Structural stressors, neurocognition in reward-related decision making and substance use risk in Puerto Rican early adolescents at two sites: study design.

BACKGROUND: Structural stressors (SS) or mutually reinforcing systems that prevent groups of individuals from accessing opportunities and resources, can impact one's health. Indicators of SS include area-level group differences in income, education and exposure to violence. SS are fundamental causes of health disparities and can be indexed by area-level measures. Area-level SS may influence substance use risk in adolescents and future substance use disorders (SUDs). SUDs often start during adolescence, a critical risk period and an ideal time for prevention and intervention efforts. Since SUD

Economic Impact of PromarkerD In Vitro Diagnostic Testing on the Management of Chronic Kidney Disease in Type 2 Diabetes: A Budget Impact Analysis.

OBJECTIVES: This study evaluated the clinical and economic impact of integrating Promarker®D, a validated biomarker-based blood test that predicts kidney function decline up to 4 years before clinical evidence of chronic kidney disease (CKD), into routine management of patients with type 2 diabetes (T2D) from a US healthcare payer perspective. METHODS: A 10-year budget impact model simulated outcomes and costs of one million adults with T2D receiving standard of care (SoC) alone or SoC plus PromarkerD testing providing risk-stratified management. Model inputs were derived from published liter

Cost-utility analysis of ropeginterferon alfa-2b to manage low-risk patients with polycythemia vera as compared to phlebotomy only in the Austrian healthcare system.

Treatment of polycythemia vera (PV) aims to maintain hematocrit on target to reduce risk of thrombotic complications, while preventing disease progression to myelofibrosis (MF) and acute myeloid leukemia (AML). This analysis evaluated cost-effectiveness of adding ropeginterferon alfa-2b (ropegIFNα) to phlebotomy in patients with low-risk PV (those younger than 60 years without prior thrombosis), compared to phlebotomy alone. We combined a 12-month decision tree with a semi-Markov cohort model comparing ropegIFNα to the standard treatment from the Austrian healthcare system perspective over 30

Cost-Effectiveness Analysis of Patiromer in Combination with Renin-Angiotensin-Aldosterone System Inhibitors for Chronic Kidney Disease in Sweden.

OBJECTIVES: Patients with chronic kidney disease (CKD) are commonly treated with renin-angiotensin-aldosterone system inhibitors (RAASi) in order to delay progression of renal disease. However, research has shown that RAASi in CKD patients increases hyperkalaemia (HK) prevalence, which leads to RAASi discontinuation or dose reduction with the loss of benefits on the kidney. Patiromer is a novel therapy for HK treatment and may enable patients to remain on their RAASi regimen. This study aimed to assess the cost-effectiveness of patiromer from a Swedish healthcare perspective. METHODS: A Marko

Distinct brain structure and behavior related to ADHD and conduct disorder traits.

Attention-Deficit/Hyperactivity Disorder (ADHD) and conduct disorder (CD) exemplify top-down dysregulation conditions that show a large comorbidity and shared genetics. At the same time, they entail two different types of symptomology involving mainly non-emotional or emotional dysregulation. Few studies have tried to separate the specific biology underlying these two dimensions. It has also been suggested that both types of conditions consist of extreme cases in the general population where the symptoms are widely distributed. Here we test whether brain structure is specifically associated to

Renal cortical deterioration in children with spinal dysraphism: analysis of risk factors.

BACKGROUND/OBJECTIVE: Because hydronephrosis and reflux are reversible, we believe cortical loss represents true renal deterioration in children with spinal dysraphism. Our goal was to better define risk factors for cortical loss. METHODS: After institutional review board approval, we reviewed the medical records of 272 children with spinal dysraphism. The following factors were evaluated: age, sex, renal and bladder imaging, urodynamic parameters, medications, catheterization program, continence, infections, and surgical history. Renal cortical loss was defined by scarring or a differential

Antepartum or postpartum isoniazid treatment of latent tuberculosis infection.

