW, as registered by the largest Swiss accident insurer, was tested against demographic, trauma and treatment characteristics, and patients' contemporaneous two year rating of

Impact of statin on long-term outcome among patients with end-stage renal disease with acute myocardial infarction (AMI): a nationwide case-control study.

BACKGROUND: Use of statin has been associated with reduced risk of cardiovascular diseases events and mortality. However, in patients with end-stage renal disease (ESRD), the protective effects of statin are controversial. To evaluate the impact of chronic statin use on clinical outcomes of patients with acute myocardial infarction (AMI) with ESRD. METHODS: We enrolled 8056 patients with ESRD who were initially diagnosed and admitted for first AMI from Taiwan's National Health Insurance Research Database. Of which, 2134 patients underwent statin therapy. We randomly selected and use age, sex,

Predictors of subsequent injury at work: findings from a prospective cohort of injured workers in New Zealand.

OBJECTIVES: People who have experienced a work-related injury can experience further work injuries over time. This study examines predictors of subsequent work-related injuries over 24 months among a cohort of injured workers. METHODS: Participants were those recruited to the earlier Prospective Outcomes of Injury Study (POIS) who had a work-related injury (the 'sentinel' injury). Data from POIS participant interviews were combined with administrative data from the Accident Compensation Corporation (New Zealand's no-fault universal injury insurer) and hospital discharge data. Modified Poisson

Assessment of medical resource utilization for Taiwanese children hospitalized for intracranial injuries.

BACKGROUND: Compared to adults, children and adolescents are at greater risk for traumatic brain injury (TBI), with increased severity and prolonged recovery when compared to adults. It is a challenge to provide care for those children who are at risk for complications of TBI under health care resource constraints. AIM: To investigate hospitalization among children with intracranial injuries in terms of incidence and factors related to length of stay (LOS) and medical cost. METHODS: Data from the National Health Insurance Research Database from 2007-2009 were used. In total 8632 children age

Management of windshield facial injuries.

Between Jan. 1986 and Dec. 1990, 99 patients with automobile windshield facial injuries were hospitalized in Kaohsiung Medical College hospital. All patients were injured by old type windshields in car accidents. Seventy-three patients (74%) were male and twenty-six (26%) were female. Ages ranged from 6 to 62 years (mean age 31 years). Fifty-seven patients (58%) were car drivers and forty-two (42%) were front seat passengers. All patients had typical multiple U-shaped lacerations on the face. The major injury sites were localized to the upper one third of the face. The wounds were repaired imm

Sickness absence > 14 days following sport-related traumatic brain injuries: a nationwide register-based study in Sweden.

BACKGROUND: Sport-related traumatic brain injuries (SR-TBI) have received increasing concerns regarding potential long-term consequences. For adults in the general population, one of these consequences is inability to resume daily activities, most notably sickness absence (SA) from work. The aim of this study was to investigate how often SR-TBI is followed by a registered period of SA. METHODS: A nationwide register-based study was conducted. Source population: working aged individuals (18-63 years), living in Sweden during 2014-2016. Using the national patient register we included patients w

A road traffic injury surveillance system using combined data sources in Peru.

A national hospital-based nonfatal road traffic injury surveillance system was established at sentinel units across Peru in 2007 under the leadership of the Ministry of Health. Surveillance data are drawn from three different sources (hospital records, police reports, and vehicle insurance reports) and include nonfatal road traffic injuries initially attended at emergency rooms. A single data collection form is used to record information about the injured, event characteristics related to the driver of the vehicle(s), and the vehicle(s). Data are analyzed periodically and disseminated to all s

The relationship of farm residency status to demographic and service characteristics of agricultural injury victims in central Wisconsin.

This study performed a surveillance of a defined population in central Wisconsin during a two-year period that included six months of follow-up. The study included those who worked and lived on farms and those who only worked on farms. The injury rate for farm residents was 3.15 injuries per 100 persons per year. However, 195 of the 510 injuries (38.2%) involved persons who were not farm residents. For nonfarm residents, 88.7 percent of the victims were male, whereas for farm residents 79 percent of the victims were male. More than one half of the farm residents who were injured were the owner

Analysis of the risk and risk factors for injury in people with and without dementia: a 14-year, retrospective, matched cohort study.

BACKGROUND: Most previous studies on dementia and injuries have focused on a particular type of injury, and few studies have investigated overall injury in people with dementia. In this study, we investigated the risk factors and risk of overall injury, including the diagnosis, cause, and intentionality of injury, in people with and without dementia in Taiwan. METHODS: We collected relevant data between 2000 and 2013 from the National Health Insurance Research Database (NHIRD). Overall, 455,630 cases, consisting of 91,126 people with dementia and 364,504 people without dementia, were included

The challenges of managing acute extradural hematoma in a Nigerian neurosurgical center--still a long way to go.

BACKGROUND: Acute extradural hematoma (EDH) is the collection of blood in the potential space between the dura mater and endocranium within 3 days of an incident. It is usually, but not always, a neurosurgical emergency, and early treatment reduces morbidity and mortality, although the outcome is still affected by some other determinants. In Nigeria, the National Health Insurance Scheme does not cover neurosurgical cases, and patients have to pay for all of their treatment regardless of the emergency status. We evaluate epidemiologic patterns, management protocols, and outcome, especially in r

Pneumatosis cystoides intestinalis in a trauma patient presenting with pneumoperitoneum: a case report.

BACKGROUND: Pneumatosis cystoides intestinalis is a rare and usually benign condition in which multiple thin-walled cysts develop in the submucosa or subserosa of the gastrointestinal tract. While usually asymptomatic, severe cases can result in pneumoperitoneum, which can be managed surgically or medically depending on circumstances. We present a case of a patient with pneumatosis cystoides intestinalis, which presented as pneumoperitoneum following trauma. To our knowledge, there are no other published cases in which a trauma patient with pneumoperitoneum was found to have radiologic evidenc

Concentration distribution of the marijuana metabolite Delta9-tetrahydrocannabinol-9-carboxylic acid and the cocaine metabolite benzoylecgonine in the department of defense urine drug-testing program.

Urine drug testing has been employed for punitive purposes by the Department of Defense since December 1981 (Memorandum 62884, Deputy Secretary of Defense Frank C. Carlucci). Federal Workplace Drug Testing Programs were initiated in response to Executive Order 12564 issued on September 15, 1986, that required Drug-Free Federal Workplaces be established. In their respective programs, a positive urine drug test may be referred to a military court martial or to an administrative board. To address safety and insurance requirements, the testing of civilians has expanded beyond Federal Programs to i

The Association of Social and Biological Factors with Clinical Outcomes in Patients with Lung Cancer and DNA Damage Repair Pathway Mutations: A Single-Institution Experience.

Background: Lung cancer remains the leading cause of cancer-related mortality in the United States, characterized by poor overall survival rates (OS), particularly for advanced-stage disease. While smoking is a primary risk factor, other contributors include environmental exposures, genetics, comorbidities, and socioeconomic factors. Methods: We included 232 patients with lung cancer (96% NSCLC). Patients were categorized into DNA damage response (DDR)-mutant (DDRmt, n = 67, 29%) and DDR-wild-type (DDRwt, n = 165, 71%) groups.
