Retrospective analysis of the surgically treated temporal lobe arteriovenous malformations with focus on the visual field defects and epilepsy.

BACKGROUND: Authors investigate the surgical outcomes of the temporal lobe arteriovenous malformation (AVM) with focus on the visual field deficit and seizure. METHODS: Between 1981 and 2004, we experienced 294 cases of intracranial AVMs. Among the 294 cases, 45 (15.3%) were located in the temporal lobe. Twenty-six of the 45 cases underwent microsurgical excisions of the AVMs. RESULTS: The male-female ratio of 26 surgically treated temporal lobe AVMs was 15:11. The mean age was 34.2 years, ranging from 7 to 63 years. The sites of lesion were classified as polar in 1, dorsal in 2, laterobasal

Voluntary running wheel activity reduces seizure burden and affords neuroprotection in a mouse model of acquired epilepsy.

OBJECTIVE: Physical exercise may improve neurological deficits and neuronal damage after acute brain injuries and decrease established seizures. We investigated whether voluntary running wheel (RW) activity affects epileptogenesis in a mouse model of acquired epilepsy compared to sedentary mice. METHODS: Epilepsy was induced by intra-amygdala kainate causing status epilepticus (SE) in adult male mice. Sham mice were implanted with electrodes and injected with saline, and matched to experimental mice. In the RW-1 protocol, SE mice were trained to run for 5 weeks before SE induction and for 6 w

Effect of stereo-EEG versus subdural EEG on functional and seizure outcome in pediatric and adult epilepsy surgery: A 21-year single-center experience.

OBJECTIVE: (1) To compare use, benefits, and complications of stereo-EEG (SEEG) and subdural EEG (SD) in presurgical epilepsy candidates. (2) To evaluate the effectiveness of both methods in delineation of the epileptogenic zone (EZ) and guiding tailored resective surgery. METHODS: We included patients with SEEG or SD evaluations in children and adults at the Bethel Epilepsy Centre, Germany, between 2000 and 2020. We retrospectively assessed epilepsy related parameters, main indication of iEEG (intracranial EEG, for identification of the EZ and functional mapping), complications, seizure, and

EEG Ictal Power Dynamics, Function-Structure Associations, and Epilepsy Surgical Outcomes.

BACKGROUND AND OBJECTIVES: Postoperative seizure control in drug-resistant temporal lobe epilepsy (TLE) remains variable, and the causes for this variability are not well understood. One contributing factor could be the extensive spread of synchronized ictal activity across networks. Our study used novel quantifiable assessments from intracranial EEG (iEEG) to test this hypothesis and investigated how the spread of seizures is determined by underlying structural network topological properties. METHODS: We evaluated iEEG data from 157 seizures in 27 patients with TLE: 100 seizures from 17 pati

Human amygdala response to unisensory and multisensory emotion input: No evidence for superadditivity from intracranial recordings.

The amygdala is crucially implicated in processing emotional information from various sensory modalities. However, there is dearth of knowledge concerning the integration and relative time-course of its responses across different channels, i.e., for auditory, visual, and audiovisual input. Functional neuroimaging data in humans point to a possible role of this region in the multimodal integration of emotional signals, but direct evidence for anatomical and temporal overlap of unisensory and multisensory-evoked responses in amygdala is still lacking. We recorded event-related potentials (ERPs)

Electrocorticographic high-gamma modulation with passive listening paradigm for pediatric extraoperative language mapping.

OBJECTIVE: This prospective study compared the topography of high-gamma modulation (HGM) during a story-listening task requiring negligible patient cooperation, with the conventional electrical stimulation mapping (ESM) using a picture-naming task, for presurgical language localization in pediatric drug-resistant epilepsy. METHODS: Patients undergoing extraoperative monitoring with subdural electrodes were included. Electrocorticographic signals were recorded during quiet baseline and a story-listening task. The likelihood of 70- to 150-Hz power modulation during the listening task relative t

Beneficial Outcome of Urethane Treatment Following Status Epilepticus in a Rat Organophosphorus Toxicity Model.

The efficacy of benzodiazepines to terminate electrographic status epilepticus (SE) declines the longer a patient is in SE. Therefore, alternative methods for ensuring complete block of SE and refractory SE are necessary. We compared the ability of diazepam and a subanesthetic dose of urethane to terminate prolonged SE and mitigate subsequent pathologies. Adult Sprague Dawley rats were injected with diisopropylfluorophosphate (DFP) to induce SE. Rats were administered diazepam (10 mg/kg, ip) or urethane (0.8 g/kg, s.c.) 1 h after DFP-induced SE and compared to rats that experienced uninterrupt

Bidirectional Control of Generalized Epilepsy Networks via Rapid Real-Time Switching of Firing Mode.

Thalamic relay neurons have well-characterized dual firing modes: bursting and tonic spiking. Studies in brain slices have led to a model in which rhythmic synchronized spiking (phasic firing) in a population of relay neurons leads to hyper-synchronous oscillatory cortico-thalamo-cortical rhythms that result in absence seizures. This model suggests that blocking thalamocortical phasic firing would treat absence seizures. However, recent in vivo studies in anesthetized animals have questioned this simple model. Here we resolve this issue by developing a real-time, mode-switching approach to dri

Perception of brain-computer interface implantation surgery for motor, sensory, and autonomic restoration in spinal cord injury and stroke.