OBJECTIVE: To compare health outcomes and costs of different strategies for treatment of latent tuberculosis infection in pregnancy. METHODS: Using a Markov decision-analysis model, the following three strategies were evaluated for treatment of latent tuberculosis infection in pregnancy, defined as positive tuberculin skin reaction of 10 mm or greater and negative chest radiograph: no treatment, antepartum isoniazid administration, in which women were given 300 mg of isoniazid with pyridoxine beginning at 20 weeks' gestation for 6 months; and postpartum isoniazid, in which women were given is

The Information Void in Asymptomatic Chronic Disease: A Digital Health Framework for Understanding Social Media Health Information Seeking in Young Adults.

Nearly 1 in 4 young adults has a chronic condition, yet many feel well despite their diagnosis. Asymptomatic conditions such as prediabetes and hypertension create a unique vulnerability to digital health misinformation, particularly on platforms where inaccurate content is prevalent. Conventional clinical responses, which often just warn patients about online misinformation, fail to address the underlying drivers of this behavior. This viewpoint proposes a novel disease characteristic-based vulnerability framework to understand this challenge, grounded in established behavioral science theori

Perturbations in risk/reward decision making and frontal cortical catecholamine regulation induced by mild traumatic brain injury.

Mild traumatic brain injury (mTBI) disrupts cognitive processes that influence risk taking behavior. Little is known regarding the effects of repetitive mild injury (rmTBI) or whether these outcomes are sex specific. Risk/reward decision making is mediated by the prefrontal cortex (PFC), which is densely innervated by catecholaminergic fibers. Aberrant PFC catecholamine activity has been documented following TBI and may underlie TBI-induced risky behavior. The present study characterized the effects of rmTBI on risk/reward decision making behavior and catecholamine transmitter regulatory prote

Monteggia fracture associated with ipsilateral intercondylar distal humeral fracture with posterior interosseous nerve palsy: case report, medico-legal implications, and methodological assessment analysis.

BACKGROUND: The Monteggia fracture, defined as a fracture of the proximal third of the ulnar shaft associated with an anterior or posterior dislocation of the proximal radial epiphysis, is a serious injury accounting for 0.7% of all elbow fractures and dislocations in adults. For adult patients, good results can only be obtained through early diagnosis and adequate surgical treatment. Monteggia fracture-dislocations associated with distal humeral fracture are extremely rare injuries in adults and there are few cases described in the literature. Medico-legal implications arising from such condi

Late boosting phenomenon in TST conversion among health care workers.

BACKGROUND: Available information is insufficient to guide determination of whether tuberculin skin test (TST) conversions of health care workers (HCWs) within 2 years of two-step testing are related to occupational exposures or to other causes, including late boosting. AIMS: To describe the epidemiologic factors of TST conversion in HCWs, comparing early TST conversion (≤2 years after two-step testing) with late conversion to possibly distinguish late boosting phenomenon from occupational TST conversion. METHODS: Retrospective analysis of a database of TSTs of HCWs from 1 January 1998, thro

COMPULSIVE SEXUAL BEHAVIOR DISORDER AND IMPULSIVITY

Abstract Background Compulsive sexual behavior disorder (CSBD) is characterized by extensive sexual behavior and unsuccessful efforts to control excessive sexual behavior. It is a pathological behavior that has compulsive, cognitive and emotional consequences. There is a controversy among researchers and clinicians whether to recognize sex addiction or compulsive sexual disorder and how to classify it. Although hyper-sexuality was proposed for the DSM-V (American Psychiatric Association, 2013), it was not included due to a lack of research into diagnostic criteria for compulsive sexual behavio

Serotonergic dysfunction in patients with impulse control disorders in Parkinson's disease.

Impulse control disorders (ICDs) are frequent and particularly distressing neuropsychiatric symptoms in patients with Parkinson's disease, which are related to impaired behavioural inhibition. Multiple PET imaging studies indicate that striatal dopaminergic abnormalities contribute to hyperdopaminergic functioning in Parkinson's disease patients with ICD (PDICD+) and to the dysregulation of the limbic fronto-striatal networks, which are critical for reward-related decision impulsivity. However, the serotonergic system is central to response inhibition and plays a critical role in neuropsychiat

[Impulsivity as an immediate factor between addictive disorders and Attention Deficit-Hyperactivity Disorder].

