We studied the factors related to delayed gastric emptying after operation for obstructing peptic ulcer disease. The records of 76 consecutive patients who underwent operation for obstructing peptic ulcer were examined retrospectively. Delayed gastric emptying (unable to tolerate solid food for at least 2 weeks after operation) occurred in 11 of 76 patients (14.5%). It was not related to the type of operation performed, including procedures that employed truncal vagotomy. We retrospectively reviewed the records of 60 patients who had been referred for gastrointestinal manometry because of stasis after gastric surgery. Nausea, vomiting, bloating, abdominal pain, and weight loss were the most common symptoms. The risk of developing gastric cancer has been investigated in a case-control study and in a prospective investigation. In the case-control study, 1495 cases of gastric cancer were identified in five city hospitals and matched with autopsy controls from the same hospitals. An increased incidence of cholelithiasis has been widely reported after gastric surgery. In the early phase of patient selection, previous gastric surgery has been considered a relative contraindication to laparoscopic cholecystectomy. The Mallory-Weiss syndrome is often associated with acute ulceration of the gastric and duodenal mucosa, which gives grounds for a supposition of the existence of common mechanisms of pathogenesis of these diseases. Because of bad results of the operative treatment of patients with such combinations, the method of choice should be conservative therapy and curative endoscopy. A pathogenetically substantiated operative intervention--suturing the source of bleeding in combination with vagotomy and pyloroplasty is preferable when operation is inevitable.
