The complexity of cerebral blood flow regulation: the interaction of posture and vasomotor reactivity.

Arterial carbon dioxide ([Formula: see text]) and posture influence the middle (MCAv) and posterior (PCAv) cerebral artery blood velocities, but there is paucity of data about their interaction and need for an integrated model of their effects, including dynamic cerebral autoregulation (dCA). In 22 participants (11 males, age 30.2 ± 14.3 yr), blood pressure (BP, Finometer), dominant MCAv and nondominant PCAv (transcranial Doppler ultrasound), end-tidal CO2 (EtCO2, capnography), and heart rate (HR, ECG) were recorded continuously. Two recordings (R) were taken when the participant was supine (R

[New clinico-therapeutic directions in upper airway pathology].

The main issues regarding the pathology of the primary airways in children, such as persistent rhinitis, recurrent disease of the pharynx and laryngitis, have been re-examined and updated on the basis of the latest findings regarding immunology and respiratory physiopathology as well as recent technological progress in the field of diagnosis. As far as rhinitis is concerned, recently demonstrations that nasal mucosa, like bronchial mucosa, responds to different kinds of stimuli with the same type of inflammation, have led to a new nosographic viewpoint: apart from classic allergic rhinitis, mu

Pharmacologic Sedation-Related Respiratory Complications in Pediatric Dentistry: A Decade-Long Scoping Review.

CONTEXT: Procedural sedation is widely used in pediatric dentistry to facilitate dental care in uncooperative children. From a pharmacological perspective, sedative agents differ substantially in their mechanisms of action, routes of administration, and safety profiles. Among the reported adverse effects, respiratory complications remain the most clinically significant and potentially life-threatening. Despite extensive clinical use, a comprehensive synthesis of respiratory adverse events (RAEs) associated with commonly used sedative drugs in pediatric dental settings is lacking. This scoping

Shared autonomic phenotype of long COVID and myalgic encephalomyelitis/chronic fatigue syndrome.

INTRODUCTION: Long COVID and myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) are relatively common and disabling multisystem disorders that share overlapping features, including post-infectious onset and similar clinical manifestations such as brain fog, fatigue, muscle pain, and dysautonomia with orthostatic intolerance. These similarities suggest that Long COVID and ME/CFS may share common pathophysiological mechanisms, though the underlying mechanisms remain poorly understood, partly due to the difficulty in quantifying many of the symptoms. MATERIALS AND METHODS: This retrospe

Emergency medical services protocols for traumatic brain injury in the United States: A call for standardization.

BACKGROUND: Traumatic brain injury (TBI) with acute elevation in intracranial pressure (ICP) is a neurologic emergency associated with significant morbidity and mortality. In addition to indicated trauma resuscitation, emergency department (ED) management includes empiric administration of hyperosmolar agents, rapid diagnostic imaging, anticoagulation reversal, and early neurosurgical consultation. Despite optimization of in-hospital care, patient outcomes may be worsened by variation in prehospital management. In this study, we evaluate geographic variation between emergency medical services

Avoid the Goose! Paramedic Identification of Esophageal Intubation by Ultrasound.

OBJECTIVES: Rapid identification of esophageal intubations is critical to avoid patient morbidity and mortality. Continuous waveform capnography remains the gold standard for endotracheal tube (ETT) confirmation, but it has limitations. Point-of-care ultrasound (POCUS) may be a useful alternative for confirming ETT placement. The objective of this study was to determine the accuracy of paramedic-performed POCUS identification of esophageal intubations with and without ETT manipulation. METHODS: A prospective, observational study using a cadaver model was conducted. Local paramedics were recru

An appropriate inspiratory flow pattern can enhance CO2 exchange, facilitating protective ventilation of healthy lungs.

BACKGROUND: In acute lung injury, CO2 exchange is enhanced by prolonging the volume-weighted mean time for fresh gas to mix with resident alveolar gas, denoted mean distribution time (MDT), and by increasing the flow rate immediately before inspiratory flow interruption, end-inspiratory flow (EIF). The objective was to study these effects in human subjects without lung disease and to analyse the results with respect to lung-protective ventilation of healthy lungs. METHODS: During preparation for intracranial surgery, the lungs of eight subjects were ventilated with a computer-controlled venti

Tissue Blood Flow During Remifentanil Infusion With Carbon Dioxide Loading.

The aim of this study was to investigate the effect of changes in end-tidal carbon dioxide tension (ETCO2) during remifentanil (Remi) infusion on oral tissue blood flow in rabbits. Eight male tracheotomized Japan White rabbits were anesthetized with sevoflurane under mechanical ventilation. The infusion rate of Remi was 0.4 μg/kg/min. Carbon dioxide was added to the inspired gas to change the inspired CO2 tension to prevent changes in the ventilating condition. Observed variables were systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial pressure (MAP), heart rate (HR),

Unsteadiness of breathing in patients with hyperventilation syndrome and anxiety disorders.