Impulsivity is a behavior in everyday life of mentally healthy individuals. But in some cases the degree of impulsivity begins to create negative effects on one's life and acquires pathological connotation. Two clinical entities in which can be observed a high degree of impulsivity is Attention Deficit Hyperactivity Disorder (ADHD) and Addictive disorders. In these disorders, both impulsive act and impulsive choice are observed. According to some researchers, genetic factors are involved in the control of impulsivity. The impulsive choice, in the sense of delay discounting, may be an intermedi

Cost-effectiveness of early surgical intervention for asymptomatic cervical spinal cord compression:a Markov model analysis.

BACKGROUND CONTEXT: Asymptomatic cervical spinal cord compression (ACSCC) is common in aging populations and can progress to degenerative cervical myelopathy (DCM). The management of ACSCC remains controversial, with uncertainty about whether early prophylactic surgery provides sufficient benefit to justify its risks and costs. PURPOSE: To evaluate the cost-effectiveness of early anterior cervical discectomy and fusion (ACDF) compared to observation in patients with ACSCC and to identify the progression rate threshold at which early surgery becomes economically favorable. STUDY DESIGN/SETTIN

Associations Between Polygenic Scores for Cognitive and Non-cognitive Factors of Educational Attainment and Measures of Behavior, Psychopathology, and Neuroimaging in the Adolescent Brain Cognitive Development Study.

BACKGROUND: Both cognitive and non-cognitive (e.g., traits like curiosity) factors are critical for social and emotional functioning and independently predict educational attainment. These factors are heritable and genetically correlated with a range of health-relevant traits and behaviors in adulthood (e.g., risk-taking, psychopathology). However, whether these associations are present during adolescence, and to what extent these relationships diverge, could have implications for adolescent health and well-being. METHODS: Using data from 5,517 youth of European ancestry from the ongoing Adol

Differences in attentional bias to smoking-related, affective, and sensation-seeking cues between smokers and non-smokers: an eye-tracking study.

RATIONALE: One of the behavioural features of tobacco use disorder is the presence of attentional bias (AB) to smoking-related stimuli. However, much of the research investigating these associations have been limited to the use of reaction-based indices. OBJECTIVES: We aimed to investigate differences in AB to smoking, affective, and sensation-seeking cues in smokers and non-smokers using novel, free-viewing, eye-tracking technology. Secondary aims included investigating impulsivity-by-group interaction effects on AB to sensation-seeking cues. METHODS: Participants were either otherwise-heal

Cost-Effectiveness of First-Line Versus Second-Line Use of Daratumumab in Older, Transplant-Ineligible Patients With Multiple Myeloma.

PurposeThe MAIA trial found that addition of daratumumab to lenalidomide and dexamethasone (DRd) significantly prolonged progression-free survival in transplant-ineligible patients with newly diagnosed multiple myeloma, compared with lenalidomide and dexamethasone alone (Rd). However, daratumumab is a costly treatment and is administered indefinitely until disease progression. Therefore, it is unclear whether it is cost-effective to use daratumumab in the first-line setting compared with reserving its use until later lines of therapy.MethodsWe created a Markov model to compare healthcare costs

Effects of Two Preemptive Post-transplant Desensitization Regimens Upon Renal Allograft Survival and DSA Elaboration.

We used a simple point-based algorithm to identify patients who might benefit from desensitization because of their higher risk of antibody-mediated chronic rejection and graft failure. Points were assigned to known but easily determined risk factors (panel reactive antibody, flow crossmatch, delayed graft function) and calculated immediately after deceased donor kidney transplantation. Point totals were used to identify: 1) which patients would receive desensitization; and, 2) which regimen each patient would receive. This standardized approached resulted in improved overall graft survival in
