[Possibilities and results of shortened hospital treatment in pediatric surgery (author's transl)].

2582 surgical operations on children - principally abdominal hernias and phimoses - are reported. Inpatient treatment required a stay of 3.2 days, 430 children were treated as outpatients. The 11 pediatricians working in the afflux area unanimously judged the shortened stay in hospital positive. Of the parents interviewed, only 1.3% did not agree with the short hospitalization and 4.8% disagreed with the outpatient therapy. The well-known psychic disturbances of small children by admission to hospital confirmed. The economic importance of a stay which can be reduced by 10 days compared with th

"Incidentaloma" as Occasional Finding During Laparoscopic Surgery: How Laparoscopy Makes The Difference!

BACKGROUND: In the scientific nomenclature, "incidentaloma" is the medical term to describe incidentally found asymptomatic tumors. We adapted the term "incidentaloma" to describe the finding of additional pathology during laparoscopic surgery conducted for a different indication. This study aimed to highlight the role of laparoscopy in identifying and addressing hidden concurrent conditions, thereby preventing the need for further surgical interventions. METHODS: Five hundred ninety-five patients undergoing laparoscopic procedures from January 2018 to January 2023 were enrolled. Patients wer

Intestinal endometriosis: A rare cause of acute care surgery.

BACKGROUND: Intestinal endometriosis is a rare condition that can cause gastrointestinal symptoms such as abdominal pain, constipation, and diarrhea. It occurs in approximately 5%-15% of women with endometriosis. Although it rarely leads to obstruction or perforation, there is no clear consensus on its optimal management. Hormonal therapy is considered the first-line treatment for endometriosis; however, the best approach for intestinal involvement remains controversial. This study aims to contribute to the literature by evaluating patients with intestinal endometriosis who underwent surgery f

The Great Mimicker: Extragonadal Endometriosis Presenting as an Appendiceal Mass and Acute Appendicitis.

A 45-year-old female presented to the Emergency Department with acute right lower quadrant (RLQ) pain. Her past medical history was significant for heavy menstrual bleeding and moderate dysmenorrhea, though a diagnosis of endometriosis had never been established previously. Initial work-up revealed anemia and leucocytosis. A CT of the abdomen and pelvis showed an enlarged structure in RLQ along the medial border of the cecum, suspicious for acute appendicitis with a retained rupture. CA19-9, CA-125, and CEA were sent, of which only CA-125 was elevated at 92.7 U/mL (reference range: 0-35 U/mL).

The Clinicopathological Significance of Tumor Cell Subtyping in Appendiceal Neuroendocrine Tumors: A Series of 135 Tumors.

Appendiceal neuroendocrine tumors (NETs) are common and often are identified as incidental lesions at the time of appendectomy. The guidelines for management are based on tumor size, degree of invasion, and the Ki67 proliferation index. Most small bowel NETs are composed of serotonin-producing EC-cells, but there are multiple other neuroendocrine cell types. In the rectum, there are L-cell tumors that express peptide YY (PYY), glucagon-like peptides (GLPs), and pancreatic polypeptide (PP); they are thought to have a better prognosis than serotonin-producing tumors. We investigated whether the

[Robotic Left Hepatectomy Using the Glissonean Pedicle Approach for the Treatment of Caroli's Syndrome].

BACKGROUND: Caroli's syndrome is a rare disease characterised by non-obstructive dilation of intrahepatic bile ducts, hepatic fibrosis, and an increased risk of developing cholangiocarcinoma. Minimally invasive liver resection has recently been increasingly adopted for the treatment of patients with localised Caroli's syndrome. However, robot-assisted liver resection for the treatment of Caroli's syndrome has not been published. MATERIALS AND METHODS: We report a case of a 72-year-old Asian female who was referred to our hospital with multifocal cystic dilation of liver segments II, III, and

Familial hypocalciuric hypercalcaemia type 1 caused by a novel heterozygous missense variant in the CaSR gene, p(His41Arg): two case reports.

BACKGROUND: Familial hypocalciuric hypercalcaemia (FHH) is a rare, inherited disorder of extracellular calcium sensing. It is clinically characterised by mild to moderate parathyroid hormone dependent hypercalcaemia, an autosomal dominant pattern of inheritance, and a normal to reduced urinary calcium excretion in spite of high serum calcium. CASE PRESENTATION: We report two cases of FHH in a family caused by a novel pathogenic missense variant in the CaSR gene, p. His41Arg. Case 1, describes a 17 year old female with no significant past medical history, admitted with acute appendicitis requi

Epidemiology of surgery in a protracted humanitarian setting: a 20-year retrospective study of Nyarugusu Refugee Camp, Kigoma, Western Tanzania.

BACKGROUND: There are 80 million forcibly displaced persons worldwide, 26.3 million of whom are refugees. Many refugees live in camps and have complex health needs, including a high burden of non-communicable disease. It is estimated that 3 million procedures are needed for refugees worldwide, yet very few studies exist on surgery in refugee camps, particularly protracted refugee settings. This study utilizes a 20-year dataset, the longest dataset of surgery in a refugee setting to be published to date, to assess surgical output in a setting of protracted displacement. METHODS: A retrospectiv

[175 years of anesthesia and narcosis-Towards a "human right to unconsciousness"].

