Endoscopic exploration of the IVth ventricle.

Regarding to the upcoming techniques in neuroendoscopy the IVth ventricle was examined. First in a series of 30 fresh and fixed anatomical specimens-the vessels injected with LATEX-the fourth ventricle was investigated endoscopically. There are three possibilities to reach the IVth ventricle: coming from the IIIrd ventricle via the aqueductus cerebri, using the basal cisterns through the apertura lateralis Luschkae and coming via the cerebellomedullar cistern through the foramen of Magendi. Using different kinds of endoscopes (rigid, flexible and steerable flexible)-diameter ranging from 5 to

Distal mechanisms in robotic surgical instruments: a structured review of surgical access, miniaturisation constraints and procedure-specific performance trade-offs.

The distal end of a robotic surgical instrument is where access, dexterity, tissue interaction and procedural capability converge. In flexible, endoluminal, transoral, percutaneous, vascular and neuroendoscopic surgery, the clinical value of an instrument depends not only on kinematic complexity but also on miniaturisation, usable lumen, transmission reliability, stiffness, sensing and validation maturity. This article presents a structured narrative review of distal mechanisms for robotic surgical instruments, based on a technical-clinical synthesis of studies identified through IEEE Xplore,

Impact of Orbital Rim Osteotomy on Surgical Freedom in Transorbital Endoscopic Approaches: A Cadaveric Study and Novel Methodology.

PURPOSE: Transorbital neuroendoscopic surgery (TONES) is an emerging minimally invasive skull-base approach. The addition of an orbitotomy may increase the size of the surgical corridor, but this measurement has not previously been quantified. METHODOLOGY: Six fresh-frozen cadaveric specimens (11 orbits) underwent bilateral superior eyelid crease incisions and periorbital dissection to the meningo-orbital band and superior orbital fissure. This was followed by bilateral sphenoidotomy to expose the trigeminal ganglion and temporal dura. A custom surgical probe with an Augmented Reality, Univer

Use of neuroendoscopy in metastatic brain tumours: A systematic review.

Neuroendoscopy allows the performance of biopsy and the resection of metastatic brain lesions using a minimally invasive approach. The present systematic review aimed to survey the existing evidence surrounding the use of neuroendoscopic techniques in the setting of metastatic brain lesions. A systematic search of the available literature was performed using the approach outlined by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) method. The search identified 34 studies that enrolled 150 participants. The average age of the patients was 57 years (range, 37-80 ye

An Older Patient with a Symptomatic Arachnoid Cyst in the Velum Interpositum: Considerations of Functional Neuroanatomy.

We report a patient with an arachnoid cyst in the velum interpositum (VI) and discuss the mechanism of the symptoms based on functional neuroanatomy. A 68-year-old woman presented with difficulty in doing housekeeping and with route-finding disorientation in known locations. Her performance intelligence quotient (PIQ) score was 68, significantly lower than her verbal intelligence quotient (IQ) of 103. Significantly low scores were obtained for the picture arrangement, picture completion, and symbol search tasks (4, 1, and 5, respectively) in the PIQ subtests. Her copies of the interlocking pen

[Endoscopic interventions for gunshot craniocerebral injuries].

UNLABELLED: Full-endoscopic (neuroendoscopic) removal of foreign bodies from the cranial cavity is a relatively new technique with limited information available in the literature. Compared to open surgeries, this minimally invasive approach is associated with less impact on the surrounding brain tissues and fewer complications. The safety of such procedures is enhanced by the use of CT navigation. This article describes the surgical technique of endoscopic removal of metallic fragments in combat-related gunshot cranial injuries, based on five clinical cases, and presents the results of a syste

Hydrocephalus as an Initial Presentation of Myelin Oligodendrocyte Glycoprotein (MOG) Antibody-Associated Disease: A Case Report and Review of Tumor-Mimicking Demyelination.

Myelin oligodendrocyte glycoprotein (MOG) antibody-associated disease (MOGAD) is a demyelinating disease of the central nervous system (CNS) that manifests as optic neuritis, transverse myelitis, acute disseminated encephalomyelitis, and cortical encephalitis. Some patients with MOGAD present with tumor-like brain lesions. However, hydrocephalus as an initial presentation is rare. We present the case of a 23-year-old Japanese man with an acute onset of headache, nausea, and diplopia, who was initially suspected of having a germinoma but was later diagnosed with MOGAD. Brain magnetic resonance

Endoscopic transorbital extradural anterior clinoidectomy: A stepwise surgical technique and case series study [SevEN-013].