INTRODUCTION: Stroke and spinal cord injury (SCI) can profoundly diminish quality of life across physical and psychosocial domains, with motor and sensory deficits often persisting despite current therapies. Invasive brain-computer interface (BCI) systems, particularly electrocorticography (ECoG)-based approaches, offer a potential means to bypass neural injury and restore function. To inform development and deployment, it is critical to understand candidate users' willingness to adopt such technology and how that willingness relates to their functional goals and rehabilitation priorities. Target-specific modulation of cortical narrowband gamma by basal ganglia nuclei: a case report.

INTRODUCTION: Narrow-band gamma (NBG) is an oscillatory activity in the gamma band (60-90 Hz) that has been observed in the subthalamic nucleus (STN), the motor cortex, and, more recently, the globus pallidus internus (GPi) of Parkinson's disease (PD) patients, often in the on-medication state. Therapeutic DBS has been shown to modulate NBG, often entraining cortical and subcortical NBG to half the stimulation frequency, often referred to as finely tuned gamma (FTG). However, the differential effect of STN, GPi, and GPe DBS on cortical NBG has never been explored in the same patient. METHOD:

Predictors of Postoperative Epilepsy/Seizures in Patients Readmitted After Meningioma Resection.

Background: Epilepsy/seizures in meningioma patients may occur pre- or postoperatively, causing significant morbidity and impaired quality of life. Surgical excision is considered a standard management with variable rates of epilepsy/seizure resolution reported after surgery. Employing a national database, we examined the pre- and postoperative incidences of epilepsy/seizures and risk factors associated with postoperative epilepsy/seizures in patients readmitted within 30 days and/or 90 days following meningioma resection. Methods: The 2010-2014 Nationwide Readmissions Database was analyzed. C

Carotid body stimulation as a potential intervention in sudden death in epilepsy.

OBJECTIVE: To investigate carotid body (CB) mechanisms related to sudden death during seizure. Ictal activation of oxygen-conserving reflexes (OCRs) can trigger fatal cardiorespiratory collapse in seizing rats, which presents like human sudden unexpected death in epilepsy (SUDEP). The CB is strongly implicated in OCR pathways; we hypothesize that modulating CB activity will provide insight into these mechanisms of death. METHODS: Long-Evans rats were anesthetized with urethane. Recordings included: electrocorticography, electrocardiography, respiration via nasal thermocouple, and blood pressu

Epilepsy surgery in children with tuberous sclerosis complex: presurgical evaluation and outcome.

PURPOSE: Children with tuberous sclerosis complex (TSC) benefit from excisional surgery if seizures can be localized to a single tuber. We evaluated the role of noninvasive studies to localize the epileptogenic tuber/region (ET/R) and the outcome of focal resection. METHODS: We identified 21 children with TSC, ages 3 months to 15 years (mean 4.8 years). All had video-(electroencephalogram) EEG and magnetic resonance imaging (MRI) scans, and 18 also had ictal single photon emission-computed tomography (SPECT) studies. An ET/R was localized in 17 patients. Thirteen patients underwent resection

Complex technical methodologies and their applications in the surgery of intracranial meningiomas.

As neurosurgery moves into the twenty-first century, improved visualization/localization techniques, neuromonitoring, and advanced instrumentation will become standards of care for all intracranial procedures. This article has focused on current available technologies that can be used to facilitate operations on intracranial meningiomas. Preoperative anatomic localization with MR imaging, CT, MR angiography, and angiography are standard techniques. Preoperative functional assessments with MR imaging, magnetic source imaging, PET, and functional MR imaging are crucial to recognize and preserve

Relative entropy as a biomarker for vagus nerve stimulation response in epilepsy: Insights from intracerebral electroencephalography.

OBJECTIVE: This study aims to analyze differences in pre-implantation invasive electroencephalographic (IEEG) data between vagus nerve stimulation (VNS) responders (>50% seizure reduction) and VNS nonresponders (<50% seizure reduction). METHODS: We retrospectively identified 19 patients with drug-resistant focal epilepsy who underwent IEEG followed by VNS implantation instead of resective brain surgery. Resting-state IEEG recordings (30 min) were selected for all patients. Three biomarkers were analyzed in the IEEG data: (1) the number of interictal epileptiform discharges (IEDs), (2) the num

Propagation mapping using iterative independent component analysis for seizure onset zone localization in temporal lobe epilepsy.

BACKGROUND: Epilepsy affects approximately 70 million people worldwide, with a third of them being drug-resistant and requiring surgical intervention. Accurate localization of the seizure onset zone (SOZ) is crucial for effective surgery but remains challenging. NEW METHOD: We proposed a method using iterative independent component analysis (ICA) to map seizure propagation of drug-resistant temporal lobe epilepsy (TLE). For each assumed seizure origin, ICA was applied to the remaining contacts to identify propagation components, with the highest correlating component being the propagating sig

Topographical map of subjective states evoked by focal seizures and electrical stimulation of the human insula.

OBJECTIVE: The goal of this study was to investigate how the topographical map of human subjective experiences induced by intracranial electrical stimulation (iES) compares to the map of subjective auras experienced by patients during seizures involving the same cortical areas (here, the insular cortex). METHODS: We recruited 14 patients with insular epilepsies confirmed with intracranial electroencephalography in the United States (N = 7) and France (N = 7). We identified insular regions involved early in seizures (i.e., presumed seizure-onset zones [SOZs]), and documented the auras reported