The Ether Day, a key moment in the history of mankind, commemorates its 175th anniversary on 16 October 2021. On that day the dentist William T. G. Morton successfully gave the first public ether anesthesia in Boston. From then on it was possible to save people from pain with justifiable risk and at the same time to protect them from psychological damage by inducing unconsciousness. The German philosopher Peter Sloterdijk, one of the most renowned and effective philosophers of our times, deduced that from then on humans, to some extent, had a right to unconsciousness when in psychophysical dis

Clinical and genetic analysis of 2 rare cases of Wiskott-Aldrich syndrome from Chinese minorities: Two case reports.

RATIONALE: Wiskott-Aldrich syndrome (WAS) is a rare X-linked recessive disease characterized by thrombocytopenia, small platelets, eczema, immunodeficiency, and an increased risk of autoimmunity and malignancies. X-linked thrombocytopenia (XLT), the milder phenotype of WAS, is always limited to thrombocytopenia with absent or slight infections and eczema. Here, we illustrated the clinical and molecular characteristics of 2 unrelated patients with WAS from Chinese minorities. PATIENT CONCERNS: Patient 1, a 13-day-old male newborn of the Chinese Lahu minority, showed a classic WAS phenotype, in

Management of epithelial cancer of the ovary, fallopian tube, and primary peritoneum. Short text of the French Clinical Practice Guidelines issued by FRANCOGYN, CNGOF, SFOG, and GINECO-ARCAGY, and endorsed by INCa.

An MRI is recommended for an ovarian mass that is indeterminate on ultrasound. The ROMA score (combining CA125 and HE4) can also be calculated (Grade A). In presumed early-stage ovarian or tubal cancers, the following procedures should be performed: an omentectomy (at a minimum, infracolic), an appendectomy, multiple peritoneal biopsies, peritoneal cytology (grade C), and pelvic and para-aortic lymphadenectomies (Grade B) for all histologic types, except the expansile mucinous subtypes, for which lymphadenectomies can be omitted (grade C). Minimally invasive surgery is recommended for early-st

Delayed intestinal stricture following non-resectional treatment for non-occlusive mesenteric ischemia associated with hepatic portal venous gas: a case report.

BACKGROUND: Hepatic portal venous gas associated with non-occlusive mesenteric ischemia is indicative of a serious pathology that leads to bowel necrosis and it has a high mortality rate. Although non-occlusive mesenteric ischemia is acknowledged as a condition that requires early surgical treatment, it has been reported that bowel necrosis and surgical resection of the gangrenous lesion may be avoided if the condition is identified quickly and the cause is detected at an early phase. However, no reports or guidelines have been published that describe the management of patients in whom bowel n

Total Leukocyte Count Depicting the Degree of Inflammation in Acute Appendicitis.

Introduction Acute appendicitis (AA) is the most common cause of acute abdomen in young adults. The diagnosis is usually made on various clinical findings. However, a missed case of acute appendicitis is a catastrophe as it later presents with life-threatening complications and results in medicolegal issues. Raised total leukocyte count (TLC) is a frequent finding in patients with acute appendicitis. As a convention, a normal leukocyte count usually rules out the differential diagnosis of acute appendicitis. Recent studies claimed that a substantial proportion of patients with normal TLC also

Acute Appendicitis at Stem Cell Infusion During Autologous Transplantation for HIV-Associated Diffuse Large B-cell Lymphoma: A Case Report.

Acute appendicitis is an exceptionally rare complication of autologous hematopoietic stem cell transplantation (HSCT) and represents a diagnostic challenge, since its presentation overlaps with conditioning-related gastrointestinal toxicity and neutropenic enterocolitis. Published data are limited to isolated case reports, and its occurrence during autologous stem cell transplantation (ASCT) for human immunodeficiency virus (HIV)-associated lymphoma has not previously been described. We report the case of a 39-year-old man with HIV-associated diffuse large B-cell lymphoma (DLBCL), non-germinal

Phenotypic Heterogeneity in Crohn's Disease-Associated Intestinal Strictures: An Exploratory Retrospective Cohort Study.

Background: Crohn's disease-associated intestinal strictures represent a major source of morbidity and frequently require endoscopic or surgical intervention. However, patients with stricturing Crohn's disease demonstrate substantial clinical heterogeneity regarding disease localization, penetrating complications, systemic manifestations, metabolic alterations, and treatment exposure. This study aimed to explore phenotypic heterogeneity within patients with Crohn's disease-associated intestinal strictures. Methods: In this retrospective exploratory cohort study, 96 patients with Crohn's diseas

Heavy metal - habitual licking of a lead tape as rare cause of chronic lead poisoning: a case report.

BACKGROUND: Lead poisoning remains a major environmental health risk for children, as no safe blood lead concentration has been identified and even low-level exposure can cause neurocognitive, behavioral, cardiovascular and renal impairments. Although blood lead levels have declined substantially in high-income countries due to regulatory measures such as the elimination of leaded gasoline and stricter environmental controls, lead exposure continues to pose a significant global public health burden, particularly in low- and middle-income countries. Moreover, uncommon exposure pathways still oc