BACKGROUND: Anterior clinoidectomy is an important procedure for approaching the central skull base lesions. However, anterior clinoidectomy through the endoscopic transorbital approach (ETOA) still has limitations due to technical difficulties and the structural complexity of the anterior clinoid process (ACP). Therefore, the authors designed a stepwise surgical technique of extradural anterior clinoidectomy through the ETOA. The purpose of this study was to evaluate the feasibility of this technique. METHODS: Anatomical dissections were performed in 6 cadaveric specimens using a neuroendosc

Endoscope-assisted resection of brainstem cavernous malformations.

Targeted surgical precision and minimally invasive techniques are of utmost importance for resectioning cavernous malformations involving the brainstem region. Minimisation of the surgical corridor is desirable but should not compromise the extent of resection. This study provides detailed information on the role of endoscopy in this challenging surgical task. A retrospective analysis of medical documentation, radiologic studies and detailed intraoperative video documentation was performed for all consecutive patients who underwent surgical resection of brainstem cavernous malformations betwee

Endoscopic Evacuation of Massive Intraventricular Hemorrhages Reduces Shunt Dependency: A Meta-Analysis.

Intraventricular hemorrhage (IVH) is characterized by severe prognosis. The amount of intraventricular blood is the most important, disease-specific, prognostic factor, as acute complications are strictly dependent on clot formation. Although external ventricular drain (EVD) placement is the standard treatment, in the past 15 years neuroendoscopic (NE) evacuation of IVH has been advocated, but available comparative data are limited. A systematic review of the literature was performed according to Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Included a

Endoscopic Third Ventriculostomy and Endoscopic Intracranial Cyst Fenestration in an Outpatient Ambulatory Surgery Center Yields Reduced Cost But Equal Efficacy and Safety Compared with Surgery in the Hospital.

BACKGROUND: A transition is underway in neurosurgery to perform relatively safe surgeries outpatient, often at ambulatory surgery centers (ASC). We sought to evaluate whether simple intracranial endoscopic procedures such as third ventriculostomy and cyst fenestration can be safely and effectively performed at an ASC, while comparing costs with the hospital. METHODS: A retrospective chart review was performed for patients who underwent elective intracranial neuroendoscopic (NE) intervention at either a quaternary hospital or an affiliated ASC between August 2014 and September 2017. Groups wer

Endoscopic aqueductoplasty and stenting in the treatment of isolated fourth ventricle in children: 20-year institutional experience.

BACKGROUND: Endoscopic aqueductoplasty with aqueductal stenting is an effective surgical procedure for the treatment of isolated fourth ventricle (IFV). Due to the rarity of the underlying pathology, it can be considered a rare procedure that can be performed with different surgical techniques and approaches. OBJECTIVES: To assess long-term functioning of permanent aqueductal stents implanted in children affected by hydrocephalus and IFV and to describe some variations of the same procedure. METHODS: We reviewed retrospectively all the patients presenting at our institution in the years 1999

Intra-operative cerebrospinal fluid sampling versus post-operative lumbar puncture for detection of leptomeningeal disease in malignant paediatric brain tumours.

INTRODUCTION: Leptomeningeal disease is a feared sequelae of malignant paediatric brain tumours. Current methods for its detection is the combined use of cranio-spinal MRI, and CSF cytology from a post-operative lumbar puncture. In this study, the authors hypothesize that CSF taken at the start of surgery, either from an external ventricular drain or neuroendoscope will have equal sensitivity for positive tumour cells, in comparison to lumbar puncture. Secondary hypotheses include positive correlation between CSF cytology and MRI findings of LMD. From a clinical perspective, the key aim of the

Endoscopic biopsy of intra- and paraventricular brain tumors.

INTRODUCTION: Selection of the optimal treatment method of intra- and paraventricular tumors often requires histopathological verification that can be obtained by endoscopic biopsy. AIM: To discuss the usefulness of the method in their own experience. MATERIAL AND METHODS: The results of 32 biopsies carried out during a 15-year period were reviewed retrospectively. All tumors were located supratentorially, 25 of them were intraventricular and 7 paraventricular. In 18 patients the tumor was accompanied by internal hydrocephalus. If the ventricular system was narrow, the biopsy was supported b

Minimally Invasive Transpalpebral Endoscopic-Assisted Amygdalohippocampectomy.

BACKGROUND: Although anterior temporal lobectomy may be a definitive surgical treatment for epileptic patients with mesial temporal sclerosis, it often results in verbal, visual, and cognitive dysfunction. Studies have consistently reported the advantages of selective procedures compared with a standard anterior temporal lobectomy, mainly in terms of neuropsychological outcomes. OBJECTIVE: To describe a new technique to perform a selective amygdalohippocampectomy (SAH) through a transpalpebral approach with endoscopic assistance. METHODS: A mini fronto-orbitozygomatic craniotomy through an e
