Ultrasound biomicroscopic quantification of the change in anterior chamber angle following laser peripheral iridotomy in early chronic primary angle closure glaucoma.

AIMS: To prospectively evaluate by ultrasound biomicroscopy (UBM) and gonioscopy the anterior chamber angle widening following laser peripheral iridotomy (LPI) in eyes with early chronic primary angle closure glaucoma (CACG). METHODS: A total of 55 eyes of 55 patients with CACG presenting with less than 180 degrees peripheral anterior synechiae (PAS) were enrolled in the study. Angles were assessed by gonioscopy (Shaffer's grading) and UBM, before and 4 weeks after LPI. The angle opening distance at 250 and 500 microm from the scleral spur (AOD 250 and AOD 500) was computed. Results were anal

Use of ultrasound biomicroscopic evaluation in preoperative planning of penetrating keratoplasty.

PURPOSE: To evaluate the use of ultrasound biomicroscopy (UBM) in examining the status of anterior-segment structures in eyes with corneal opacification. METHODS: Evaluation with UBM was performed between 1 and 2 weeks before surgery in 21 patients (21 eyes) with corneal opacities who were scheduled for penetrating keratoplasty. UBM findings were correlated with the findings noted during penetrating keratoplasty. The efficacy of UBM evaluation was calculated in terms of positive and negative predictive values (PPVs and NPVs) for the presence or absence of each finding. RESULTS: Seventeen eye

The capsular complications of cataract extraction.

The author considered the following important points:(1) Anterior capsular synechia to a corneal incision (made by a keratome) after the evacuation of a traumatic cataract. This might be detached early by the use of a blunt-ended knife following a perforation of the cornea with a sharp-pointed knife, much like a tenotome.(2) The involuntary prolapse of capsule with a cataract incision.(a) The danger of this was demonstrated as the cause of glaucoma, especially if it be found necessary to divide opaque capsular membrane after the extraction.(b) The danger of sympathetic ophthalmia.Prolapse migh

Short-term outcomes of selective laser trabeculoplasty in pseudophakic primary angle-closure glaucoma after iridotomy: a retrospective pilot comparison with primary open-angle glaucoma.

BACKGROUND: Selective laser trabeculoplasty (SLT) is a widely accepted treatment for primary open-angle glaucoma (POAG), but its role in primary angle-closure glaucoma (PACG) remains less defined. This study aimed to evaluate the short-term outcomes of SLT in pseudophakic PACG patients who had undergone prior laser peripheral iridotomy (LPI), and to compare outcomes with those in POAG patients. METHODS: This retrospective study included pseudophakic patients diagnosed with PACG after LPI or POAG who underwent SLT. Intraocular pressure (IOP) and topical antiglaucoma medication use were assesse

Long-term effect of YAG laser iridotomy on corneal endothelium in primary angle closure suspects: a 72-month randomised controlled study.

PURPOSES: To evaluate the effect of YAG laser peripheral iridotomy (LPI) on corneal endothelial cell density (ECD) and morphology in primary angle closure suspects (PACS) over 72 months. METHODS: The Zhongshan Angle Closure Prevention Trial is a single-centre randomised controlled trial. Subjects with bilateral PACS received YAG LPI prophylactic treatment in one eye randomly, while the fellow eye served as control. Central corneal ECD and morphology were assessed using non-contact specular microscopy (SP-2000P, Topcon) at baseline, 6, 18, 36, 54 and 72 months postoperatively. Mixed model anal

Long-term follow-up of 42 patients with small ciliary body tumors with ultrasound biomicroscopy.

PURPOSE: To establish the growth behavior of small ciliary body tumors in a relatively large cohort of patients over an extended period. DESIGN: Retrospective, noncomparative case series. METHODS: Ciliary body tumors less than 4 mm in size within the penetration power of ultrasound biomicroscopy (UBM) were included. Tumor height was assessed by ultrasound biomicroscopy. Tumor growth was defined as an increase in height of at least 20% from baseline, as measured on 2 consecutive UBM readings. The data were collected longitudinally, and a statistical analysis was performed. RESULTS: Forty-two

An unusual case of acute angle closure glaucoma following argon laser pan retinal photocoagulation.

Acute angle closure glaucoma (AACG) is a painful, potentially blinding condition with patients frequently presenting within hours of onset with ocular pain, nausea and vomiting. Argon laser pan-retinal photocoagulation (PRP) is one of the commonest ophthalmic outpatient procedures. It is used in the treatment of vascular disorders of the retina; most commonly diabetic retinopathy. AACG following PRP is a recognised, but rarely occurring, complication. Prompt treatment alleviates pain and improves visual prognosis. The authors report a case of AACG following PRP treatment which was initially mi
