PURPOSE: To evaluate the outcomes of the first 30 cataract surgeries performed with a new disposable, injector-free, small-pupil expansion device.
METHODS: This consecutive case series included 30 eyes from 29 patients who underwent cataract surgery using a new disposable small-pupil expansion device called the Canabrava Ring (AJL Ophthalmic S.A, Spain). It is the first iris expansion ring produced with indents that do not align with each other in the superior and inferior regions, resulting in a small vertical length (0.4 mm) that minimizes the risk of endothelial contact. All eyes had poorl

INTRODUCTION: A subpopulation of children with high myopia and neurobehavioral disorders is noncompliant with spectacle wear and ill-suited to correction using contact lenses. We report the results of refractive surgery in a series of these children treated using lensectomy alone (clear lens extraction), or lensectomy with simultaneous implantation of an intraocular lens (refractive lens exchange).
METHODS: Clinical course and outcome data were collated prospectively in a group of 13 children (mean age 10.4 years, range 1 to 18 years) with neurobehavioral disorders exacerbated by chronic nonc

Posterior capsule opacification (PCO) is a common post-cataract surgery complication treated with Nd:YAG laser posterior capsulotomy, which may cause intraocular pressure (IOP) spikes and threaten vision. Brimonidine and apraclonidine are widely used to prevent such elevations. This prospective, double-masked, randomized clinical trial evaluated the efficacy of topical dexmedetomidine, a novel ophthalmic drop, in preventing IOP rise after Nd:YAG laser treatment. A total of 111 eyes from 89 pseudophakic patients were randomized to receive dexmedetomidine 0.008% or brimonidine 0.2% one h before

Posterior capsule opacification (PCO), the most common complication after cataract surgery, is caused by the proliferation, migration and epithelial-mesenchymal transition (EMT) of residual lens epithelial cells in the capsule bag. Although the surface modification and drug loading of intraocular lens (IOLs) have been effective in preventing PCO to some extent, the intraocular safety of anti-proliferative drug application is still a major limitation in clinical application. In this study, we used non-viral gene delivery systems in combination with layer-by-layer (LBL) self-assembly technology,

BACKGROUND: Posterior capsular opacification (PCO) is a common complication of cataract surgery and is strongly associated with the epithelial-mesenchymal transition (EMT) of lens epithelial cells (LECs). Integrin β3 (ITGB3) has been implicated in various pathological processes; however, its role in PCO remains insufficiently understood.
OBJECTIVES: To investigate the regulatory role of ITGB3 in PCO progression and determine whether ITGB3 modulates transforming growth factor-β2 (TGF-β2)-induced migration and EMT in human lens epithelial cells.
MATERIAL AND METHODS: Human lens epithelial HLE-

PURPOSE: Inflammation, oxidative stress, and epithelial-mesenchymal transition (EMT) play crucial roles in forming posterior capsular opacification (PCO), particularly in fibrotic PCO. Here we investigated the protective effects of mefunidone (MFD), a novel compound with potent antifibrotic properties, which could be useful in preventing PCO.
METHODS: We utilized an extracapsular lens extraction (ECLE) surgery in mice to simulate the development of PCO in vivo. Treatment was performed immediately postsurgery through the intracameral injection of MFD solution. Expression levels of EMT and infl

Hematic pseudocysts are fibrous, nonepithelial lined capsules containing blood byproducts that typically present remotely following orbital fracture hardware implantation. Trauma, implant migration, and tissue erosion are believed to cause hemorrhage to pool within the capsular space. Risk factors include inadequate posterior fracture reduction and use of nonporous material which prevents fibrovascular ingrowth and stabilization. Mass effect from these lesions may cause patients to present with pain, lid swelling, hyperglobus, proptosis, lid retraction, motility restriction, or blurry vision.

Reduced quality of life and financial burden due to visual impairment and blindness begin to increase dramatically when individuals reach the age of 40. The major causes of age-related vision loss can be traced to changes to the structure and function of the lens, one of the tissues responsible for focusing light on the retina. Age-related nuclear cataracts, which are caused by aggregation and condensation of proteins, diminish vision because they impede the transmission and focusing of light on the retina. In addition to the slow-developing age-related form, cataracts often develop rapidly as

Tens of millions of cataract surgeries are done every year and the number is increasing heavily. Posterior capsule opacification is the major postoperative complication with an incidence of 10 to 50% within 5 years, depending on the age of the patient. We present a novel approach for secondary cataract treatment in a noninvasive manner. Photochemically triggered drug release from a polymer enables repeated drug applications for cataract treatment years after implantation of the intraocular lens, just when needed. However, light in the visible spectral range must pass through the lens but must

Cataracts, a prevalent ocular disorder, represent the leading cause of global blindness. Evidence indicates that oxidative stress-induced lens epithelial cells (LECs) apoptosis constitutes a key pathogenic mechanism in age-related cataract (ARC). Although the transcription factor EGR2 has been recognized as both a novel senescence regulator and a critical mediator in ophthalmic diseases including uveitis and diabetic retinopathy, its precise role in cataract remains undefined. In this study, EGR2 was identified via transcriptome sequencing as a potential core regulator of cataract development.

Epithelial-mesenchymal transition (EMT) contributes to fibrotic disease in multiple organs and underlies post-surgical complications such as posterior capsule opacification (PCO), the leading cause of vision loss following cataract surgery. The molecular mechanisms that preserve epithelial integrity and restrain EMT remain poorly defined. Connexins are conventionally regarded as gap junction proteins mediating intercellular communication, but here we identify connexin 50 (Cx50) as a previously unrecognized key regulator of EMT. Using in vitro, ex vivo, and in vivo chick and mouse models, we sh

Posterior capsule opacification (PCO) is a common delayed complication of cataract surgery, particularly in younger patients and those receiving multifocal intraocular lenses (IOLs). Intraoperative posterior capsulorhexis has been proposed as a preventive technique, but its use in adult cataracts is less documented. A 27-year-old patient presented with bilateral congenital posterior subcapsular cataracts, high myopia, and astigmatism. Comprehensive preoperative examination, including visual function and retinal imaging, was performed. The patient underwent phacoemulsification with toric multif

Diclofenac (DCF) is a non-steroidal anti-inflammatory drug (NSAID) with analgesic, anti-inflammatory and antipyretic effects, commonly used to treat painful and chronic inflammatory conditions linked to angiogenesis. It works by inhibiting cyclooxygenase (COX) enzymes and leukocyte migration, reducing prostaglandin synthesis and inflammation. Topically, DCF is used to manage ocular inflammation such as uveitis and keratitis, prevent cystoid macular edema post-surgery, and maintain mydriasis during operations. Clinical studies show it provides anti-inflammatory benefits comparable to dexamethas

PURPOSE: Continuous curvilinear capsulorhexis (CCC) quality affects posterior capsule opacification and visual outcomes after cataract surgery. We developed a novel metric, the Capsulotomy Proficiency Index (CPI), to objectively evaluate the CCC manipulation during cataract surgery.
METHODS: We developed a self-design code using MATLAB software to outline the motion trail of the capsulorhexis forceps, the surgical field motion amplitude, the CCC profile (circularity, deviation, decentration), the number of grasps and the total and effective CCC operating time, etc. Considering the weight of e