The breathing pattern of 399 patients with hyperventilation syndrome (HVS) and/or with anxiety disorders and that of 347 normal controls was investigated during a 5 min period of quiet breathing and after a 3 min period of voluntary hyperventilation. The diagnosis of HVS was based on the presence of several suggestive complaints occurring in the context of stress, and reproduced by voluntary hyperventilation. Organic diseases as a cause of the symptoms were excluded. The anxiety disorders were diagnosed by means of an abbreviated version of the Anxiety Disorders Interview Schedule (ADIS). Ther

An Official American Thoracic Society/European Respiratory Society Workshop Report: Evaluation of Respiratory Mechanics and Function in the Pediatric and Neonatal Intensive Care Units.

Ready access to physiologic measures, including respiratory mechanics, lung volumes, and ventilation/perfusion inhomogeneity, could optimize the clinical management of the critically ill pediatric or neonatal patient and minimize lung injury. There are many techniques for measuring respiratory function in infants and children but very limited information on the technical ease and applicability of these tests in the pediatric and neonatal intensive care unit (PICU, NICU) environments. This report summarizes the proceedings of a 2011 American Thoracic Society Workshop critically reviewing techni

Rate of progression and predictive factors for pulmonary outcome in children and adults with Pompe disease.

Respiratory insufficiency is a serious threat to patients with Pompe disease, a neuromuscular disorder caused by lysosomal acid alpha-glucosidase deficiency. Innovative therapeutic options which may stabilize pulmonary function have recently become available. We therefore determined proportion and severity of pulmonary involvement in patients with Pompe disease, the rate of progression of pulmonary dysfunction, and predictive factors for poor respiratory outcome. In a single-center, prospective, cohort study, we measured vital capacity (VC) in sitting and supine positions, as well as maximum i

Cerebral vasoconstriction in vasovagal syncope: any link with symptoms? A transcranial Doppler study.

BACKGROUND: Cerebral vasoconstriction has been described previously in vasovagal syncope (VVS). This phenomenon appears paradoxical in view of the well-known decrease of systemic vascular resistances taking places during VVS. We aimed to assess (1) whether cerebral vasoconstriction in VVS is an independent paradoxical phenomenon and (2) whether cerebral vasoconstriction has any link with symptoms and/or VVS onsets. METHODS AND RESULTS: Seven young patients with recurrent VVS participated in the study. Each patient underwent monitoring of heart rate, blood pressure, cerebral blood flow velocit

Recommended intravenous infusion rate of sufentanil during induction of general anesthesia to prevent coughing: a randomized, single-blind, prospective clinical trials.

BACKGROUND: Sufentanil-induced cough (SIC) is a prevalent and potentially hazardous reflex during anesthesia induction. While slow injection is known to reduce SIC, the precise dose-response relationship between infusion rate and cough suppression remains poorly defined. This study aimed to identify the optimal intravenous infusion rate of sufentanil to reduce SCI and to estimate the 50% and 95% cough inhibition thresholds (IS₅₀ and IS₉₅) using probit regression analysis. METHODS: A total of 70 adult patients (ASA I-II) scheduled for general anesthesia with tracheal intubation were prospectiv

Impact of positive end-expiratory pressure and recruitment maneuver on healthy lungs in dogs assessed by functional and anatomical monitoring methods.

INTRODUCTION: Atelectasis is a common occurrence during anesthesia, and positive end-expiratory pressure (PEEP) ventilation and recruitment maneuvers (RM) can be used to mitigate this. However, both techniques may be associated with side effects in healthy lungs, and close monitoring is indicated. This study aimed to evaluate the effects of PEEP and RM in healthy dogs and to compare functional lung monitoring methods by electrical impedance tomography (EIT), volumetric capnography (VCap), and blood gas analysis with the gold-standard anatomical monitoring provided by computed tomography (CT).

The upper frequency limit of dynamic cerebral autoregulation.

KEY POINTS: Dynamic cerebral autoregulation (CA) is expressed by the temporal pattern of cerebral blood flow (CBF) recovery following a sudden change in arterial blood pressure (BP). Transfer function analysis of BP as input and CBF velocity as output can express dynamic CA through its amplitude (or gain) and phase frequency responses. The upper frequency limit (FupLim ) at which dynamic CA can operate is of considerable physiological interest and can also provide additional information about worsening CA due to disease processes. In healthy subjects FupLim was strongly dependent on arterial P

Efficacy of Intravenous Cobinamide Versus Hydroxocobalamin or Saline for Treatment of Severe Hydrogen Sulfide Toxicity in a Swine (Sus scrofa) Model.

BACKGROUND: Hydrogen sulfide (H2 S) is a potentially deadly gas that naturally occurs in petroleum and natural gas. The Occupational Health and Safety Administration cites H2 S as a leading cause of workplace gas inhalation deaths. Mass casualties of H2 S toxicity may be caused by exposure from industrial accidents or release from oil field sites. H2 S is also an attractive terrorism tool because of its high toxicity and ease with which it can be produced. Several potential antidotes have been proposed for hydrogen sulfide poisoning but none have been completely successful. OBJECTIVE: The obj
